GHS/GES collaborate to ensure group meetings are planned & Conducted
Document and Report on Number of SMART school group meetings planned and Conducted
Document and Report on Number of SMART school group meetings planned and held
Take PICTURES of activities done and share Document and Report on Number of fruits Day planned and carried out
Document and Report on Number of inspection days Planned and carried out
Document and Report on Number of health education sessions planed and carried out
Document and Report on Number of general cleaning sessions planned and carried out
Document and Report on Number of physical activity sessions planned and carried out
Report on activity planned and number carried out by end of term Nutrition Friendly School Activities (MUST DO)<br>

Recording in the GIFTS Class Register
Collates data from all Classes in the School to form the School Termly Reports
Submit report to the District GES through the SISO SISO Receives/Collects all Termly Reports from all implementing schools within his Circuit and Submit to the D-SHEP D-SHEP Receives/Collects all Termly Report within his district
All reports are given to the DHD for entry into the reporting template
DHD enters the reports and Send to D-SHEP as the district report
D-SHEP receive report and shares with the District Director R-SHEP & RNO receives from all district
Shared with their Respective Regional Directors
Forward to National SCHOOL CIRCUIT DISTRICT REGION National Receives from All Regions to Form National Report NATIONAL Head teachers
