License Description: Major Requirements for Certification: General Requirements -- 1) The evaluation to determine qualification for certification shall not begin until an institutional recommendation or application for certification and official transcripts, and the appropriate fees have been received by the Department. Course descriptions, verification of employment, and other documents may also be required for the evaluation. 2) The effective date of a new certificate shall be the date the evaluation is completed by the Department. The effective date of a renewed certificate shall be the date the evaluation for renewal is completed by the Department; 3) All one-year certificates shall expire one year from the date of issuance. All certificates issued for more than one year shall expire on the holder\'s birth date in the year of expiration. 4) If an applicant has not met all the requirements for the certificate or endorsement at the time of evaluation, the applicant shall have a maximum of two years to complete those requirements and request re-evaluation. One re-evaluation shall be provided at no additional fee within two years of the original evaluation. 5) Only those degrees awarded by an accredited institution shall be considered to satisfy the requirements for certification. 6) Professional preparation programs, courses, practical, and examinations required for certification shall be taken at an accredited institution or a Board-approved teacher preparation program. 7) Only those courses in which the applicant received a passing grade or credit shall be considered to satisfy the requirements for certification. 8) Teachers in grades 7 through 12 whose primary assignment is in an academic subject shall demonstrate proficiency by passing the appropriate subject area portion of the Arizona Teacher Proficiency Assessment. The subject areas of demonstrated proficiency shall be specified on the certificate. If a proficiency assessment is not offered in a subject area, an approved area shall consist of a minimum of 24 semester hours of courses in the subject; 9) Teachers of homebound students shall hold the same certificate that is required of a classroom teacher. 10) Effective January 1, 2008, all teachers and persons who are required to be fingerprinted to work in the classroom are required to have an Identity Verified Prints (IVP) Fingerprint Clearance Card. A valid non-IVP Fingerprint Clearance Card issued prior to January 1, 2008, will be accepted by Certification. Non-IVP Fingerprint Clearance Cards issued after January 1, 2008 will not be accepted. Birth-Kindergarten Certification -- 1) Must hold an Early Childhood certificate or endorsement by July 1, 2012. 2) Bachelor\'s degree 3) Elementary Teaching Certification -- Provisional Certification (10,547 certificates): 1) Bachelor\'s degree; 2) One of the following: a) completion of a teacher preparation program in elementary education from an accredited institution or a Board-approved teacher preparation program; or 45 semester hours of education courses which teach the knowledge and skills of Arizona professional teaching standards, including at least eight semester hours of practicum in grades K-8. Two years of verified teaching experience in grades Pre kindergarten-8 may be substituted for the eight semester hours of practicum; or a valid elementary certificate from another state. 3) Passing score on the professional knowledge portion of the Arizona Teacher Proficiency Assessment;

License Description: Veterinarian Major Requirements for License: By Examination -- 1) Graduate from a college of veterinary medicine accredited by the AVMA or is currently licensed as a practicing veterinarian in another state, or holds a certificate of completion issued by the ECFVG or PAVE; 2) Official transcript sent directly to this Board by the AVMA accredited veterinary college from which the applicant graduated. The transcript must show graduation date and degree earned. If applicant is a graduating student at the time of application, a letter from dean of the veterinary program must be sent to the Board stating projected graduation date and good standing. Graduation date must be within 45 days following the exam. Exam/Application Fee - $400; 3) If applicant is or was a licensed veterinarian in another state, each state or territory of the United States must send directly to this Board a verification of licensure to include current standing, status of any current investigation or discipline received for violation of a veterinary medical practice act. If applicant is a foreign graduate, submit a copy of the ECFVG Certificate of Completion issued by the AVMA (Arizona Veterinary Medical Association) or a copy of the PAVE Certificate of Completion issued by the AAVSB; 4) Pass the NAVLE (North American Veterinary Licensing Examination) with a score of at least 425 (75%) within 5 years of application for state examination and licensure. Scores must be submitted directly to our Board from the American Association of Veterinary State Boards (AAVSB). Exam Fee - $100. By Endorsement: 1) Graduate from a college of veterinary medicine accredited by the AVMA, or holds a certificate of completion issued by ECFVG or PAVE. Official transcript must be sent directly to the Board by the veterinary college, showing graduation date and degree earned; 2) Applicant is currently licensed as a practicing veterinarian in another state. recognized by the AVMA. Note: The applicant\'s practice will be limited to the scope of the applicant\'s board certification.
