Blank 3-5 Answer Sheets Only the Last Name , First Name and Student ID # are required for scanning. Students fill out the Test Code , Year and Form #s during the TA script.

Don’t Lose Answer Sheets! Students should never insert answer sheets inside the ACPs! Count your test booklets and answer sheets again when signing them back in.

Test Administrator Script (Pages 16-23)Read the script verbatim as written.Practice reading the script in advance.Do not paraphrase or disregard the script.Use the English or Spanish version based on the test version given to the student.

Make-up Testing (Pages 24-25) The make-up testing window is January 9-23, 2017 . K-2 make-ups must be entered online by January 23. Headers must be created and filled out correctly for grades 3-5 make-up tests! Testing accommodations must be administered during make-ups. Only students with excused absences can test by appointment after January 24 .
