Preventing Influenza

Mandatory Vaccinations for all Healthcare WorkersSlide2

IntroductionNearly 60 million people are employed in the health care profession. These people are placed at an increased risk of infection.

Health care workers (HCW) have a duty to keep their patients and themselves safe.

Many organizations are

mandating their HCW to receive their vaccinations in hopes of increasing the compliance rates.Slide3

Learner ObjectivesImportance of HCW receiving the annual influenza vaccinationSupport towards mandatory influenza vaccinations for HCWSupport towards not receiving annual influenza vaccinationRecommendations for increased influenza vaccine

complianceSlide4

Influenza Vaccination: Why it’s ImportantInfluenza causes more than 200,000 hospitalizations annuallyInfluenza causes 36,000 deaths annually1.1 – 6.6 million fewer cases & 7,700 – 79,000 fewer hospitalizations PER SEASON from annual vaccinations.

About 7.2 million illnesses, 3.1 million attended illnesses, & 90,000 hospitalizations from the flu have been prevented.Slide5

The Healthcare Environment Influenza is often a hospital-acquired infection (HAI)HAI’s are one of the top causes of death in the U.S.U.S. is ranked last in quality outcomes for developed countriesOne survey across 37 states found 65% of hospitals

did not have recommended policies in place to prevent most common HAI’s.

HWC vaccination rate averages around 75%Healthy People 2020 goal: > 90%

Nurses can make an impact!Slide6

Reasons against mandatory influenza vaccinations.Violates right to refuse unwanted medical interventionsInfringes upon informed consentDoes not allow HCW to make decision about own healthEthical:Personal:

Do not believe in vaccine’s effectivenessBelieve the flu virus is not as serious as it is depicted

Feel they are in good health and not at high riskBelieve the vaccine is inconvenient

Fear of needles or side effects from vaccineSlide7

Reasons for mandatory influenza vaccinationsDuty to protect the patient.Decreased chance of harm to the patient, staff, and self.Sets an example for the public regarding disease prevention.

To remain healthy in order to provide optimal care.

Decreased absenteeism, which increases staff morale and patient safety.

People who are hospitalized are more susceptible to obtaining hospital acquired influenza.Slide8

ImplicationsCauses 3-5 million severe cases of illness per year250,00-500,000 deaths per year worldwide36,000 deaths per year in the U.S.

16-60% of deaths in the U.S. each year are from hospital acquired influenza.

A standard HCW comes into contact with an average of 13 patients per day.

A person can be contagious 1-4 days before symptoms arise, placing multiple patients at risk.Slide9

ConsequencesRhode Island: non-compliant employees have to wear a mask through entire shift or pay $100 each time it is found off.

Maine: Unpaid leave until they receive vaccine, or until flu season

is over.

Arkansas: Hospitals are penalized if they have low compliance rates.Slide10

CDC recommendations for health care facilities: 1) Educate nurses and all HCW’s 2) Offer vaccine annually to all HCW’s 3) Provide vaccine at HCW work site at no cost 4) Non-compliant HCW’s should sign declination form 5) Monitor vaccine coverage/declination regularly

6) Vaccination coverage as patient safety qualitySlide11

Recommendations for nurses 1) GET THE INFLUENZA VACCINE each year 2) Be leaders in the adoption of best practices 3) Use evidence-based research to guide best practices 4) Nurse leaders-awareness building strategies Continuing educationStaff meetingsFlyers/brochuresIn-services“Peer champions”

Hold staff accountable for competenciesSlide12

ConclusionShow How Much You Care, Get Your Flu Vaccine! Staggering statistics support the need for HCW influenza vaccination.

HCW’s can either spread the flu or reduce the rate of flu within the healthcare environment. There are multiple reasons to support mandatory flu vaccination, yet some arguments exist in opposition to this proposed regulation.

Recommendations are based on evidenced-based practices, as well as best practice statistics.Slide13
