Improved Fetal Heart Rate Monitor Sakina Girnary, Karoline Linde, Jennifer Gilbertson, Ida Neuman, Frode Liland, Helge MyklebustEXECUTIVE SUMMARY Moyo is an innovative and low-cost fetal heart rate (FHR) monitor that can accurately detect FHR within 5 seconds. It can be used intermittently as well as continuously. Moyo will alarm if abnormal FHR is detected, allowing healthcare workers to provide timely obstetric decisions. With this FHR monitor, the capacity, competence and con�dence of healthcare workers in providing labour monitoring care can be improved and many more lives can be saved at birth.An estimated 1.2 million intrapartum stillbirths occur annually, of which 98% take place in low and middle-income countries. In addition, one million infants who survive birth asphyxia develop neurocognitive problems and learning difficulties due to birth related complications. FHR is a key indicator during labour and birth to identify early a fetus at risk. Detection of abnormal FHR should trigger an appropriate and timely obstetric response to prevent unecessary injury and deathChallenges with current fetal heart rate devices In resource-rich environments, CTG machines are the gold standard of care, however they are prohibitively expensive and reduce the delivering mother’s mobility. Affordable devices, such as the Pinard and simple, hand-held dopplers are time-consuming and not feasible for continued monitoring. They require very experienced users to determine an accurate fetal position to detect an adequate FHR signal.FHR monitoring according to guidelines is often not done due to lack of staff or equipment.Provides quick and accurate fetal heart rate measurementsWith a 9-crystal sensor, similar to those used in CTGs, Moyo is able to accurately detect and display FHR over a larger surface areaAllows providers to determine FHR within 5 seconds as opposed to 1-3 minutes, which is the case with a Pinard or single-crystal Doppler.Increases competence & con�dence by reducing skill-related barriers As Moyo is able to detect FHR over a wide range of area on the abdomen, the device does not require healthcare workers to palpate the abdomen to find the back of the fetus in order to get a strong FHR signal during intermittent monitoring. Provides a colour coded number display that correlates to the fetal heart rate. A good FHR is represented in green, whereas an abnormal FHR is represented in red. Key features not common in other FHR monitoring devicesCan monitor continuously using a display that can be carried on the mother’s arm or around her neck, allowing the mother to be mobile. Provides a histogram of the FHR trend in the last 30 minutes. Uses dry electrodes to simultaneously measure the mother’s heart rate, in order to avoid confusing the maternal heart rate with that of the fetal heart rate. Triggers an audio-visual alarm if FHR is detected outside of the normal range.Affordable, portable and durableHas a low purchase price as compared to other continuous monitoring devices. Lightweight and portable.Has internal rechargeble batteries with a battery run time of over 10 hours. Saving Lives at Birth-grant to support �eld test in TanzaniaMoyo FHR Monitor, as a part of a larger research and development project ’Safer Births’, won a prestigious Saving Lives at Birth grant. The grant supported a field study in Tanzania to validate the fetal heat rate monitor. Study results showed a positive change in practice for FHR monitoring and newborn outcomes after introduction of Moyo, including increased documentation and increased frequency of FHR monitoring rate.
