Gender

Merriam Webster Dictionary:Gender: the state of being male or female

Sex: the state of being male or female Slide6

Gender Identity: a person's internal, personal sense of being a man or a woman … For transgender people, the sex they were assigned at birth and their own internal gender identity do not match.

Transgender: term used to describe people whose gender identity differs from the sex the doctor marked on their birth certificate

Sacred Scripture:

Gen 1:27 “God created mankind in his image; in the image of God he created them; ‘male and female

’ - he created them.”Gen 2:22-24

The Lord God then built the rib that he had taken from the man into a woman. When he brought her to the man, the man said, “This one, at last, is bone of my bones and flesh of my flesh; This one shall be called ‘woman,’ for out of man this one has been taken. This is why a man leaves his father and mother and clings to his wife, and the two of them become one body.”

Gen 3:20

The man gave his wife the name “Eve,” because she was the mother of all the living.

Chapters 2 and 3 in Genesis point to importance of the unitive and procreative dimensions of marriage, which is only possible with a man and a woman.

Theology of the Body-St.John Paul II

The Root of the Ethos of the Human Body“In the mystery of creation, man and woman

were in a particular way “given” to one another by the Creator, not only in the dimension of that first human pair and of that first communion of persons, but in the whole perspective of the existence of the human race and of the human family.

The fundamental fact of this existence of man in every stage of his history is that God “created them male and female”; in fact, he always creates them in this way, and they are always such. The understanding of the fundamental meanings contained in the very mystery of creation

, such as the spousal meaning of the body (and of the fundamental conditioning of this meaning

), is important and indispensable for knowing who man is and who he out to be, and therefore how he should shape his own activity. It is something essential and important for the future of human ethos.”

(General Audience of February 13, 1980. Theology of Original Innocence)Slide12

Sexual Desire/ Identity

Developmentally complexEach person desires and longs for deep intimacy with other human beings. There is a great deal of plasticity in developmentVery young: prepubescent often—who notice distinct sex roles in the culture and, exploring how they fit in, begin imitating the opposite sex.Slide13

Secular feminists in science and pseudo-science, literature, politics readily adopted Money’s work.1975: Marta Llamas: Theory of 5 sexes in regional conference and argued a person’s identity as a man or woman is simply socially constructed.

A Follow-Up Study of Girls with GID

22/25 (88%) girls did not report distress about their gender identity at follow up.

:

70-80% of children tracked without medical or surgical treatment at

Vanderbilt University and London’s Portman

Clinic spontaneously lost transgender feelings

. 25% still has persisting feelings

(see WSJ article with Paul McHugh)Slide18

What are the next steps?

“Watchful waiting” “I don't think the goal of therapy is to make a child feel bad about who they are. It's helping kids understand themselves better and what might be causing them to develop what I call a "fantasy solution," that being the other sex will make them happy… There are so many kids we see in the clinic where the desire to change their sex goes away, for whatever reason. So there are many kids we'd see in adolescence and you'd never have known gender identity was an issue for them in childhood. Not all, but a good segment. The so-called natural history of GID in children is complicated, and to make dogmatic assertions is overly simplistic.”-Dr.K.Zucker

“A population that is susceptible to suggestion from ‘everything is normal’ sex education.—they become persuaded that seeking a drastic physical change will banish their psycho-social problems. ‘Diversity’ counselors in schools are more like cult leaders encouraging youth to distance self from families.” –Dr.McHugh

Support of transition

I would say that I think that there is a subgroup of children who, if we listen to them carefully, will tell us, "I know who I am. And if you let me be who I am, I will be a healthy person. And if you try to bend my twig" — which is what I think Zucker does — "then I will be a repressed, suppressed, depressed person who will learn to do what other people expect of me and I'll hide who I really am.“ –Dr.D.Ehrensaft

What does the Literature say?

Accurate scientific information is important for adequately informing our public discourse, policy debates, and individual lives related to these issues.Be clear on: 1. What we do know 2. What we still do not know 3. What conclusions can be drawn by the current body of researchSlide21

Studies supporting Sex Reassignment

Pfafflin F., Junge A. (1992) Sex Reassignment: 30 Years of International Follow-up Studies after SRS: A Comprehensive Review, 1961-1991 This study looked at 70 previous studies and reviews on outcomes following sex reassignment surgery. These included 2000 individuals from 1961 to 1991. This doesn’t take into account individuals who transition without surgery.

About 70% of MTF individuals were satisfied and 90% of FTM individuals. This study shows little or no regrets possibly due to surgical advances

.

Hess J. et al (2014) Satisfaction with Male-to-Female Gender Reassignment Surgery Results of a Retrospective Analysis

.

Deutsches Ärzteblatt International | Dtsch Arztebl Int 2014; 111: 795–801

High rates of subjective satisfaction and the surgical outcomes indicate that gender reassignment surgery is beneficial

(*Caution about interpreting these results 46.9% filled out response)Slide22

Psychiatric Comorbidity

180 Transsexual youth who underwent SRS, 106 female to male and 74 male to female2 to 3 fold increased risk of psych disorders including

Depression

Anxiety disorder

Suicide ideation

Suicide attempts

Self harm without lethal intent

(e.g. cutting/burning)Increased inpatient and outpatient treatment compared to controls. Slide23

“There is huge uncertainty over whether changing someone’s sex is a good or a bad thing.”

Birmingham University’s Aggressive Research Intelligence Facility assessed 100 follow up studies on Post-operative transsexuals. They concluded:

Most research was poorly designed. There was no evaluation of whether other treatments such as long-term counseling might help transsexuals or whether their gender confusion might lessen over time. Many patients were lost to follow up.

Complications of hormone/genital surgery have not been thoroughly investigated: e.g. deep vein thrombosis, incontinence.Slide24

“No one can change their sex.” ‘Sex change’ is biologically impossible … People who undergo sex-reassignment surgery do not change from men to women or vice versa. Rather, they become feminized men or masculinized women. Claiming that this is civil-rights matter and encouraging surgical intervention is in reality to collaborate with and promote a mental disorder.”

He equates the disordered assumptions to Anorexia Nervosa

and Body Dysmorphic Disorder.

Dr.Paul McHugh Slide25

Dr. McHugh Continued

Solipsism: The view or theory that the self is all that can be known to exist

“For the transgendered, this argument holds that one’s feeling of “gender” is a conscious, subjective sense that, being in

one’s mind, cannot be questioned by others.

The individual often seeks not just society’s

tolerance of this “personal truth” but affirmation

of it. Here rests the support for “transgender equality”, the demands for government payment for medical and surgical treatments, and for access to all sex-based public roles and privileges.”

Recommend Article : “Surgical sex” from First Things

http://www.firstthings.com/article/2004/11/surgical-sex

Slide26

Meyers and Reters Archives of Gen Psych 1979:

“Sex reassignment surgery confers no objective advantage in terms of social rehabilitation, although it remains subjectively satisfying to those who have rigorously pursued a trial period and who have undergone it.”

Compared to before surgery, there were no significant improvements after surgery in any of the measured criteria of life adjustment, including observable criteria of adaptation, family relationships, and ability to adapt at important life intervals

Another interesting (more recent) article

out of Johns Hopkins

Slide27

2011 study Karolinska Institute in Sweden

“Persons with transsexualism, after sex reassignment, have considerably higher risks for mortality, suicidal behaviour, and psychiatric morbidity than the general population. Our findings suggest that sex reassignment, although alleviating gender dysphoria, may not suffice as treatment for transsexualism, and should inspire improved psychiatric and somatic care after sex reassignment for this patient group.”

20 fold suicide mortality

Depression and low quality of

life after surgery

Increased risk of

psychiatric hospitalizationSlide28

What are Psychiatrists/Health Professionals to do?

Paul McHugh: “Psychiatrists obviously must challenge the solipsistic concept that what is in the mind cannot be questioned. Disorders of consciousness….represent psychiatry’s domain; declaring them off-limits would eliminate the field. (he cites the solipsists of the “recovered memory” craze accusing parents of sex abuse, and how this was deemed unquestionable

) -WSJ article.Decline to take the pt’s claims at face value, while still recognizing/not dismissing their distress.Slide29
Slide30

Human Being(William May from moral theology no. 8): “To be a human being, therefore, is to be a being of incalculable worth, of irreplaceable value, of precious dignity

. To be a human being, moreover is to be a bodily being, living flesh. This means that to be human is to be a sexed being, for in the beginning ‘God created man in his own image. . . male and female he created them’ (Genesis 1:27). . . And every living human body, every human person

, whatever its age or sex or race or condition is a being of precious worth, irreplaceable and non-substitutable, a being that ought to be wanted.”Slide31

What the Catholic Church is clear on:

It does not accept that people can change their sex.Baptismal records cannot be altered.People claiming to be the other sex cannot marry.Women claiming to be men cannot be ordained.

Direct Sterilization is unjustified (Interpretation of The Principle of Totality).Slide32

Bishop Robert Barron: Love, tolerance, and the making of distinctions

The moral and spiritual context for transgenderism is, from a classically Christian standpoint, problematicNo place for hatred:

To love, Thomas Aquinas says, is to will

the good of the other

. Consequently,

hatred

is not primarily a feeling but

desiring evil for another. Some of the muddiest water surrounds the concepts of love/hate and tolerance/intolerance.Slide33

Tolerance and Intolerance

Current day society “Tolerance:” “Acceptance” “celebration”. Actual Tolerance: “

In point of fact, the term implies the willingness to countenance a view or activity that one does not agree with.”Slide34

“For Biblical people, human love is never a disembodied reality. Furthermore,

love—which is an act of the will—does not hover above the body, but rather expresses itself through the body and according to the intelligibility of the body.

To set the two in opposition or to maintain that an inner act is somehow more important or comprehensive than the body is to walk the gnostic road—which is just as dangerous a path as it was in the time of St. Irenaeus.” –Bishop BarronSlide35

What can we do as Catholic/Christian healthcare providers?

Remember God is loving each person where they are and calling them to holinessWelcome and accompany persons spirituallyFoster full human development

Affirm the person but not all their choicesCharitably challenge false assumptions about truth and reality

Pope Francis

in regard to care for homosexual persons. “In life, God accompanies persons…and we must accompany them, starting from their situation. It is necessary to accompany them with mercy.”

CCC 1997, 2358

he or she “must be accepted with respect, compassion, and sensitivity. Every sign of unjust discrimination in their regard should be avoided. These persons are called to fulfill God’s will in their lives.”

CMA November Linacre Quarterly issues:

“when the research is solid, men and women of faith, whether entrusted with medical care or pastoral care, need not fear to confront the culture as they strive to

speak the truth in love.” Slide36

“Parents, family members, educators, politicians and clergy have a moral responsibility to be familiar with medical science and to provide prudent counsel to youth that will protect and not harm them.”- Dr. R. Fitzgibbons

Some closing remarks:Slide37

Pope Benedict XVI

“The Chief Rabbi of France, Gilles Bernheim, has shown in a very detailed and profoundly moving study that the attack we are currently experiencing on the true structure of the family, made up of father, mother, and child, goes much deeper. While up to now we regarded a false understanding of the nature of human freedom as one cause of the crisis of the family, it is now becoming clear that the very notion of being – of what being human really means – is being called into question. He quotes the famous saying of Simone de Beauvoir: “one is not born a woman, one becomes so” (on ne naît pas femme, on le devient). These words lay the foundation for what is put forward today under the term “gender” as a new philosophy of sexuality. According to this philosophy, sex is no longer a given element of nature, that man has to accept and personally make sense of: it is a social role that we choose for ourselves, while in the past it was chosen for us by society. The profound falsehood of this theory and of the anthropological revolution contained within it is obvious. Slide38

Pope Benedict XVI continued

People dispute the idea that they have a nature, given by their bodily identity, that serves as a defining element of the human being. They deny their nature and decide that it is not something previously given to them, but that they make it for themselves. According to the biblical creation account, being created by God as male and female pertains to the essence of the human creature. This duality is an essential aspect of what being human is all about, as ordained by God. This very duality as something previously given is what is now disputed. The words of the creation account: “male and female he created them” (Gen 1:27) no longer apply. No, what applies now is this: it was not God who created them male and female – hitherto society did this, now we decide for ourselves. Man and woman as created realities, as the nature of the human being, no longer exist. Man calls his nature into question. From now on he is merely spirit and will. The manipulation of nature, which we deplore today where our environment is concerned, now becomes man’s fundamental choice where he himself is concerned. Slide39

Pope Benedict XVI continued

From now on there is only the abstract human being, who chooses for himself what his nature is to be. Man and woman in their created state as complementary versions of what it means to be human are disputed. But if there is no pre-ordained duality of man and woman in creation, then neither is the family any longer a reality established by creation. Likewise, the child has lost the place he had occupied hitherto and the dignity pertaining to him. Bernheim shows that now, perforce, from being a subject of rights, the child has become an object to which people have a right and which they have a right to obtain.

When the freedom to be creative becomes the freedom to create oneself, then necessarily the Maker himself is denied and ultimately man too is stripped of his dignity as a creature of God, as the image of God at the core of his being.

The defense of the family is about man himself.

And it becomes clear that when God is denied, human dignity also disappears.

Whoever defends God is defending man.”Slide40

Questions For Discussion:

- How do we give witness to truth, love, and mercy in terms of addressing gender reality and gender ideology in the workplace?- As Catholic providers, how do we respond when presented with a patient who is undergoing transition?

Can one really interiorly and inwardly “experience” what it is like to be another gender? (e.g. a woman’s distinctive procreative capacity/maternity, perceptual, affective, or cognitive experiences that differ between men/women).

What assumptions about human identity and personhood are underlying the view that sex change surgery is morally justifiable? Do these views align with the Church’s understanding of the human person and God’s plan for human nature? What is human nature?

Is sex change surgery psychiatrically justified to resolve a patient’s discomfort? Will it truly take care of the comorbidities? What came first—the chicken or the egg?

What is wrong with mutilating a healthy human body according to the tradition of good medical practice? Am I prudently using my talents as a healthcare provider?
