1. Having reviewed the TALC Process Chart School/ Agency identify a Young Person (YP) they want to discuss at the TALC Multi-agency Operational Group MeetingSchool/ Agency to inform the YP that:A referral has been made The outcomes and purpose support once decidedConsent is obtained to engageAll service providers are required to obtain consent from YP and/ or Family/Carer to engage the YP for the provision of the serviceConsent gained (Progress to next step) School/ Agency to inform Parent(s)/ Carer(s) of referral to TALC Operational Group , explaining how this operates and their rights.All information is discussed at the TALC operational group meeting to identify services to assist with the YP needs. Action plan is created identifying the responsibilities of each party. Outcomes are communicated to the school to inform the YP and/or Family/ CarerSchool/ Agency complete TALC involvement referral and submit to TALC@gloucestershire.gov.uk via encrypted email 5 days prior to TALC meeting. Triage will determine if the referral is appropriate for TALC and inform the school of the outcome Referral is shared with TALC membership to review records held to begin data gathering and assessing the needs of the YP. Appendix 1 a - Process for Information Sharing ahead of a Team Around the Locality Cluster (TALC) Operational Group MeetingPlease Note: All request for information such as FOI/ SARs or data subject rights request will be dealt with by GCC. School/ Agency identify the names of the YP they want to discuss at the TALC Prior to any referral being submitted school/ agency should discuss a referral with their Early Help or Outcome co-ordinator, review the TALC Process Chart and Ready Reckoner to ensure that support can not be provided by individual agencies or existing multi-agency groups that will meet needs in a timely manner.2. School/ Agency complete request for TALC involvement referral The TALC Involvement referral should be fully completed in conjunction with the TALC Flow Chart and Ready Reckoner which will provide information about services and agencies that should have been considered prior to a referral, ensuring this is adhered to will reduce the likelihood that a referral will be returned or signposted to an alternative intervention. Once a referral has been completed it should be submitted 5 days in advance of the relevant TALC Operational Group Meeting to TALC@gloucestershire.gov.uk by encrypted email. 2. School/ Agency to inform the YP of the referral and the process that will be followed at the TALC operational Group - If the Young person is aged 13 years or older, and is considered to be Gillick competent, it is important that the process of the TALC Operational Group is shared with them and they are kept informed about any information shared with other agencies. Best Practice would dictate that Gloucestershire’s Your Information leaflet is provided and you talk them through the TALC Families Directory webpages for future reference . Where a child is not Gillick competent then the school should inform at least one of the YP’s Parents or guardian using the Notification of referral. 3. Sharing Family details ahead of the TALC Operational Group Meeting Where Triage identifies a referral as appropriate for the TALC operational group the full referral will be shared with ‘core members’* of the operational group meeting as well as the Name, address and date of birth being supplied to ‘Additional Members’* by secure email 72 hours prior to the TALC meeting to allow individual agencies the time to research their databases for information that is held on family members. These details will be distributed by the dedicated TALC Business Support Officer..*Additional Members - Where Triage identifies there is likely** to be an active role in providing support to the YP or family the full referral will be shared with that agency **a triage professional makes an informed judgement based on what is recorded within the referral or is held on GCC case management systems (including Liquid Logic, Early Help Module, Capita, CAMHS Records and other internal agency records)4. Sharing the action plan with all the agencies once the TALC operational group have identified a need to intervene to support the child and young person. The action plan (once agreed by the TALC chair) will be circulated along with the TALC referral (if not already shared) to those agencies who have been identified as being able to support the YP or wider family members to ensure that actions can be implemented and tracked in the interests of the child/young person.
