Step A: Conducting a Search Request 1. Log in with your individual username and password via providers.bcidaho.com at the top of the web page 2. Hover over Authorizations/Notification

When the JIVA Dashboard page loads select Search Request at the top left. a. Enter required search criteria: Members Last Name, First Name and Date of Birth b.
