Slide3What is a SMART Health Card?
A SMART Health Card is a paper or digital version of a COVID-19 Vaccination Record. They allow individuals to keep a copy of their records on hand and easily share this information with others if they choose. SMART Health Cards contain information in a secure, portable format that is easy for entities to verify.
When scanned, a verifier can easily see the information on the card has not been tampered with, making SMART Health Cards a safe and reliable way to demonstrate vaccination status.

Download the CommonHealth app. Follow instructions in the app to access your Covid-19 vaccine credential, by scanning or uploading a SMART Health QR code from a compatible issuer.

SMART Health Card Verifier App
The SMART Health Card Verifier App allows someone to scan a SMART Health Card QR code to quickly verify an individual’s COVID-19 vaccination status.
The Verifier App determines if the card is valid, and if the issuer is within the provider network.
