I understand an d agree that this application may be rejected or revoked at any time, either before or after the start of play in the Pro - Am, without liability or obligation and that, in such event, I shall not be eligible to caddie or continue to caddie therein, as the c ase may be. I understand and agree that I am not eligible to participate in the Pro - Am prizes. I understand and agree that I must wear soft spike golf shoes during the Pro - Am. I further understand and agree that if, in the judgment of the PGA TOUR Executi ve Director, or his designee, adverse weather conditions or other occurrence or conditions beyond the control of PGA TOUR renders commencement or continuance of the Pro - Am impractical then, in any of such events, the Pro - Am may be delayed, postponed, short ened or canceled, whichever may be deemed appropriate by the PGA TOUR Executive Director, or his designee; and, PGA TOUR shall pay me an applicable pro rata portion of the fee per round, if any, based on actual competition completed at the time of such pos tponement, rescheduling or cancellation. If I am unable to participate in the postponed or rescheduled Pro - Am, I shall not be entitled to payment of any fee hereunder. In consideration of PGA TOUR services in cosponsoring the Pro - Am, I hereby grant(s) and assign(s) to PGA TOUR, without limitation, my individual rights in and to (a) any form of television, radio motion picture, photographic, electronic, Internet, “Internet Applications” (as defined below) and/or any other form of audio or video presentation which is now known or hereafter may exist, including, without limitation, free over - the - air broadcast, cablecast, DBS, MDS, satellite radio, broadband, wireless, pay television, subscription television and on - demand platforms; (b) statistical and scoring information in any media; and (c) all other ancillary media and other rights with respect to my participation in the Tournament, Pro - Am or any other golf event conducted in conjunction with the Tournament or Pro - Am ( e.g. , Clinics, long - drive contests), or any portion thereof. “Internet Applications” shall mean: (1) the presentation of the Tournament and/or Pro - Am in any medium that permits the viewer/listener/user to interact with or manipulate the presentation or access other relevant information during th e presentation and/or (2) video games. I agree to refrain from any action which will interfere with PGA TOUR’s ownership of the rights hereunder granted and assigned to PGA TOUR or with any authorized use thereof. I also agree to abide by the PGA TOUR Tour nament Regulations currently in effect and by the Rules of Golf of the United States Golf Association, subject to any modifications thereof approved by PG A TOUR. By participating in the Pro - Am or entering onto the grounds of the Tournament, I voluntarily a ssume all risk and danger incidental to the game of golf, whether occurring prior to, during or subsequent to the actual event, including the risk of being injured by a golf shot or by other spectators or players, and I release PGA TOUR, the Tournament, So noma Golf Club, television broadcasters, Tournament sponsors, vendors and volunteers, participating players and all agents thereof from any and all liabilities arising out of such cases. All caddies MUST be at the Cochise Course by 7 :0 0 am . For driving directions , the address is: 10333 Rockaway Hills, Scottsdale, AZ 85262  The minimum age requirement is 16 years of age.  Caddie experience is not required but basic golf kn owledge is a must.  Each caddie is an independent contractor engaged by PGA TOUR. Nothing herein shall be construed to make either party the partner, employee, joint venturer or agent of the other party, and neither party shall have the right or power to obligate or bind the other in any manner whatsoever. Caddies shall not be deemed nor hold themselves out as PGA TOUR’s agents, contractors, employees or servants. PGA TOUR shall not withhold from any of the consideration paid hereunder, except when require d by law , any amount for federal income taxes, social security, medicare, or any other legal deductions for caddies, payment of which shall be the caddie’s responsibility. Further, PGA TOUR shall not make premium payments or contributions for any workers' compensation or unemployment compensation benefits for any caddies, payment of which shall be the caddies’ responsibility.  Gratuity is at the discretion of the amateur and is strongly encouraged by the Charles Schwab Cup Championship.  Free park ing and sh uttle service provided. A parking pass will be provided upon confirmation of your application. Shuttle service to the course will begin at 6: 3 0 am.  Lunch coupon to be redeemed at any concession stand. N OTE : Please be sure to eat during the round as food options are very limited following the competition.  Required attire: Khaki pants/ Bermuda shorts & a white collared shirt.  Towels: Caddies must bring their own towels. In the event that you arrive on - site without your towel you may borrow one from us u pon payment of a $10 refundable deposit .  Caddying for the Pro - Am does NOT include eligibility for a free round of golf at Desert Mountain Club .  You must comply with all safety requirements associated with your position and support related PGA TOUR polic ies and procedures. You will not be eligible to caddie in the future and/or may be dismissed during the Pro - Am for (but not limited to) the following: o Consumption/being under the influence of alcohol or drugs while on duty o Theft o Insubordination, abuse or mistreatment of players, fans, tournament staff, visitors, guests or volunteers o Failure to abide by tournament polices and procedures o Failure to complete your shift(s) o Allowing another person to use your tourn ament credential o Seeking autographs outside of the designated autograph area o Entering restricted areas without proper credentials o Not being punctual and/or leaving your position unattended  PGA TOUR reserves the right to accept or reject applications in it s sole discretion.  Caddies may not use the tournament name or logo, website content, written agreement or any material and publication that carry the tournament marks in any way without the written permissions of PGA TOUR.
