Sussex Health and Care Partnership (SHCP) 8 July 2020 Contents Defining our system Guiding principles for how we work as an ICS Governance arrangements Mental Health Collaborative The role of Sector Connector 2 Defining our system 3 Defining our system We serve a large population of 1.7 million people and are responsible for £4bn of health and care spending, delivered by over thirty thousand staff. We are one of the largest and most complex systems with a varied population and pockets of significant socio-economic deprivation, alongside significant wealth. Our population includes some of the most quickest ageing demographics in the country with the associated rapid growth in long term conditions and demand for service. Our Population Health Check sets out our understanding of the challenges we face in meeting the needs of our population. Our system in Sussex We are responsible for leading and improving population health and operational performance whilst also delivering transformational change for health and care across Sussex. To do this our ICS brings together all Sussex stakeholders to redesign care and improve population health as set out in the NHS Long Term Plan Our ICS uses three levels to do this and these are: System – which means Sussex Place – In Sussex this divided into three areas; East Sussex, West Sussex and Brighton & Hove Neighbourhood – covered by Primary Care Networks (PCNs) and community services that link into them We have an agreed set of design principles which define how all partner work together and at what level, we are also developing a new ICS ‘Compact’ to support the ongoing development. 4 We are deploying a collaborative leadership model – with clear, common purpose, drawing support from all parts of the system to develop the partnership culture needed to create and sustain system-wide improvement. Partnership Executive Drawn from participating commissioners, NHS trusts, primary care networks, local authorities, the voluntary and community sector and regulators Governance Collaborative and inclusive multi-professional leadership and governance backed by ongoing involvement of community partners, service users and the public in decision making Sub groups: Portfolio Management Group Brighton & Hove Clinical & Professional Cabinet, Chief Operating Officers Group, Strategy Directors Group, Finance Group, Acute Care Collaborative Primary Care Full engagement with primary care, including through a named accountable Clinical Director of each primary care network Supporting Infrastructure Dedicated capacity at system, place
