for the previous year.
UIC-24: Class I Quarterly Report
Automated form must be submitted quarterly within 30 days following the end of the reporting quarter.
UIC-33/34: Class III Quarterly Report
Automated form must be submitted quarterly within 15 days following the end of the reporting quarter.
