Métodos E xame fundoscópico de uma paciente apresentando uma lesão única e elevada no olho esquerdo OE Fig01 Resultados F eminino 72 anos com baixa acuidade visual no OE há cerca de dois meses Paciente refere tratamento de tuberculose pulmonar tbc há cerca de quinze anos e internação devido a pneumonia comunitária há quatro meses Ao exame oftalmológico apresentou uma acuidade visual de 2020 e 20200 Snellen no olho direito OD e no OE respectivamente À biomicroscopia apresentava uma catarata nuclear simétrica e sem alterações corneanas O exame fundoscópico do OE mostrou uma lesão extensa amelanótica elevada com mobilização de pigmento em periferia e hemorragia préretiniana Ecografia ocular do OE revelou lesão coroideana regular nodular de 53mm X 110mm com média a alta reflexibilidade acústica e ausência de ângulo kappa bem como de escavação coroideana Fig02 Aventada a possibilidade de metástase coroideana devido às características ecográficas Exame de escarro para pesquisa de tbc foi negativo Tomografia computadorizada de tórax mostrou nódulos pulmonares irregulares difusos A biópsia pulmonar apresentou características de fragmentos com necrose caseosa características da tbc além de fragmentos com características histológicas de adenocarcinoma papilar bem diferenciado do pulmão Paciente iniciou quimioterapia e radioterapia externa no entanto evoluiu com hemorragia vítrea no OE glaucoma de células fantasmas e perda da visão no OE Aguarda evisceração ocular à esquerda.
