CongenitalInfectiousNeoplasticNeck nodes
Two ways to think of possible causes:
Slide3Ear-related, congenital
Accessory auricle
Slide4Ear-related, congenital
Pre-auricular sinus / abscess
Slide5Ear-related
Pre-auricular sinus / abscess,
infected
Slide6New growths/Neoplasia
Squamous Cell Carcinoma
Basal Cell Carcinoma
Slide7Chondrodermatitis
nodularis helicis
Skin/dermatological conditions
Slide8Parotid gland lumps, infection
Parotitis
Acute viral infection, Mumps
Slide9Parotid gland lumps, infection
Parotid abscess
a). Bacterial,
secondary to dehydration, sialadenitis, sialectasis etc.
B). Mycobacterial, atypical mycobacteria, children, bluish red swelling
Slide10Parotid gland tumours, benign
Commonest
Pleomorphic adenoma
Slide11Parotid gland tumours, benign
Warthin’s tumour (adenolymphoma)
can be bilateral
Slide12Parotid gland tumours, malignant
Malignant parotid lumps
E.g. Adenocystic Carcinoma
May present with facial palsy
Slide13Abscesses around ear
Related to lymph nodes
(some nodes are inside parotid gland)
Slide14Cervical (neck) lymph nodes groups
Slide15Abscesses around ear, dental and oral causes
Check teeth and oral cavity
with buccal swelling
Slide16Other sites of neck swellings
Slide17Always check sites that drain into the lymph node systems of the neck, for infection, tumour etc.
Scalp
Oropharynx
Nasopharynx
Oral cavity
Tonsils…etc
Quinsy
Slide18Ear related swellings due to infection of pinna, ear canal, middle ear or mastoid bone
Lumps over the mastoid:
Lymph node; common, small mobile node, no pain.
Nodes or abscess from acute otitis externa.
Pain, hot, red, fluctuant, most tender at mastoid tip.Abscess over mastoid due to CSOM, sub-periosteal or subcutaneous. Fluctuant, tender, hot, most tender behind the top of the pinna.
Abscess below mastoid tip, from CSOM, going down into neck, Bezold abscess.Patient is ill, swelling, pain, fever.
