License Description: Major Requirements for Certification: Note: Criteria below pertains to all three titles listed -- 1) At least 18 years of age. 2. Not have been convicted of any felony or currently be under indictment for a felony; and within five years immediately preceding the application for an associate, security guard or armed security guard registration certificate, not have been convicted of any misdemeanor act involving: a) personal violence or force against another person or threatening to commit any act of personal violence or force against another person; b) misconduct involving a deadly weapon as provided in section Arizona Revised Statutes 13-3102; c) dishonesty or fraud, arson, theft, domestic violence, sexual misconduct; 3. Not be on parole, on community supervision, on work furlough, on home arrest, on release on any other basis or named in an outstanding arrest warrant; not be either of the following: a) adjudicated mentally incompetent; b) found to constitute a danger to self or others; not have a disability as defined in section ARS 41-1461, unless that person is a qualified individual with a disability as defined in section ARS 41-1461; and not have been convicted of acting or attempting to act as an associate security guard or armed security guard without a license if a license was required. 4) Successfully complete all background screening and training requirements. 5) Completes eight hours of pre-assignment training before applying for and being granted a security guard registration certificate; 6) Complete a training verification form with a signature from an instructor and a qualifying party affirming that the security guard met the training requirements; 7) For armed security guard, in addition to Nos. 5 and 6, complete 16 hours of firearms-safety instruction 8) For armed security guard, complete a firearms-safety training verification form with a signature from an instructor and a qualifying party affirming that the security guard met the training requirements. License Description: Major Requirements for Certification: General Requirements -- 1) The evaluation to determine qualification for certification shall not begin until an institutional recommendation or application for certification and official transcripts, and the appropriate fees have been received by the Department. Course descriptions, verification of employment, and other documents may also be required for the evaluation. 2) The effective date of a new certificate shall be the date the evaluation is completed by the Department. The effective date of a renewed certificate shall be the date the evaluation for renewal is completed by the Department; 3) All one-year certificates shall expire one year from the date of issuance. All certificates issued for more than one year shall expire on the holder\'s birth date in the year of expiration. 4) If an applicant has not met all the requirements for the certificate or endorsement at the time of evaluation, the applicant shall have a maximum of two years to complete those requirements and request re-evaluation. One re-evaluation shall be provided at no additional fee within two years of the original evaluation. 5) Only those degrees awarded by an accredited institution shall be considered to satisfy the requirements for certification. 6) Professional preparation programs, courses, practical, and examinations required for certification shall be taken at an accredited institution or a Board-approved teacher preparation program. 7) Only those courses in which the applicant received a passing grade or credit shall be considered to satisfy the requirements for certification. 8) Teachers in grades 7 through 12 whose primary assignment is in an academic subject shall demonstrate proficiency by passing the appropriate subject area portion of the Arizona Teacher Proficiency Assessment. The subject areas of demonstrated proficiency shall be specified on the certificate. If a proficiency assessment is not offered in a subject area, an approved area shall consist of a minimum of 24 semester hours of courses in the subject; 9) Teachers of homebound students shall hold the same certificate that is required of a classroom teacher. 10) Effective January 1, 2008, all teachers and persons who are required to be fingerprinted to work in the classroom are required to have an Identity Verified Prints (IVP) Fingerprint Clearance Card. A valid non-IVP Fingerprint Clearance Card issued prior to January 1, 2008, will be accepted by Certification. Non-IVP Fingerprint Clearance Cards issued after January 1, 2008 will not be accepted. Birth-Kindergarten Certification -- 1) Must hold an Early Childhood certificate or endorsement by July 1, 2012. 2) Bachelor\'s degree 3) Elementary Teaching Certification -- Provisional Certification (10,547 certificates): 1) Bachelor\'s degree; 2) One of the following: a) completion of a teacher preparation program in elementary education from an accredited institution or a Board-approved teacher preparation program; or 45 semester hours of education courses which teach the knowledge and skills of Arizona professional teaching standards, including at least eight semester hours of practicum in grades K-8. Two years of verified teaching experience in grades Pre kindergarten-8 may be substituted for the eight semester hours of practicum; or a valid elementary certificate from another state. 3) Passing score on the professional knowledge portion of the Arizona Teacher Proficiency Assessment;

License Description: Major Requirements for Certification: 1) 18 years or older; 2) Satisfactory evidence of graduation from a two-year curriculum in veterinary technology, or the equivalent of such graduation as determined by the Board in a college or other institution approved by the Board; or satisfactory evidence that the applicant has been employed for at least two years as a veterinary assistant under the supervision of a veterinarian regularly licensed in this state and is recommended to the Board by the employing veterinarian or veterinarians; 3) Application/examination fee - $150; 4) Typewritten letter to the Board or an updated resume giving background information, experience and qualification; 5) If applying for certification based upon graduation from an accredited two-year college, an official final transcript must be sent directly to this Board by the school from which you graduated; or if the application is based on experience, following is required: a) verification of employment giving satisfactory evidence of being employed for at least two years (4,160 hours) as a veterinary assistant under the supervision of a veterinarian regularly licensed in this state. The hours may be completed at more than one premise; b) a notarized letter from each Arizona licensed veterinarian who employed the applicant during the two years, verifying the employment, indicating the length of employment and recommending the applicant. The form, \"\"Veterinary Technician Professional Letter of Recommendation,\"\" must be used to complete this requirement. Certification Fee: Initial - $50 (odd year); $25 (even year); Renewal (Biennially) - $50 Continuing Education: 10 credit hours during renewal period; 5 credit hours if initial license issued in even year.
