*RED OUTLINE INDICATES A REQUIRED FIELD WITH THE EXCEPTION OF “DEPARTMENT HEAD SIGNATURE”* - Fill out all sections in ‘Part I – User Information” as well as ‘Part II – Department Head Verification’ AS STATEDNotes: - Use the drop down menu to select the “Clinic Location” at which you’ll be working. **If your clinic does not appear on the drop down list, select anything, and then note the correct clinic you will be working at in the notes section on the sharepoint request form- DoD Email will be your official @mail.mil email address - In THIS BOX (Optional) , you can input the name of a coworker whose clinic application menus you wish to mirror. If none, or unknown, leave blank. - In THIS SECTION, please check all the boxes that apply to you.- MAKE SURE TO SIGN THE DOCUMENT ONCE COMPLETED- Once this form is completed, save it to your (P:) Drive in the following format:LAST.FIRST DDMMMYYYY CIS Of these applications , check the boxes of the clinical systems you’ll need access to. Ask your supervisor if you are unsure. DMIX Joint legacy viewer training Pt. 2 - Once at the JKO Homepage, select “ Login using my CAC/VA PIV ” - Make sure you select the “DoD Email” certificate when prompted - You’ll be brought to the main DHA or JKO homepage. From here, select the “ Course Catalog ” Tab 5
DMIX Joint legacy viewer training Pt. 3 Once in Course Catalog follow these steps. - In the “ Title Key Word Box ”, Type ‘ DMIX ’, the hit ‘Enter’ on the keyboard. - The training you want is called “DMIX: Joint Legacy Viewer (JLV) New User Training (25 min)”- Once you find the right training, click “Enroll”
DMIX Joint legacy viewer training Pt. 3 - Once you have enrolled in the JLV course, Click on the “ My Training ” tab. - Scroll down to the bottom of the page until you see your “ Elective Training ” - Click “ Launch ” to start training . - Upon completion of training , you will be presented with a certificate, and you will need to save it to your (P:) Drive - When viewing the certificate, click the “ Save ” icon at the bottom middle of the screen, and save the training certificate to your (P:) Drive with the following format: LAST.FIRST DDMMMYYYY JLV 7
At this stage, your (P:) Drive should consist of a completed SAAR-N Form, a completed CIS Request Form, a HIPPA Training certificate, a Cyber Awareness training certificate, and a Joint Legacy Viewer Training certificate. **It is important that your supervisor, and department head’s names and emails, as well as your own email, are filled out on the SAAR-N form and CIS Request before continuing.** 8
