FARMERS ALLIANCE COMPANIES Guidelines for handling AUTO GLASS LOSSES Claims Check Authority is granted by the Marketing Representative for each territory. A packet of Claims Checks, including instructions, is then distributed to the agency. Claims Check Authority is limited to $1,000 ($3,000 for Advantage Agents). ABOUT CLAIMS Rev. 01-2015 CONTACT INFO: Fax: 844-322-5246 loss-notice@fami.com Mail: PO Box 1401 McPherson, KS 67460 OPTIONS FOR AUTO GLASS LOSSES OPTION 1: Insured chooses repair shop, arranges own appointment. (Agent may assist with this.) Glass is repaired, or replaced if necessary. If a replacement, the insured pays the deductible to the repair shop. If a repair, the deductible is waived. Payment options: Shop can send or fax the bill to the agency. Agency can pay the bill with a Claims Check, if they have Claims Check Authority (see instructions provided with Claims Checks) OR Shop can send or fax the bill to the agency. Agency can forward the bill to Dena Miller at Farmers Alliance (see address below) OR Shop can send or fax the bill Claims Of�ce, attn: Dena Miller (see the red box). OPTION 2: Insured or agent contacts the Quest Glass Network, 866.856.6636 Network staff arranges appointment with the insured and the shop. Glass is repaired, or replaced if necessary. If a replacement, the insured pays the deductible to the repair shop. If a repair, the deductible is waived. Payment process: The glass network bills Farmers Alliance, and after they receive our payment they pay the shop. NOTE: We continue to waive the “Other than Collision” deductible on chip repairs. IMPORTANT NOTE: We do not require ACORD Loss Notices to be completed for sure the Policy Number and date of loss are on the bill/invoice instructions in this document.
