3. Spinal anesthesia or - subarachnoid block - intrathecal anesthesia - spinal analgesia Definition is a form of regional anesthesia involving injection of a local anesthetic into subarachnoid space ( August Bier 1898)
4. Anatomy Vertebrae (spine) Cervical - 7 Thoracic - 12 Lumbar - 5 Sacral - 5 (เชื่อมติดกัน) Coccygeal - 4 (เชื่อมติดกัน) Total 33

8. Meningesแบ่งเป็น1. Dura mater - นอกสุด, หนาสุด, สิ้นสุด ~ S2 (adult) - ประกอบด้วย fibrous tissue - เหนือ epidural space (Batson’s plexus, segmental artery, fat, lymphatics, spinal nerve root) - ใต้ subdural space
9. 2. Arachnoid mater - ใต้ subarachnoid space ซึ่งมี CSF 3. Pia mater - บางมาก, แนบชิดกับ spinal cord
10. Spinal cord ~ 60% สิ้นสุดที่ขอบล่าง L1 (30% ที่ T12, 10% ที่ L3) terminal branch cauda equina นิยมทำ SA or LP ต่ำกว่า L1
11. Spinal nerves ออกทาง intervertebral foramen เพื่อไป supply ตาม dermatomes มีทั้งหมด 31 คู่ - C1-8 - T1-12 - L1-5 - S1-5 - coccyx 1 คู่

14. Blood supply ด้านหน้า - ant. spinal artery (single) Adamkiewicz ระดับ T8-L2 (2/3) - เสี่ยงต่อ ant. cord ischemia no collateral flow ด้านหลัง - post. spinal artery (pair) collateral flow มาก (1/3) ขอบด้านข้าง - vasocorona (เชื่อม ant. กับ post.spinal artery)

18. CSF สร้างจาก choroid plexus ปริมาณ 100-150 ml ( ½ in brain, ½ in subarachnoid space) specific gravity at 37º C = 1.003-1.008
19. Indication “Neuraxial block is indicated when surgical procedure can be accomplished with a sensory level of anesthesia that does not produce adverse patient outcome”นิยม - Lower extremity, hip : femur, tibia, foot - Lower abdomen : hernia, C/S - Perineum : anus, rectum, TUR-P
20. Contraindication Absolute - Pt.refusal Other - hypovolemia (relative) - increased intracranial pressure - coagulopathy - infection - septicemia - valvular heart stenosis - preexisting neurological deficits - uncooperative Pt. - severe spinal deformity - prior back surgery

Differential Nerve block เส้นประสาทแต่ละชนิดถูกยับยั้งโดยยาชาเฉพาะที่ได้ยากง่ายแตกต่างกัน, fiber แต่ละชนิด sensitive ต่อยาชาไม่เหมือนกัน โดยขึ้นกับ - size - myelinated or unmyelinatedโดยเส้นประสาทที่มีขนาดเล็กและเป็นชนิด myelinated จะถูกยับยั้งกระแสประสาทได้ง่ายที่สุด B fiber C fiber Aδ Aß A alpha พบว่าระดับ sympathetic blockade 2 dermatomes sensory blockade 2 dermatomes motor blockade

Drug Local anesthetics - ออกฤทธิ์สกัดกั้นการนำกระแสประสาทโดยจับกับ receptor ของ Na channel - ส่วนของยาที่มีประจุบวกจับกับ membrane receptor ที่มีประจุ ไฟฟ้าตรงข้ามที่ผนังด้านในของ Na channel Na channel ปิด Na ไหลผ่าน axoplasm ไม่ได้ ไม่เกิด deporalizaton “Surface charge , specific receptor theory”
25. Drug Local anesthetics ยาชาที่มี pKa ใกล้เคียงกับ pH ของ tissue (7.4) จะออกฤทธิ์ ได้เร็ว เช่น lidocaine (pKa 7.8) จะออกฤทธิ์เร็วกว่า bupivacaine (pKa 8.1) เนื่องจากมี non-ionized form ที่จะซึมผ่าน epineurium และ nerve membrane เยอะกว่า หากมีการเปลี่ยนแปลง tissue pH จะมีผลต่อการออกฤทธิ์ของยาชา
26. Devided into 3 groups 1. Ester group - cocaine (1st), procaine, chloroprocaine, tetracaine - side effect มาก 2. Amide group - lidocaine, mepivacaine, prilocaine, bupivacaine, etidocaine, ropivacaine และ dibucaine 3. Saxitoxin, tetrodotoxin - สกัดจากปลาปักเป้า - ออกฤทธิ์นานหลายวัน - ปัจจุบันยังอยู่ในขั้นทดลอง
27. Bupivacaine(marcaine) Hypobaric แบ่งเป็น 3 ชนิด ตาม baricity Isobaric Hyperbaric นิยมใช้ 1. 0.5% Isobaric bupivacaine ออกฤทธิ์นาน 3-3.5 hr, ไม่สามารถปรับระดับการชาได้ 2. 0.5% hyperbaric bupivacaine ออกฤทธิ์นาน 2-2.5 hr, ควบคุมระดับการชาได้ดีกว่า

37. 2. Respiratory system block midthoracic level มีผลน้อยต่อ pulmonary Fn ในผู้ป่วยที่ไม่มี preexisting lung disease- high block abdominal+intercostal m. paralysis ระวังใน Pt.severe pulmonary disease (ควรต่ำกว่า T7) 3. GI system - parasym. เด่น sphincter relaxation , peristalsis
38. 4. GU system bladder atony - autonomic blockade (S2-4) bladder sphinter tone Urinary retention 5. Metabolic & endocrine - ลด stress response
39. Factors influencing block height Major - dose - site of injection - baricity - posture Other - age - height - CSF - anatomy of spine - intraabdominal pressure - technique of injection
40. Complications Acute onset - hypotension - high-total spinal block - cardiac arrest - anaphylaxis - nausea & vomiting - paresthesia - shivering
41. Late onset - backache - urinary retention - PDPH (age, gender, needle size, bevel, pregnancy, No. of dural puncture) - spinal, epidural hematoma - meningitis, arachnoiditis - cauda equina syndrome
