1. WEEK 4Central Line Associated Blood Stream Infection (CLABSI) – a bloodstream infection in the presence of a central line that is not secondary to an infection at another body site ASSESSMENTKNOW YOUR LINES – know your patient’s central line type and indicationDiscuss catheter need daily and remove when no longer medically necessaryCarefully assess catheter site Remove central line if site is erythematous or tenderROOT CAUSE ANALYSIS (RCA)Change dressings: every 7 days for CHG transparent dressings, every 48 hours for gauze dressings, or when saturated or peelingKeep dressing clean, dry and intactUse alcohol caps on all infusible ports and access portsUse red end caps for all male luer connectorsScrub access ports with alcohol x 15 seconds immediately prior to each subsequent accessUtilize pulsatile flushing procedureDaily CHG bath or showerCollect blood from peripheral sites for culture. Do not routinely draw blood cultures from central lines - If absolutely necessary, Physician order is requiredHD catheters can ONLY be accessed by specifically trained staff for HD and CVVHCentral Lines placed emergently with compromised technique need to be changed within 24 hrsMAINTENANCE BEST PRACTICESPerformed on all major hospital-acquired infections like CLABSIGOALS: identify gaps in our current processes in a non-punitive environmentformulate plans of action to prevent future infections in similar patientsYOUR INSIGHT INTO THE PATIENT’S CASE IS INVALUABLE.
