Or go to guardiananytime.com and click on “Find a Provider” for the Guardian PPO. Or call Guardian, 800-848-4567. Choose a dentist and make an appointment. You don’t need an individual ID card. Simply provide the Group Plan number, 381084, to your dental office at the first visit.
Members enrolled in the Delta Dental HMO simply call the dentist assigned by Delta to make an appointment.
Slide3Davis VisionGo to davisvision.com and use the “Find a Doctor” feature, or call 800-999-5431. Identify yourself as a PSC-CUNY Welfare Fund member or dependent and Davis Vision member. Provide the office with your name, SS#, and the name and date of birth of any covered member/dependent needing services.
The provider’s office will verify your eligibility. Make an appointment.

HearUSA know that you are a member of the PSC-CUNY Welfare Fund. You will be given the names of three participating HearUSA practitioners in your area and the nearest store.
Slide5CVS/CaremarkYour CVS/Caremark Prescription Card was mailed to you. If not, call CVS at 866-209-6177. You can print a temporary card from the Welfare Fund website, psccunywf.orgUse your CVS Card at drugstores such as Duane Reade, Rite Aid, Walgreen, Target, most independent drugstores, as well as CVS.
To fill prescriptions for diabetic medications, use your basic health insurance card. Injectable and chemotherapy drugs are covered by the NYC PICA plan—see your campus benefits officer for information or call 212-306-7464.
