License Description: Major Requirements for License: Independent Substance Abuse Counselor (1,750 licenses) -- 1) Master\'s or higher degree from a regionally accredited college or university in a behavioral health service with a minimum of 24 semester credit hours of counseling related coursework as determined by the substance abuse credentialing committee. Coursework must include a significant clinical component, such as statistics coursework. Graduate coursework completed outside of the master or higher degree and submitted to meet curriculum requirements shall constitute no more than three semester credit hours; 2) Pass Level II or higher examinations offered by the National Association of Alcoholism and Drug Abuse Counselors. 3) Complete a minimum of 3,200 hours of supervised work experience in substance abuse counseling in no less than 24 months. The supervised work experience is limited to the use of psychotherapy for the purpose of assessment, diagnosis and treatment of individuals, couples, families and groups as they relate to substance abuse and chemical dependency issues. The 3,200 hours of supervised work experience shall include a minimum of 1,600 hours of direct client contact. 4) During the supervised work experience required in R4-6-705, an applicant for associate substance abuse counselor and independent substance abuse counselor licensure shall receive a minimum of 100 hours of clinical supervision in no less than 24 months. 5) During supervised work experience applicant shall demonstrate satisfactory performance in the following areas as they relate to substance abuse counseling: intake, diagnostics, assessment, triage, crisis intervention, treatment planning, family, group and individual psychotherapy, outreach, and consultation with other professionals by having the applicant\'s clinical supervisor submit a performance evaluation on forms available from the Agency. Fees: Application - $250 Initial License - $250; Renewal (Biennially) - $350 Continuing Education: 30 hours during 2-year renewal period Associate Substance Abuse Counselor (118 licenses) -- 1) Bachelor degree from a regionally accredited college or university in a behavioral health service with a minimum of 30 semester credit hours of counseling related coursework as determined by the substance abuse credentialing committee; or a master\'s or higher degree from a regionally accredited college or university in a behavioral health service with a minimum of 24 semester credit hours of counseling related coursework as determined by the substance abuse credentialing committee. The Board shall not accept coursework that does not include a significant clinical component, such as statistics coursework, as meeting the coursework requirements in this Section. 2) Pass Level II or higher examinations offered by the National Association of Alcoholism and Drug Abuse Counselors. 3) Complete a minimum of 3,200 hours of supervised work experience in substance abuse counseling in no less than 24 months. The supervised work experience is limited to the use of psychotherapy for the purpose of assessment, diagnosis and treatment of individuals, couples, families and groups as they relate to substance abuse and chemical dependency issues. The required degree must contain a minimum of 18 hours of coursework specifically related to chemical dependency. 2) The substance abuse credentialing committee may waive the education requirement in subsection if the applicant demonstrates the following: a) provides services pursuant to a contract or grant with the federal government under the authority of 25 U.S.C. 450 - 450(n) or 25 U.S.C. 1601 - 1683; has obtained a high school diploma or equivalent degree; because of cultural considerations, obtaining the degree required for substance abuse technician licensure would be an extreme hardship for the applicant; has completed a minimum of 6,400 hours of supervised work experience in substance abuse counseling in no less than 48 months completed within the seven years before the date of application; and has completed a minimum of 200 hours of clinical supervision in no less than 48 months within the supervised work experience submitted; 3) Pass Level II or higher examinations offered by the National Association of Alcoholism and Drug Abuse Counselors. By Reciprocity: Yes 1) Applicant is currently licensed or certified in another state by a state regulatory entity in the discipline for which the applicant is requesting licensure in Arizona and the license or certification is in effect and in good standing; 2) Passage of an examination was required for the certification or licensure and the applicant passed the required examination or the applicant has passed the licensure examination required for the discipline and level license sought by the applicant; 3) The credentialing committee determines that the applicant\'s qualifications substantially meet the current requirements for Arizona licensure; 4) Prior to applying for licensure in Arizona, an applicant was engaged in the practice of behavioral health for a minimum of 800 hours each year for at least five out of the last seven years in the state issuing the license; 5) Prior to applying for licensure in Arizona, an applicant for licensure as a bachelor social worker or a master social worker has practiced as a social worker for a minimum of 800 hours each year for at least five out of the last seven years in the state issuing the license used by the applicant to qualify for licensure by reciprocity; and the applicant is not the subject of a pending complaint before the Board or any other state behavioral health regulatory entity and has not had a license or certificate to practice a health care profession suspended or revoked by another state behavioral health regulatory entity. License Description: Veterinarian Major Requirements for License: By Examination -- 1) Graduate from a college of veterinary medicine accredited by the AVMA or is currently licensed as a practicing veterinarian in another state, or holds a certificate of completion issued by the ECFVG or PAVE; 2) Official transcript sent directly to this Board by the AVMA accredited veterinary college from which the applicant graduated. The transcript must show graduation date and degree earned. If applicant is a graduating student at the time of application, a letter from dean of the veterinary program must be sent to the Board stating projected graduation date and good standing. Graduation date must be within 45 days following the exam. Exam/Application Fee - $400; 3) If applicant is or was a licensed veterinarian in another state, each state or territory of the United States must send directly to this Board a verification of licensure to include current standing, status of any current investigation or discipline received for violation of a veterinary medical practice act. If applicant is a foreign graduate, submit a copy of the ECFVG Certificate of Completion issued by the AVMA (Arizona Veterinary Medical Association) or a copy of the PAVE Certificate of Completion issued by the AAVSB; 4) Pass the NAVLE (North American Veterinary Licensing Examination) with a score of at least 425 (75%) within 5 years of application for state examination and licensure. Scores must be submitted directly to our Board from the American Association of Veterinary State Boards (AAVSB). Exam Fee - $100. By Endorsement: 1) Graduate from a college of veterinary medicine accredited by the AVMA, or holds a certificate of completion issued by ECFVG or PAVE. Official transcript must be sent directly to the Board by the veterinary college, showing graduation date and degree earned; 2) Applicant is currently licensed as a practicing veterinarian in another state. recognized by the AVMA. Note: The applicant\'s practice will be limited to the scope of the applicant\'s board certification. Each state or territory of the United States must send directly to this Board a verification of licensure to include current standing, status of any current investigation or discipline received for violation of a veterinary medical practice act. If applicant is a foreign graduate, submit a copy of the ECFVG Certificate of Completion issued by the AVMA or a copy of the PAVE Certificate of Completion issued by the AAVSB; 3) Typewritten letter or current resume summarizing experience and qualifications. 4) Verification of employment as a veterinarian for at least three of the preceding five years or six of the preceding 10 years in one or more states before filing an application for licensure in Arizona. Endorsement/Application Fee - $750; License Fee: Initial - $200 (odd year); $100 (even year) Renewal (Biennially) - $350 Continuing Education: 20 credit hours per renewal period; 10 credit hours if initial license in even year.

License Description: Major Requirements for Certification: 1) 18 years or older; 2) Satisfactory evidence of graduation from a two-year curriculum in veterinary technology, or the equivalent of such graduation as determined by the Board in a college or other institution approved by the Board; or satisfactory evidence that the applicant has been employed for at least two years as a veterinary assistant under the supervision of a veterinarian regularly licensed in this state and is recommended to the Board by the employing veterinarian or veterinarians; 3) Application/examination fee - $150; 4) Typewritten letter to the Board or an updated resume giving background information, experience and qualification; 5) If applying for certification based upon graduation from an accredited two-year college, an official final transcript must be sent directly to this Board by the school from which you graduated; or if the application is based on experience, following is required: a) verification of employment giving satisfactory evidence of being employed for at least two years (4,160 hours) as a veterinary assistant under the supervision of a veterinarian regularly licensed in this state. The hours may be completed at more than one premise; b) a notarized letter from each Arizona licensed veterinarian who employed the applicant during the two years, verifying the employment, indicating the length of employment and recommending the applicant. The form, \"\"Veterinary Technician Professional Letter of Recommendation,\"\" must be used to complete this requirement. Certification Fee: Initial - $50 (odd year); $25 (even year); Renewal (Biennially) - $50 Continuing Education: 10 credit hours during renewal period; 5 credit hours if initial license issued in even year.
