Prior to Taking A Leave

Managers/Supervisors may direct their employees requesting a leave of absence for Medical/Pregnancy, Workers’ Compensation, Family Medical Leave, Intermittent and/or Military Leave to the Leave of Absence (LOA) website: www.hr.uclahealth.organd contact our LOA Team at (310) 794-0078 and/or LOATeam@mednet.ucla.eduFor questions and/or once employee has the medical certification completed by their healthcare provider and has completed the online LOAN

Prior to Leave of Absence

u Will Find OnEmployees with a planned medical procedure date and/or expectant mothers should provide managers/supervisors and HR with at least a 30-day advance notice ahead of their anticipated leave via a completed medical certificate/FML.  In case of an emergency or an unplanned leave, employee has 15 days from the date they notify department of their leave request to provide a medical certification to their department and to the LOA team.u Will Find On The Leave Website The Le

Extending a Leave and Returning to Work

Leave ExtensionsEmployee is required to provide managers/supervisors and their assigned LOA analyst with an updated note from their healthcare provider before their original return to work (RTW) date with extension dates.  Returning to Work After a LeaveEmployee must provide managers/supervisors and LOA analyst with a medical clearance to return to work.  Returning to Work with Medical Restrictions After a LeaveManagers/supervisors must confer with our disability manager and review the medical restrictions in order to determine whether restrictions can be accommodated.

Inform and Plan

Provides

employee with their rights and

responsibilities, expectations while on leave, and discusses any contract guidelines/policies that may applyMaps out leave using appropriate accruals (sick, vacation, holiday hours), disability period, leave without pay (LWOP) to ensure completion of timesheets while employee is on leave and ensures our Benefits team bills employee for benefit premiums while on LWOPDiscusses parking with employee and instructs them to contact respective parking office in order to retain or cancel parking

3 Track and CoordinateSends completed LOAN to Manager/Supervisor for acknowledgement/approval and timesheet completionEnters all leave phases (leave with pay and without pay) of an employee’s leave into our EDB system for payroll processing and complianceTracks leave and follows up with managers/supervisors to verify RTW status or leave extensions

4 Wrap up and WelcomeAnswers any post-leave questions from both department and employeeReminds employee to review all of their benefits and deductions on their first few paychecks upon after their RTW
