1. AntihypertensivesHypertension occurs when resting systolic blood pressure exceeds 130 or diastolic blood pressure exceeds 80 mm Hg.Even stage 1 hypertension (<140/90) has been reported to increase the risk of end-organ damage

MECHANISMS FOR CONTROLLINGBLOOD PRESSUREArterial blood pressure is directly proportional to cardiac output and peripheral vascular resistanceCardiac output and peripheral resistance are controlled mainly by two overlapping mechanisms: the sympathetic baroreflexes and the renin–angiotensin–aldosterone system Most antihypertensive drugs lower blood pressure by reducing cardiac output and/or decreasing peripheral resistance.

Baroreceptors and the sympathetic nervous systemA fall in blood pressure causes pressure-sensitive neurons (baroreceptors in the aortic arch and carotid sinuses) to send fewer impulses to cardiovascular centers in the spinal cord. This prompts a reflex response of increased sympathetic and decreased parasympatheticoutput to the heart and vasculature, resulting in vasoconstriction and increased cardiac output.

5. Renin converts angiotensinogen to angiotensin I, which is converted in turn to angiotensin II, in the presence of angiotensin converting enzyme (ACE). Angiotensin II is a potent circulating vasoconstrictor, constricting both arterioles and veins, resulting in an increase in blood pressure. Angiotensin II stimulates aldosterone secretion, leading to increased renal sodium reabsorption and increased blood volume, which contribute to a further increase in blood pressure

Treatment StrategiesGoal for treating HT is: a systolic blood pressure of less than 130 mm Hg and a diastolic BP of less than 80 mm Hg Current recommendations are to initiate therapy with first line (ABCD): ACE inhibitors or angiotensin receptor blocker (ARB), β blockers, calcium channel blocker, and diuretic (thiazide)

Compensation for reducing BPespecially in vasodilators Reflex tachycardia & retention of salt & water Unfortunately, the baroreceptor reflex and the renin response are reset to maintain the higher BP. As a result, they respond to a therapeutically lowered BP with compensatory homeostatic responses, which may be significant. These compensatory responses can be counteracted with β blockers and diuretics or angiotensin antagonists.
