I have inspected the equipment, and I authorize and grant permission to the Lake Mills Area School District/Lake Mills Recreation Department to charge my credit/debit card in the eve nt of damage due to neglect, misuse, or loss. I have read the damage charges listed on the back of this rental agreement. I understand that rental is charged on a 24 hour basis. If snowshoes are returned late, I will be charged an additional day for each late day, charged upon return. I fully release and discharge the Lake Mills Area School District/Recreation Department and its officers, agents and employees from any and all claims and damages, including claims and damages arising from injuries, death, or property damage, which may arise out of, or occur in connect with my use of Lake Mills Schools/Recreation Department snowshoes, except for those resulting from the intentional or reckless acts of the Lake Mills School s / Recreation Department or its offic ers, agents and employees . A $25 (refundable) security deposit is required at time of rental. Refer to damage repair table for individual costs. If snowshoes are returned damaged, repairs fees will be charged to the credit card number held for deposit. If snowshoes are not returned, the renter will be charged for a replacement pair of snowshoes. Sizes Youth size snowshoes are 19 inches long and recommended for a weight of 40 - 90 pou nds.
