Aims and Objectives of The Old Vicarage The Caldeco tt Foundation exists to provide care education, fostering and support to the children and young people in the greatest need who have faced significant trauma and deprivation. Our primary aim is to provide each child/young person with the necessary tools a nd identified support needs to aid their transition back into foster care, home or a less intense provision. The cycle of assessment, monitoring, evaluation and review will identify the support and provision required to ensure the child/young person is ab le to build on areas of strength and make progress in all areas of need. Provision mapping and multi - disciplinary case conferencing will ensure that all professionals involved with the child/young person can determine the best care/placement plan. The Ca ldecott Foundation aims to empower children and young people in the decision making process . The Old Vicarage is able to offer placements on a planned and an emergency basis and works specifically with children and young people who have a variety of needs . The children and young people will come from a variety of backgrounds, display challenging behaviour and raw emotions which are hard for other people to understand and withstand. Throughout the period of placement a detailed outline of risk, behaviour management strategies and an understanding of the child/young person‟s resilience factors will be developed in order to facilitate transition to an identified placement. From the outset the adults working alongside the child/young person will be observing and reporting on the child/young person‟s daily life . The information gathered and evaluated will contribute to the final assessment report delivered in preparation for the child/young person‟s transitional plans . The assessment model used by The Old Vi carage is based on the Framework for the Assessment of Children in Need and their Families (See Appendix 1) . At The Old Vicarage we believe in children and young people ‟s resilience and the understanding that normal development can take place under very d ifficult conditions. We aim to identify the child/young person‟s strengths, passions, ambitions and dreams and support them to achieve these. During the assessment the children and young people can expect us to provi de a safe, and containing environment with clear and thoughtful boundaries, which are reliable without being rigid. The adults will provide an experience of being cared for , having fun with firm and consistent boundaries without being punitive or persecutory. We aim to help support the chil dren and young people to be able to have their say, take control of their lives and be able to foster a sense of respo nsibility for their decisions and actions. See Appendix 2 for Statement of Overall Aims & Objectives 2. Facilities and Services The Old V icarage is a large home sited in rural Nottinghamshire with extensive grounds, providing ample space for outdoor activities and pursuits. The home has spacious rooms including a large lounge, kitchen and conservatory/dining room and is centrally heated throughout. There are five children/young people‟s bedrooms, two staff bedrooms and two bathrooms upstairs. There are separate bathroom facilities for staff and visitors. 2 The Old Vicarage – Statement of Purpose The Old Vicarage has access to a range of professionals who may deliver indivi dual service to a child/young person but may also work with the staff to ensure a reflective and professional approach to the needs of each child/young person. The aim of The Old Vicarage is to determine the range of need and the approach that will best s upport and provide a caring and safe environment to grow and progress. This integrated approach will ensure holistic thinking and a consistent, clear and evidence based approach to meet the needs of the child/young person. The Old Vicarage works within Ev ery Child Matters (B eing Healthy, Staying Safe, Enjoying and Achieving, Making a Positive Contribution and Achieving Economic Well Being) whilst also thinking about the daily individual needs of each child /young person and their medium and long term plans. The Old Vicarage has its own dedicated classroom in a separate annexe with a Teacher , which is registered as part of Caldecott School. Education will be an on going part of the assessment package. Staff from The Old Vicarage will support each child/you ng person to achieve their potential within the educational context through close working relationships with the class team and ensuring opportunities for education are provided outside of the class. Children/young people shall be supported to engage and attend school, will be encouraged to experience new challenges and learn new skills. Staff in The Old Vicarage work with each child/young person to help them to begin to develop positive relationships; to reinforce positive behaviour and work with negativ e behaviour. This process is mapped and monitored within an integrated system across all key areas of the child‟s or young person‟s life. Each child/young person will have an initial assessment which will draw together the information gathered from Local Authority, Education, Health and any other relevant professionals . The collating of the chronological and topographical issues around each child/young person will identify the area for assessment and support. Staff set out day to day care plans coveri ng the minutiae of the child /young person ‟s day and the broader, longer term plans for the child /young person . The child/young person‟s future is held in mind as the goal to which all staff work. The aims and goals are clearly identified and set out on a dmission and reviewed in line with statutory LAC and Statement Reviews. The model of care employed in The Old Vicarage and across the Caldecott Foundation, enables children/young people to have greater autonomy as to who they choose to help and support t hem and decreases the reliance on one or two people without whom the child /young person feels unable to function. The knowledge and understanding of the child/young person‟s needs is held with the house and school team in the first instance but is quality assured by the multi - disciplinary (integrated service) professionals across the Foundation. Each child will have an initial placement planning meeting which may be attended by the education and therapy team and significant others, including family and ot her professionals, related to the care of the child. This meeting will take place, wherever possible, prior to placement and placement targets and aims of placement will be discussed and agreed . Prior to post admissions four month Looked After Child Revi ews a case conference will review the aims and targets to ensure progress is monitored and new targets set. Local Authority representation will be invited to attend if particular areas that require forward planning prior a LAC or statement review are ident ified. Further case conferences will take place before the LAC review on a six monthly basis. At this conference the following information will be evidenced: 3 The Old Vicarage – Statement of Purpose  Initial targets set/target review  Annual Reviews (This forms part of the continuing baseline se tting)  Termly summary of progress  Behaviour – personal/social  Baseline evidence  Risk Assessments  Weekly summaries  Care Plan (this will incorporate the ICP and the IEP)  Reading and Spelling review  On going Assessment information  Speech and Language/Therapy report (Where applicable)  Clinical/Educational Psychology report (Where applicable)  Other external professional reports  Behaviour checklist, communication audit and life skills audit This information will be used to evidence and inform provision mapping for each child/young person and determine evidenced based outcomes under the Every Child Matters agenda . This will be reviewed and updated on a regular basis. Systems and processes within the cycle of review will be u sed to inform and evidence the R eview documentation. All children/young people will be given the opportunity to participate in learning essential life skills in line with their individual needs and level of development. These will include cooking, shopping and taking responsibility for their own laundry and bedroom cleaning with appropriate levels of support. The placement will be maintained all the while the young person is engaging in the assessment and is participating within the activities and programmes within the Home, and that the expe rience for the child/young person as well as their carers continues to be a positive one. The staff realise that there will be periods of extreme disruption to work through, but if this fails to improve and/or the individual poses an unacceptable danger t o others or indeed their self then a disruption meeting will be called to discuss the contin ued viability of the placement. 3. NG22 0UU See Appendix 3 for the Foundation‟s organisational structure. 4. Qualifications and Experience of the Responsible Individual and Manager Resp onsible Individual . Helen Jones has worked with Looked After Children across social care and education for more than 23 years both in the UK and in America. Helen is a strong advocate for children and believes that all children should be supported to re ach their potential. She has used her knowledge of the law and standards to improve the outcomes for children. Helen began her career at the Caldecott Foundation in 1989 as a Residential Support Worker. She progressed to Senior Residential Support Worke r prior to moving to America to work with Vision Quest where she worked with „troubled youth‟ as part of a wilderness programme. She started out as a Tepee parent and progressed to Quality Assurance and Treatment Director. On her return to the UK, Helen did some voluntary work with adults with learning disabilities before returning to the Caldecott Foundation in September 1995, to work within the school as a support assistant. Helen became a deputy manager in May 1997 and joined the Throughcare Social Wo rk team in August 2000. Helen qualified as a Social Worker in 2004 and completed her training with a six month placement in the Duty and Initial Assessment team in East Sussex. Helen became a Senior Social Worker in April 2007, moving to Quality Assuranc e in April 2010. Helen then took on the role of referral manager and became Head of Care in November 2012. Manager . Sarah Orriss has worked within residential care since 2004, and has previously been a Registered manager in an intensive support home wor king with young people aged 10 - 17 years, where she achieved and sustained an Ofsted „outstanding‟ rating for the home. Sarah has a Foundation Degree in Children and Youth studies and also holds the LMCS (Leadership and Management in Care Services) award. 5. Staffing Arrangements As this is a new service, there will be a phased approach to opening beds for residents and additional staffing will be recruited in line with the phased opening. 5 The Old Vicarage – Statement of Purpose The house will eventually be staffed by One Manager, 2 Deputy Re gistered Managers , four Seniors and eight full time Residential Care Workers or Education care Workers (or part time equivalent). During the night 2 members of staff will sleep in. The children‟s doors are alarmed so if a child comes out of their room b oth staff members are aware and will respond to the child. However, if the on - going assessment evidences that the child no longer needs to have this measure the door will no longer be alarmed. Support will be available throughout the night via the home on call system and all child protection procedures will be observed. There will be a minimum of 2 staff on duty during the school day, s taffing ratio throughout evenings, weekends and holidays will be based on the needs of the children and reviewed in - line with the changing needs of an individual child or of group dynamics. The staffing ratio will be constantly under review and maybe increased from time to time. If it is felt a child would benefit from 2 to 1 support for a short period of time then arrange ments will be made and evidence of this support, the benefit or continuing need would be made available to the local authority to ensure individual child planning. Any shortfalls in staffing levels due to sickness or absence would in the first instance be covered by the existing staff team, and any additional staffing needs would be discussed with the senior management team to ensure that staffing levels met the needs of the home at all times. All staff will receive an in house induction over the first 4 w eeks within The Old Vicarage which helps familiarise them with house routines, structures and boundaries. They are given specified time to read the Policie s, Procedures and Practice file. All staff will attend the Caldecott Foundation induction training w ithin the first 6 months in order to introduce the professional responsibilities of each key area. A two year programme of Core training will ensure that all staff are able to practice in an informed and safe manner. The second year of training will all ow specialist knowledge that will further enhance their individual working practice and career progression. Training available to all staff: - Induction Introduction to the Caldecott Every Child Matters The Role of the ECW Child development Introduction to Education Safeguarding Training Introduction to Attachment Equal Opportunities and Anti - Discriminatory Practi c e Introduction to the therapy department Care Planning Supervision and Performance Management Mandatory Training Anti - Bullying training Dri ver Briefing Basic Fire Awareness Course Emergency First Aid at Work Food Hygiene (Awareness) Health & Safety in the Workplace Training Infection Control 6 The Old Vicarage – Statement of Purpose Manual Handling Internal Medication Course Introduction to Positive Management of Challenging Behavio ur (PRICE) Training Diploma for the Young peoples‟ Work Force Development A wide range of additional training and continual professional development is in place (see Training Handbook). Within the first year all staff will begin a range of trainings whic h covers child care practice, theory and legal aspects of working within a residential setting. The induction is linked to the CDWC Induction Standards. All staff will be qualified to NVQ3 in Health and Social Care (Children and Young People) or working towards their accreditation following successful completion of the probation period. The range of experience will vary dependent on the status of the individual staff member , and the criteria determined through the performance management framework which is competency based. The staff team are mixed gender, the Caldecott Foundation actively promotes the recruitment of both genders and minority groups and individuals are appointed to ensure the overall needs of the children are met in line with the foundatio ns equality and diversity policy. 6. Staff Supervision, Training and Development Each staff member receives a written contract of employment. All staff are afforded the opportunity for regular supervision (see Tri.x section 8.2). This will be undertaken by either the Manager or deputy manager within the home. Each supervisor has oversight of individual children‟s care and placement and this will be discussed within supervision. There is a written contract for supervision, a copy of which can be found at t he front of the supervision record book. Staff have competency based performance management at 3 months, 6 months, 12 months and annually thereafter with further built in reviews (see Tri.x section 8). Future training and development will be provided by line management assessment via the supervision and the performance management process. The teacher will receive regular supervision from the Head o f Education at The Caldecott Foundation. Case management supervision is built in and incorporated into the supervision structure. Team meetings o ccur every week . This comprises of a business meeting and case management meeting. The business meeting enables the team to deal with day to day business concerns, quality assurance, monitoring, modelling and men toring with clear targets and expectations set. Within this meeting issues raised within the child group meeting are discussed, decisions and responses indicated in preparations for reporting back to the children‟s group. Caldecott systems and process, i. e. training, holiday and sickness matrix, along with reports and action plans from regulation 33 visits and O fsted inspections within the quality assurance process are used. Regulation 34‟s are used to develop the service and outcomes for our children and young people. Planning is based on analysis of trends, patterns and improvements identified. Case Management mee tings are led by the Manager and supported by the house and school management team and a member of the Therapy team. In order to ensure prod uctive outcomes/decisions the team will collate all available information relating to the Childs strengths and difficulties. An expectation that analysis of the following is undertaken and presented to the meeting is key:  Incidents, Physical Intervention s and Accidents  Child Risk Assessment (changes and amendments)  Behaviour Support Plan 7 The Old Vicarage – Statement of Purpose  Day to Day Care Plan  Decisions from LAC Review and Statement Reviews  Relationship based information with Peers and family (Internal/external, including issues with conta ct)  Complaints, Compliments and Achievements  Triggers and Cycles and Changes to Patterns of Behaviour  Medical Issues (information from regular health checks, medication reviews and mental health concerns From this case planning can be agreed , directed and applied consistentl y by the whole team. The clinical psychologist who works at the home has her own arrangements in place for clinical supervision within her field of expertise. These include peer group supervision and case supervision. 7. Organisational S tructure of The Old Vicarage Sarah Orriss is supported by two deputy managers and a staff team consisting of Senior ECWs and ECWs (full staff details are recorded within Appendix 4). Staffing levels will meet the minimum requirement as determined by the operational needs of the Home. There will be a minimum of two staff on sleep in duty. 8. Admission Criteria The age range of the children/young people will be b etween 5 and 16+ years and will be of mixed gender . On the whole The Old Vicarage will accommo date children and young people who need to be assessed to determine which provision will best meet their needs. The home can work with a range of emotional and behavioural difficulties presented by the children and young people. Due to the nature of the b ehaviours that the majority of the children/young people placed at The Old Vicarage exhibit, children/young people resident at the home are supported and supervised by staff on a 24hr basis. The Old Vicarage will accommodate children and young people who need to be assessed, to determine which future provision will best meet their needs. Both sexes can be accommodated at the one time if both the Manager and Placing Authority deem this appropriate. The home‟s maximum occupancy is five children with the min imum staffing ratios being determined by the individual needs and presenting behaviours of the young people. Staff work closely with the young people to develop professional and constructive relationships. This forms a significant part of the work with chi ldren and young people in order to produce a professional assessment of current and future needs, in partnership with the Placing Authority, concerned professionals, the child and family. Recommendations are made in regular progress/review reports for tre atment and intervention programmes and also the needs that will need to be met by future placements. A placement at The Old Vicarage can be effective in breaking a negative cycle of behaviour and the environmental concept is such that unresolved feelings c an begin to be addressed safely. The level of support 8 The Old Vicarage – Statement of Purpose offered in a nurturing and responsive environment provides a setting where negative and disruptive behaviour can be contained and challenged constructively by boundary setting and consistency of practi ce. 9. Admission Procedure The Old Vicarage accepts either planned or emergency placements. The admission procedures vary. Planned Placements Following receipt of an initial referral from the child/young person‟s Social Worker , Education Department or Health Department the following procedure will apply: -  T he Senior Management Team will assess each referral/case which will require a variety of reports . An initial risk assessment will be undertaken to determine the level of risk (low, medium, high or critical) to enable us to identify the level of support and staffing required.  All professionals will read and comment on the papers, taking into consideration the whole range of needs, this will include education and health.  Introduction to local au thority, parents, carers, present school, education and health . Times and dates are arranged for visits to gather information about the child/young person‟s background, history, likes and dislikes and to gain everyone‟s experience of the child/young perso n. It is also an opportunity to provide information on the Foundation if it has not already been received.  Introduction to the child/young person . This is a visit by an identified professional from the multi - disciplinary team . A copy of The Old Vicarag e ‟s Children‟s Guide is presented to the child/young person.  The Old Vicarage staff team are informed , information gathered is disseminated to the team and the child/young person‟s bedroom is prepared.  Dates for visits to The Old Vicarage and visits to t he child/young person are confirmed with the Social Worker and present placement. The number of visits and the venue will depend on the needs of the child/young person. The child/young person may also have an overnight stay in The Old Vicarage prior to a dmission. Prior to admission the Social Worker will be sent papers to complete to provide the relevant information about the child/young person which will be given to the Registered Manager or Deputy Managers on admission. The admission process is comple ted when the child/young person arrives at The Old Vicarage . Each child/young person will initially meet with a senior member of staff who will explain the services on offer to the child/young person, what they can expect from the staff and the clear expe ctations of them whilst they are part of The Old Vicarage . This process will also include a tour of the house identifying all exits and procedures in the case of fire. If it is felt that the child/young person is at all confused at this stage, this will be constantly r e - enforced with the child/young person. 9 The Old Vicarage – Statement of Purpose Family and Social Worker contact will be arranged on admission and regular telephone contact will be maintained throughout the assessment process. Each child/young person‟s case will be reviewed weekl y with input from parents/carers (where appropriate), Social Worker and any other professionals involved in their case. Emergency Placements The Old Vicarage is able to receive emergency referrals. During office hours these will come through the normal referral line. Out of hours these will be handled by the house manager on call. Final decision for the placement of a child will be at the discretion of the registered Manager. This will be based on matching to the other children currently placed within the home, levels of risk and available staffing. Initial information will be gathered over the phone as to the reasons the placement is required. Where possible any supporting information will be emailed to the home. Where sufficient information is not av ailable on placement of the young person, the home has a range of generic risk assessments addressing a range of behaviours (i.e. self - harm, absenting, violence etc.) to ensure the initial safeguarding of the child, the peers and adults. Post admission a ll endeavours will be made to collate chronology/topography to inform immediate risk assessment and Day to Day Care Plan. The procedures for a planned placement will be followed wherever appropriate. There will be an internal review within 72 hours to asse ss the suitability of the young person to the placement. This will also identify a date for a placement planning meeting to be held with the Local authority within the first seven days to identify the longer term plan. Please see below The Old Vicarage In itial Emergency Admission Process. At the back o f this report under Appendices 6 you will find „Referral Process – Emergency Placements‟. The Old Vicarage can offer placements of either three or six month duration. Any extension to a three mo n th placeme nt would be agreed by all relevant parties. Any extension beyond six months would be for a period of no longer than 28 days. This would be put in place to support the child/young person‟s smooth transition to their next placement if one was not readily av ailable, or to complete a piece of work already commenced. Under exceptional circumstances this may be extended beyond the 28 days. A detailed report with clear recommendations will be provided to support all transitions. Information will be collated in line with the assessment process and appropriate visi ts will inform the transition plan. All children and young people will be provided with a record of their progress and achievements. The outcome of the assessment will be discussed and the child/young person‟s opinions sought and shared in line with thei r ongoing self evaluation . The child/young person‟s needs wi ll be identified and it will be the child/young person‟s Local Authority who will have the responsibility in terms of matching the next placement to the child/young person‟s needs. Any extensi on to a 3 month placement would be agreed by all relevant part ies . Any extension beyond 6 month s would be for a period of no longer than 28 days. This would be put in place to support the child/young person‟s 11 The Old Vicarage – Statement of Purpose smooth transition to their next placement if on e was not readily available, or to complete a piece of work already commenced. Under exceptional circumstances this may be extended beyond the 28 days. Emergency Departures Although every effort will be made to contain and work through all presenting behaviour, in exceptional circumstances serious consideration will be given to seeking the necessary removal of a child/young person in the event of the following: -  Serious sexual incident resulting in a child protection strategy discussion recommending t he removal of that child/young person to protect themselves or others.  Persistent violence resulting in serious injury to other children/young people or members of staff.  Persistently running away or going missing from care resulting in the child/young pe rson endangering themselves or others.  On the recommendation of medical advice where a child/young person is at risk of serious self harm or suicide. This decision will be taken by the Foundation‟s Senior Management Team following a multi - disciplinary me eting . In these instances, in consultation with the Local Authority , a request will be made for the child/young person to be removed and alternative accommodation sought within an agreed period of time not exceeding 72 hours. Following the decision at th e multi - disciplinary meeting to seek removal of a child/young person as an emergency measure a report will be written outlining the presenting health and safety concerns together with the assessment findings which will be present ed at the child/young perso n‟s R eview within 72 hours of their next placement. A member of staff from The Old Vicarage will make every effort to attend such a meeting. 10. Positive Aspects of Living in The Old Vicarage The Old Vicarage provides a ccommodation for 5 children/young people of mixed gender from the ages of 5 to 16+ years. The children and young people who come to The Old Vicarage may have experienced multiple placement break downs and find living in a family setting too difficult. The child ren/young people will have the opportunity to develop peer relationships within the house /school . Social communication difficulties, e.g. jealousy, rivalry and anti - social behaviour , will be addressed within children‟s meetings held on a weekly basis. All children and young people will be assessed and targeted against their personal learning and thinking skills, ability to manage emotions and behaviour with clear detailed input tied to rewards and consequences. Life skills and the ability to function independently will form part o f the assessment and working practice to enable children and young people to function within schools, colleges, clubs and possibly family settings. Detailed 1 to 1 work to encourage and support progress and development will be identified and evidenced. S taff will discuss the work undertaken with any child/young person to enable them to take steps to manage their own behaviour allowing them to grow, develop and build on their self esteem. 12 The Old Vicarage – Statement of Purpose The Caldecott Foundation employs staff with a wide range of skill s, talents and i nterests to provide a d iverse and comprehensive response in order to support the children and young people‟s interests. The Caldecott Foundation‟s core values are: -  Choice  Opportunity  Progress  Equality Talents and interests will be encou raged with external activities being identified where appropriate. Participation at outside clubs is encouraged and supported as part of the Day to Day Care Plan. 11. The Old Vicarage Ethos and Philosophy The philosophy of The Old Vicarage is that work unde rtaken within the home is child centred and with full consultation with the child/young person. The children and young people are encouraged to achieve; to reflect and monitor their own progress; to identify strengths and difficulties in order to determin e successful strategies and support requirements. The children and young people are encouraged to plan for and hold a positive view of their future. The environment of the home aims to achieve one of a caring and nurturing home where children and young pe ople are encouraged to develop, progress and take ownership of their lives and futures. The Old Vicarage will provide an environment where children and young people feel safe, understood and supported and where there is recognition of their individuality, diversity and uniqueness. Every young person is treated as an individual, with individual needs, goals and aspirations and the children/young people are supported to express their feelings and viewpoints The assessment model is based on the Framework of the Assessment of Children in Need and their Families, but specifically with the Every Child Matters criteria and 6 domain s of resilience, and recognises specific work to be identified according to a child/young person‟s strengths and areas of resilience. (Adolescence, Assessing and Promoting Resilience in Vulnerable Children by Brigid Daniel and Sally Wassell) . The Registered Manager and Deputy Registered Managers will monitor the assessments and any other aspects of the case for each child/young person e very week. There are areas around the home to which the children/young people are not permitted access. These areas are the staff office/s, staff sleeping areas and storage areas. The doors to these areas remain locked at all times. In the event of an incident within the home, other doors in the home may be secured to ensure everyone‟s safety. Our aims for the children / young people within The Old Vicarage Within the assessment framework we will aim to: -  Strive to empower the child/young person t o engage with the assessment process. 13 The Old Vicarage – Statement of Purpose  Provide for the emotional, social, educational, psychological, spiritual, cultural and developmental needs of the child/young person.  Provide consistency of care, stability and positive role modelling.  Provide an en vironment which will empower the children/young people to overcome their fears and enable the process of change.  Meet all the health needs for the child/young person.  Provide consistent support and security.  Provide opportunities for play and educationa l stimulation.  Identify the child/young person‟s strengths and needs.  Act as advocates on behalf of the child/young person.  Provide unconditionally positive regard and acceptance of the child/young person.  Provide an assessment which links with the 7 LAC dimensions and the 6 domain s of resilience tailored into the headings within Every Child Matters.  To engage in the assessment process.  To acknowledge self achievement, talents and skills.  To develop social , emotional, behaviour and educational skill s.  To develop their resilience.  An experience of positive peer interactions .  To fulfil their potential and identify their strengths and needs.  To develop an understanding of h ealth and safety around sexuality and personal boundaries.  A n a bility to con tinue to develop independently.  Develop t rust and the ability to seek appropriate support. 12. Protect and Promote the Health of Children & Young People Every child/young person will have an individual health care plan, which will be developed and monitored by the allocated worker and management team and in consultation with the child/young person. All relevant health information will be documented to ensure there is a continuation of care in this area. The Caldecott Foundation offers a holistic support pack age including both physical and mental health support. Other health care 14 The Old Vicarage – Statement of Purpose professionals involved with the care of the child/young person would also contribute to the health care plan and the child/young person would be supported to engage with all health r elated appointments. All young people are registered with a local General Practitioner, optician and dentist shortly after admission . Via the placement planning process , all relevant information regarding a child‟s health and their individual needs is do cumented to ensure that there is a continuation of care in this area. Children‟s health plans are developed and monitored by the home‟s Registered Manager to ensure medication is administered appropriately, appointments are kept and that work is done when young people refuse to attend appointments. Where there are issues of on going medical conditions referrals are made to local specialist services to ensure good levels of continuity of care. Contraception and sexual health matters, substance or alcohol misuse and the dangers will be treated sensitively by staff who will work alongside the child/young person in developing awareness and understanding. External agencies such as drug counselling and stop smoking services could be accessed for additional sup port and advice. Any prescribed medication will be kept in a secure, locked cabinet within the office and its dispersal or refusal will be recorded in line with legislative and organisational guidelines. Staff will be trained in the administration of medi cation and the administration of medication will be monitored by the management team to ensure this is dispensed and recorded appropriately. Homely remedies will also be subject to this process and procedure. Where appropriate, children and young people may be responsible for self - administration of some medication i.e. prescribed bath emollients and creams. Any self - medication will be recorded and a risk assessment implemented prior to use, and would be in line with the individual needs and understanding of the child/young person. Self - medication would be monitored by the allocated worker and management team. Children and young people will have the opportunity to plan and contribute to the weekly menu, where individual likes and dislikes and cultural and dietary needs will be taken into account. The staff team will support the child/young person to identify a healthy diet and choices, and experience and develop new tastes around food. An active and physical lifestyle will be supported through engaging i n physical activities and pursuits. Children and young people will be given the opportunity to develop emotional intelligence in respect of their health and well - being and will be given the support and skills to be able to express their feelings in appropr iate ways. The staff will use basic listening skills, support and reassurance and encourage the child/young person to actively refocus their thoughts and help establish stability and control in their lives. Positive re - enforcement through rewards, praise and positive feedback will help re - enforce their strengths and build awareness of talents and abilities. In relation to mental health we have a team of therapists working for the Foundation who can provide individual therapy as well as professional suppor t for staff teams. We also have the ability, where appropriate, to commission additional services as required. We will strive always to have a positive relationship with the local CAMHS team who may support us in renewing medication for on going conditio ns (such as ADHD). They can also advise, where necessary during periods of mental health crisis for young people to ensure children receive the appropriate levels of support and intervention. 15 The Old Vicarage – Statement of Purpose The Old Vicarage has commissioned the services of a clinical p sychologist who works one day at week at the home. The clinical psychologist will attend weekly case management meetings, reviews, staff meetings and will also undertake direct work with the children and young people where applicable. They will also cont ribute to the care planning and risk assessments for the children and young people. The assessment process uses a range of assessment tools including locus of control, empathy index, strength and difficulties and the Adaptive Behaviour Assessment System (ABAS). These assessment tools are completed on the child/young person within the first four weeks of placement and are evaluated by the Clinical Psychologist. Any findings will inform the case management meeting, child risk assessment, behaviour support plan and the final evaluation report on the individual child/young person. 13. Arrangements for Promotion of Education The Old Vicarage has its own designated classrooms and teacher. The staff team will encourage the children/young people in all educational activities and endeavour to ensure that all children/young people have their individual educational needs met. Should a child have a statement of educational need, the Teacher , the Head Of Inclusion and SEN and the Head of Education will use this to inf orm the timetable, behaviour management strategies and protocols provided in school. The teaching and care staff will work closely together to ensure continuity and consistency of support to the individual child/young person. The education department wil l request a review of the Statement of Educational need and this will form part of the planning and review process. Where it is deemed necessary, additional support, including the use of a designated teaching assistant, may be required to further support the child/young person within school. Following admission an educational assessment will be undertaken for each child/young person. Education will identify targets with weekly reviews in order to establish base line national curriculum levels, literacy (reading and spelling ages) and numeracy support needs. Opportunities for taster sessions in the surrounding Mainstream provision may be appropriate. This will allow clear observations around appropriate support settings (school and class size), transiti on difficulties and levels of educational support. Close liaison will ensure integrated support and practice to encourage learning and developmen t within a 24 hour curriculum. The Old Vicarage provides a communal study area for use by all the children/yo ung people and if any child/young person expresses a wish to have a desk in their bedroom this will be accommodated. 14. Arrangements for Promotion of Hobbies, Recreational, Sporting and Cultural Activities Access to activities which develop potential and en able personal growth are an integral part of the provision and all children/young people regardless of race, religion, cultural or sexual orientation are supported and encouraged to become involved in a range of activities and recreational pursuits. Child ren and young people will be given the opportunity to participate in activities within the home and local area in line with their own interests and needs and also to develop new hobbies and interests. The staff teams, care and education, work together in a n integrated way to promote all aspects of a child‟s development. 16 The Old Vicarage – Statement of Purpose When appropriate, and depending on the individual child‟s wants and wishes, an outside activity/club will be identified. Staff will support the child in regular attendance and encourage p articipation, ensuring that the child is properly prepared and equipped. Eventually, dependent on the child, this would be unsupported allowing further re - integration into the local community and the building of peer group social skills. Risk assessments would be implemented to assess and minimise the risks, in line with the child/young person‟s age and level of understanding and incorporating a degree of measured risk to further support the development of the child/young person. The children in The Old V icarage are regularly consulted about what toys, games , DVD‟s and magazines they would like available in the home. 15. Arrangements for Consultation with the Children/Young People The Old Vicarage will have weekly meetings to discuss and consult with the chi ldren about the operational aspects of the house. Children will contribute to these meetings and all children are invited to take part. These are child friendly and are age appropriate. Children are given the opportunity to make/voice informal complaint s both during and after the meeting. Minutes are kept as a record of discussions and outcomes. The children are consulted and may participate in the recruitment process of staff. The Manager will schedule specific time every month to have „Question Ti me‟ with the children/young people, where they have opportunity to discuss any issues with the Manager directly and receive feedback. The Manager also operates an ‟open door‟ policy for the children and young people, which will allow them to have the oppo rtunity to speak about any issues as they arise. Children and young people will be actively encouraged to participate in the development of their individual care plans, reviews and any other key meetings and will be supported and encouraged to ensure their views are heard. Monthly „Have your say‟ meetings are held at the Head Office, and the children and young people will have opportunity to contribute their views to these meetings. Any outcomes and decisions from these meetings will be fed back to the chi ldren/young people during the Home‟s residents meetings. The children/young people will have opportunity to discuss, debrief and comment following any incident, sanction or use of positive physical intervention. They will also have the opportunity to read and comment on their review reports, care and health plans. Children and young people are encouraged to personalise their bedrooms in line with their individual tastes and interests, and will also be consulted about the décor of the home and purchasing ne w items or re decoration. The children/young people will have regular one to one sessions with their allocated worker to discuss continued needs, progress, any issues and to set targets for the next meeting. 17 The Old Vicarage – Statement of Purpose The children and young people will also have reg ular monthly opportunity to discuss with the Regulation 33 inspector, Head Of Education and The Head of Care any concerns or issues they may have about the quality of care in the home. Any comments or concerns are raised and discussed with the Manager and appropriate action will be taken in response. For further information please see policy on Participation . Consultation with professionals, families and significant others relating to the care of the child and operational running of the home. The home wi ll ensure that any professionals and significant others including family where appropriate are kept informed of incidents, achievements and any concerns relating to the child. Formal consultation about the care and operational running of the home forms pa rt of the monthly Regulation 33 inspection. The manager, as part of the annual review of the Home will ensure consultation with Social Workers, Placing Authorities, families, significant others and other professionals inform the continued development of the service. 16. Details of the Home‟s Policy for Promoting Appropriate Behaviour and for the Control, Physical Intervention and Behaviour Management of Children/Young People Physical Intervention When caring for children/young people who display extremes of behaviour, it is sometimes appropriate to use physical intervention as a method of care and control, to ensure the safety of the child/young person and those who care for them. Physical intervention is always a last resort; after all other interventions h ave been exhausted. Physical intervention is a reality of the work undertaken, but The Old Vicarage, in line with the Caldecott Foundation policy work hard to decrease the need for intervention. Our belief is that the cycle of assessment, monitoring, eva luation and review allows for proactive planning rather than reactive action. All staff members are trained in Positive Management of Challenging Behaviour (see Tri.x Section 3). There is an emphasis on developing a skill base that enables control and bou ndaries without the need for physical restraint. All staff attend a 4 day Positive Management of Challenging Behaviour training course, which teaches them how to consider all options prior to physically restraining a child/young person and the physical re straint techniques. The techniques taught to staff members are based on PRICE training, and staff having received instruction in this method , are expected to sign to say they will not use any other techniques in the course of their duties. Staff will att end annual refresher training to ensure techniques are in line with PRICE practices. Only once a staff member has been observed and evaluated by the instructors are they deemed competent to undertake the techniques used. All physical interventions used are recorded, and these reports are monitored by the Manager, Head of Care, Regulation 33 inspector and where necessary the PRICE instructor to ensure techniques used are in line with legislative and operational guidelines. The PRICE instructor is availab le for consultation should the need arise and for further advice and guidance about specific behaviour management issues within the home. 18 The Old Vicarage – Statement of Purpose All significant incidents and incidents requiring restraint are recorded, and will include a written debrief and comme nts by the child/young person. The Manager/deputy manager will read, comment and sign every incident and restraint report and these will be further read by the Director of Operations/Head of Care or Quality Assurance manager to ensure that plans and strat egies consider the nature of the incident and its relevance within context. The antecedents are important in ensuring all responses are appropriate and in line with the risk assessment and child‟s behaviour plan. It is acknowledged that such an environmen t and working model will inevitably on occasion, result in an allegation of some nature against staff. In these circumstances, child protection procedures apply and are followed accordingly. Both staff and the child/young person involved in the use and ap plication of any physical intervention are debriefed following the incident and are offered medical attention. The child/young person will be closely monitored by staff following any physical intervention. Sanctions The Old Vicarage operates a policy of imposing clear and concise boundaries that children/young people must be able to understand. If the child/young person repeatedly challenges or breaks boundaries then a sanction may be imposed. Any sanction is given on the basis that it is within the com prehension of the child/young person and reasonable to the „offence‟. The child/young person will be given the opportunity to discuss any sanction imposed to offset feelings of isolation or victimisation. All sanctions are monitored by the Manager/Deputy Manager who will look at the effectiveness of the sanction, and any patterns of behaviour relating to when sanctions are imposed. All sanctions will be recorded and their authorisation discussed with staff by the manager to assess whether they are effect ive and appropriate. Where wilful damage or theft has occurred, the child/young person will be encouraged to make reparation to the victim in accordance with statutory regulations. 17. Arrangements for Child Protection and Countering Bullying Incidents are rigorously monitored (see Tri.x section 5) and any incidents which have any child protection implications are reported immediately to the Child Protection Officer (Quality Assurance and Safeguarding Manager) or Deputy Child Protection Officer (Learning an d Development Manager). All child protection incidents are thoroughly investigated in order to ascertain the nature of the incident (see Tri.x section 5). Bullying is monitored within The Old Vicarage on a daily basis and all staff members have a respons ibility to ensure that bullying is prevented at all levels (see Tri.x section 5). Bullying is a frequent topic at the children‟s weekly meetings. Staff encourage the children to voice any concerns they may have within the meeting and on a daily basis. 19 The Old Vicarage – Statement of Purpose A ll staff will receive child protection training as part of the induction process. This training will include reporting procedures and signs and symptoms of abuse. Child protection, within a working context, begins with the initial vetting procedures at the recruitment stage. All staff are CRB checked and references checked prior to commencement of employment. All lead interviewers have undertaken „Safer Recruitment‟ training. The named person responsible for Child Protection at The Old Vicarage is Sara h Orriss, Registered Manager. The designated Child Protection Officer (Quality Assurance and Safeguarding Manager) or Deputy Child Protection Officer (Learning and Development Officer) will be notified of any child protection allegations or concerns. All suspicions or allegations of abuse are firstly reported to the home‟s manager or on - call manager who will then notify the on - call Child Protection Officer. Incidents of abuse or suspected abuse will be recorded in line with legislative and organisational guidelines. All incidents will be reported to the child/young person‟s Social Worker, the local safeguarding team and Ofsted and will be fully investigated in consultation with the relevant parties, and in line with legislative and organisational guidelin es. Whistle Blowing Staff are actively encouraged to raise concerns. These are always recorded and acted upon. We will endeavour to create an „arena of safety‟ which promotes no secrets. The whistle blowing policy is highlighted during induction and rev iewed during staff meetings. Bullying Bullying is unacceptable within the home and will be challenged on an individual and group basis when encountered. The home operates a zero tolerance approach to bullying including verbal, physical and cyber bullyin g. If a child/young person will not take responsibility for their behaviour and continue to pose a risk to other children/young people then a strategy meeting will be convened with the Social Worker to discuss strategies and options. Bullying will be disc ussed regularly in resident‟s meetings, staff meetings and supervision. Any bullying or discrimination, including racism, will be recorded in the bullying/anti - discriminatory log within the home. An action plan will be drawn up in consultation with the c hild/young person, staff team and other professionals involved in the care of the child/young person(where applicable). All incidents and occurrences of bullying or other discriminatory actions will be closely monitored within the home, education and comm unity and recorded in the relevant log books. The home will support both the victim and perpetrator of any bullying through 1:1 direct work sessions and education into cause and effects and the impact of bullying. There are specific bullying policies and p rocedures in place (see Tri.x section 5). 20 The Old Vicarage – Statement of Purpose 18. Procedures for Dealing with Unauthorised Absence /Missing from Care The Old Vicarage follows the Foundation policies and Local Authority guidance on this issue. The home has on file the joint protocol for chi ldren missing from care for Nottinghamshire, and would also access the missing from care protocols relevant to the child/young person‟s local authority on placement. The home has links with the local Missing from Care Police co - ordinator, to further discu ss joint strategies should a need be identified. Should a high risk child/young person move into the home then the missing from care officer and LADO for Nottinghamshire will be informed. The local Police will also be notified of any high risk child movi ng into the home. Any incident of missing or absent without authority will be taken seriously. In line with the systems and processes in place, staff members will search the local area to try and locate the child/young person. Dependent on the nature of the child/young person as identified within the child risk assessment, age and length of time missing, the Police will be notified (including where applicable the Transport Police), the child‟s Social Worker or out of hours duty team, parents (where applic able) and the senior manager covering the Foundation‟s out of hours on - call (see Tri.x section 3). Children/young people identified as high/critical in relation to their risk category would have the relevant information shared with local services (Social Services, Police, Fire etc.) to ensure appropriate response and strategies were implemented. Every child/young person will have an individual protocol to be followed in the event of any missing or absence incident. This will be agreed jointly with the Soc ial Worker and local police and will take into account the history, the age and vulnerability of the child/young person. Each period of unauthorised absence or missing from care episode will be recorded in the absence/missing from care log with all releva nt details included. On return from any period of absence or missing, all relevant parties will be notified. The child/young person‟s immediate needs around sustenance, comfort and medical attention will be met. The child/ young person will have opportu nity to discuss their reasons for the absence and will be given opportunity to speak to their Social Worker about any concerns or issues relating to the absence and reasons for leaving. The Social Worker will be expected to discuss with the child/young pe rson the reasons for the absence or missing episode and work with the child and the home around strategies to further support and prevent absences and missing episodes from occurring. The child/young person would be given opportunity to speak to someone in dependent from the home i.e. Regulation 33 inspector or Police following any period of being missing or absent. The missing from care officer for Nottinghamshire may come to speak to the child/young person. The Child Risk Assessment, Behaviour support pla n and day to day plan would be reviewed, updated and amended if necessary in response to any missing or absence episode. If a child/young person has a history of absence or missing episodes this will be recorded and form part of the child risk assessment, care plan and behaviour management plan and will be closely monitored by the Registered Manager. Where it is evident that episodes of absence or missing are showing a pattern or increase in frequency, the Registered Manager will initiate a strategy meeti ng to review the current needs of the child/young 21 The Old Vicarage – Statement of Purpose person and the suitability of the placement. It may be appropriate for the Social Worker to consider an alternative placement should there be an escalation of absence and missing episodes. The staff team w ill work with the child/young person around any issues they have that may lead them to go absent or missing , and give them plenty of opportunity to discuss any concerns they may have. By promoting a positive and supportive environment where children and y oung people feel valued and listened to, this may lead to the children and young people feeling they have choices and options around the placement and engaging with this. 19. Electronic and Mechanical Surveillance /Surveillance of children/young people resident in the home. The Old Vicarage has alarms fitted to the children/young people‟s bedroom doors. This enables the sleeping in staff to be alerted should a child/young person leave their bedroom during the night, so they are able to offer support and guidan ce if necessary. This further reduces the risk of child protection issues as staff will be able to monitor any child/young person leaving their bedroom at night. Door alarms are switched on once the children/young people retire to their bedrooms for the n ight and staff members are going to their own sleep in rooms. There is a door alert on the front and back door that issues and audible beep in the office and alerts whoever is in the office should anyone, including staff, enter or leave the home. At point of placement, staffing levels would be agreed in line with the individual needs and behaviours of the child/young person. Staff will ensure that the children and young people in residence are supervised at all times on appropriate staffing levels in the h ome and community. 20. Fire Procedures There is a hard wire fire alarm system which is checked weekly and the results recorded. Full evacuation drills will occur monthly, or on the arrival of a new resident or staff member as part of the induction process. Drills will be undertaken in the evening as well as the day and all children/young people are expected to participate in all scheduled drills. Fire fighting equipment, fire doors etc. are checked on a weekly basis and the results recorded. All staff wil l receive fire training as part of the induction process. The Old Vicarage implements the requirements of the local fire authority and works in accordance with the National Minimum Standards. All equipment and systems are regularly inspected within the ho me and by outside agencies to s specified time schedule. All alarms, checks, drills and servicing schedules will be recorded within the fire log book as well as any maintenance required and outcomes. All visitors to the home will be recorded in the visito r‟s book and will be informed of the fire evacuation procedures within the home. 22 The Old Vicarage – Statement of Purpose 21. Arrangements for Religious Instruction and Observance and Cultural Needs Within the initial planning process and in implementing the individual care plan, any religious o bservances will be made clear and the home will ensure that religious observances are recognised and celebrated. Staff members will endeavour to facilitate access to and attendance at the relevant place of worship in line with the individual needs and wi shes of the child/young person. Staff will ensure that the cultural identity of the child/young person is maintained within the home and the community, in accordance with the child/young person and his/her family‟s wishes. This will take into account all aspects of diet, dress, healthcare, religious needs, language, customs and celebrations and positive self - identity. Any cultural, linguistic and religious needs would be identified ideally prior to placement to ensure the home could meet the cultural, ling uistic and religious needs of the child. Careful consideration would be given when placement planning to ensure all the child/young people‟s needs could be met by the home. 22. Contact Arrangements Arrangements for contact for the children/young people will be agreed in consultation with the child/young person‟s Local Authority. All contact will be discussed and negotiated at the point of admission and reviewed at regular intervals. Contact between parents, family and friends will be maintained and facilita ted as agreed and will be incorporated into the individual care plan. The staff will endeavour to ensure that all contact is facilitated as unobtrusively as possible and any supervised contact will be negotiated with the placing authority and take into acc ount the child/young person‟s wishes. All visits should be planned and arranged in advance. A telephone will be available for the child/young person to receive and make calls in line with their individual contact arrangements and agreements. The individu al care plan will detail and reflect the arrangements for telephone use and letters, as well as face to face contact. The use and access to a mobile phone will be discussed at point of placement and admission. A clear agreement between the child/young per son and the home regarding safe use of a mobile phone will be put in place prior to the use of any mobile phone. This will be done to ensure the safety of all, and in consultation with the Social Worker to ensure there are clear guidelines and expectation s in place. The child/young person will be encouraged to invite family and friends (where appropriate) to the home. Friendship s will be encouraged and supported should the child/young person have friends that they wish to maintain contact with, where appr opriate, or friends that they make locally in school or clubs in the community. The Old Vicarage follows the Foundation‟s practice guidelines on reporting and vetting visitors. 23 The Old Vicarage – Statement of Purpose 23. Complaints The Caldecott Foundation has an overall complaints policy w hich is used in The Old Vicarage . The complaints procedure consists of 3 stages: - i. The Informal or Problem Solving Stage ii. The Formal or Registration Stage iii. The Review Stage All complaints are taken seriously and the appropriate procedures a nd processes are followed (see Policy CH8). All children/young people are made aware of the complaints procedure through the “ Children‟s Guide ” and open discussions within the house. Complaint slips are readily available and contain details of people the child/young person can contact, including their Social Worker , the Ofsted Inspector; Regulation 33 Visitor ; Director of Operations; Head of Care ; Head of Education; Chief Executive; Roger Morgan, Director of Children‟s Rights ; Nottingham Social Services a nd a local police officer, if they wish to discuss an issue with someone outside of The Old Vicarage and the Foundation. All children will be encouraged to attend their Reviews or participate at some level to ensure that their views and opinions are heard. The child will always have their co worker(s) present in their Reviews or a significant other who can advocate on their behalf. Children are also encouraged to complete their own personal Review documents. 25. Accommodation The accommodation consists of one house spread over three floors. The bedrooms for the children/young people are located on t he second floor along with the staff sleep in rooms. All children/young people have their own bedrooms. There are two bathrooms on the 2 nd floor, both having showering facilities and one having a bath. There is a separate bathroom with showering facilitie s on the ground floor for staff use along with an additional separate toilet. 24 The Old Vicarage – Statement of Purpose The home has window restrictors fitted on all windows at height, including bedrooms and all doors are fire doors throughout the home. The home has extensive grounds with a range of gardens and outdoor space for play and recreation. The home is set in a rural setting and is located within an enclosed area for additional privacy. The children/young people will have opportunity to contribute to the décor and resources purchased for the home and personalising their own environment. A minimum of two staff will undertake sleep - in duties. 26. Therapeutic Techniques In line with the Organisations Model of Care which ensures that staff are able to reflect and consider appropriate safe and c aring responses to high levels of emotional and behavioural need, staff are trained in Mentalization. This is supported through consultation with therapists both in Nottingham and in Kent. The home works in a therapeutic way around the domains of resilie nce and attachment styles of the children and young people. Weekly case management meetings discuss and chart issues and concerns around the individual needs and behaviours of the child/young person . T he clinical psychologist is involved in regular case m anagement meetings with the staff team to offer a therapeutic perspective of behaviours and strategies for working with the child/young person. The Team will have access to further advice and support via the therapy team based in Kent. There are multi - di sciplinary case management meetings held regularly with the therapy and education team in Kent which the Manager attends. A report is produced for each meeting highlighting the most pertinent and current concerns around the child. The home would endeavou r to work closely and alongside local CAMHS if this were deemed appropriate. 27. Anti - Discriminatory Practice The Caldecott Foundation has an underpinning model of promoting anti - discriminatory practice at all levels. The Old Vicarage operates in a child ce ntred way always taking into account the whole child/young person, children‟s rights are observed and all children/young people have a full understanding of the complaints procedure and who they can turn to if they feel their rights are not being met. All children/young people, regardless of their ethnicity, religion, culture, disability or sexual orientation, will have their individual needs met and staff will endeavour to ensure that needs are met at all levels either internally or external provision be i ng provided if the need arises (Tri.x Section 1) . All children/young people will have their privacy respected and will have their own bedroom. There are locks on all toilet and bathroom facilities to allow for privacy when bathing. All staff will knock and wait for a response prior to entering a child/young person‟s bedroom or bathroom, unless in an emergency. There will be rooms available 25 The Old Vicarage – Statement of Purpose for a child/young person to make a telephone call or meet with family or professionals involved in their care in pr ivate. All files and confidential information relating to the child/young person will be kept in a secure locked c upboard in the office and this information will only be shared with those having a right to this information, as dictated by legislation. All children/young people placed at The Old Vicarage will be shown a level of dignity that will not discriminate against them. Children/young people will be encouraged to follow their religious and ethnic beliefs with exception. They will have adequate cloth ing, personal items and belongings that would be seen as reasonable in the climate in which they live. A child/young person‟s self - esteem and self - worth will be of paramount concern throughout their placement. The individual care plan will encourage the g rowth of the child/young person‟s self - esteem and emotional well - being and resilience through positive thoughts, actions and role modelling. Staff will support the child/young person to access education appropriate to their needs and in line with their go als and aspirations. Children and young people will be supported to make informed choices and be involved in the decision making process. Young people will be supported to explore their feelings and vocalise their viewpoints to enable the children/young p eople to feel empowered and assist them to take control and achieve positive outcomes. Whilst resident at The Old Vicarage, the children/young people will be given opportunity to make choices in everything they do, within any constraints of their care pla n, and this will include food, accommodation and activities. All children/young people will have equal rights to all services, as appropriate to their individual needs and development. Each child/young person will have the right to choice, fulfilment, ind ependence, privacy and dignity in line with their individual needs. To work closely with the children/young people in developing caring, constructive and professional relationships. 2. To regularly assess the individual‟s needs and support them on an individual and group basis. 3. Produce a professional assessment of current and future needs in partnership with the Placing Authority, key professionals, significant others, family and the children/young people. 4. To identify and comment on the future needs and requirements of any placement or pending placement to aid successful transition. 5. To support the individual child/young person with an integration programme into a new residency. 6. To actively seek professional advice and assessment and work in partnership with appropriate agencies, within the commissioning instruction of the Placing Authority. 7. To ensure positive continuation of the placement by inclusion in the participation of in - house and external activities and individual programmes, ensuring that the placement continues to be productive and appropriate for the individual child/young person. 8. To report any incident or physical intervention to the appropriate people within regulatory requirements. Ensure staff and child/young person are supported and debriefed following any incident or intervention. 9. To inform Social Worker/Placing authority of any unauthorised absence from the Home and follow individual protocols and procedures in place, including informing the Police. 10. To ensure all chil dren/young people are welcomed back from any absence and are given support, reassurance and medical attention if needed. All children/young people to have opportunity to discuss their reasons for absence and opportunity to discuss any concerns with someon e external from the Home i.e. Social Worker. All young people to have a risk assessment and protocol in place to manage any absence. All relevant parties are to be informed of return. 11. To ensure a bound complaints book is kept in which to record any compl aint about the home, staff or young people. All children/young people will be given a copy of the complaints procedure on admission to the home and all complaints are taken seriously and investigated fully. The outcome of any complaint is shared with the child/young person and Placing Authority. 12.
