slide8. Who Are You? All About Me – My Identity Book For this health project:
you will be creating your very own identity book.
you can use an actual blank notebook that you have or you can make your book from copy paper, construction paper, etc. You can also complete it on the computer and print it.
You have an opportunity to think about who you are, what makes you unique and why you act in certain ways.
you will be given a sufficient amount of time to complete this project however, make sure to stick to the due dates so you don’t have to rush to put your identity book together.
3 completed pages are due Wed. April 8; additional 3 pages are due Wed, April 29; final 3 pages are due May 6 (Cover, 6 required pages, 2 optional pages = 9 pages); You may complete pages in any order you choose.<br>
slide9. Guidelines for Your Identity Book:
 
You Identity book must include a cover page, and a total of at least 8 sections
(6 required sections and 2 from the optional sections.) Each section is explained
on the following slides, as well on the attached worksheet).
Dedicate at least 1 full page for each section. You may type or handwrite your book and it should be neat and attractive. Place the sections of your Identity book in any order you like. Include a Table of Contents in the front of your identity book (to identify the sections) and the attached rubric in the back ( for grading) .
You must include some form of pictures, illustrations, or drawings throughout your book. Make sure to add captions to explain your pictures.
To keep you on track, you must complete 3 completed sections by next Wednesday April 8, 2020. In the attachment packet, there is a checklist to help guide you toward completion.
Most of all, have fun, be proud and enjoy getting to know who you are!<br>
slide10. My Identity Book- 7 Required Sections Cover Page – should be attractive and neat
My Character- This section should contain one or several stories about ways you demonstrated honesty, compassion, accountability, respect and /or fairness Include how you felt after you demonstrated good characters
My Body- This section should include facts about you physically. Include your age and a description of you r appearance. Include things you do to keep yourself healthy
My Feelings- This section should include examples of how you’ve expressed happy and troublesome feelings in healthy ways.
My Relationships- This section should contain pictures drawings and descriptions of people who are important in your life; family, friends, neighbors, coaches etc.
My Future- This section should contain a few paragraphs about what you think your life will be like when you are 35. Include information about your job, family and activities you hope to participate in. You can also include your beliefs and values.
My Biographer- You will need to ask another person in your life to complete this section. This person will be your biographer. ( A biographer is a person who writes about the life of someone else). Your biographer can be a classmate, a relative, or anyone you choose. Your biographer should be someone that has known you for a long time. Be sure to give your biographer a deadline and enough time to do a good job. Make sure to include the name of the biographer and their relationship to you. Ask them to write about: - your strengths and great qualities - your personality - ways you are working to improve things

slide11. My Identity Book- Optional Sections You must include at least 2 Optional sections My Creativity- This section should show the creative side of you. Creativity comes in many forms. You can draw a picture, take photos or write a story, poem or song. You can write a scene for a play, make a collage, arrange flowers or draw plans for a house; anything that brings out your creative side
People I admire- This section should contain information about people yiu admire and why
My Private Self- Think about a way to tell about the you nobody knows. Your dreams feelings, worries ,thoughts, and hopes Write what you think about when your alone and not busy or waiting for someone, or listening to music. Tell what you do when you are alone and happy depressed annoyed and energetic;
Laughter- For this section, explain the kinds of things that make you laugh. Tell about a time you laughed uncontrollably.
My Past- This section should contain information about your past, you can include stories and pictures. You can write about things that made you scared, sad, excited, and/or happy.
Other- Create your own This section could include something else you want to share about yourself that wasn’t mentioned in the above sections.<br>
