LSW Required Materials

The LSW only requires you to fill out 3 forms and submit a $50 application fee.

The forms are:

1) Application for Licensure and/or Examination 

4 pages, complete on your own 

2) Health Care Workers Charged with or Convicted of Criminal Acts (CCA form) 

1 page, complete on your own 

3) Certification of Education (ED) Form. Loyola fills out the second half of this form for you, entitled "School Official"

2 pages, filled out by you, completed by Loyola

 May 1st!

The School of Social Work will process your licensure form within

30- 45 days after your degree has been conferred. 

Approximate timeframe is mid-June or early July for Spring

graduates. Summer graduates can expect to receive their form in September.

All materials must be completed in black ink only. 

There are no refunds on application fees so be sure to submit all paperwork correctly.

Make sure to complete this form in its entirety

Once you have received the completed Certification of Education form in the mail from the SSW; open the

envelope. 

You will get a memo from the SSW with a reminder to add your social security number, and the proper

date.

Compile

all your materials; the application, CCA form and

Certification of Education form- mail them to the Illinois Department of Professional Regulation (IDFPR). The address is included in your

packet as well as in the memo that you will receive in the mail.

Get Approved to Sit for the Exam

Once the IDFPR receives your application and supporting documentation, the IDFPR notifies the Association of Social Work Boards (www.aswb.org) when you are approved to sit for the exam.

 Once an application is approved, you will receive a candidate handbook, a letter of approval, and a test code. (Turnaround time is approximately two-three weeks).

Exam Prep Information

The ASWB (Association of Social Work Board) offers study guides and practice tests.  The

ASWB Guide to the Social Work Exams (print edition $20 and the e-book edition $15)

is the only guide published by the creators of the social work examinations.

You can order the ASWB online practice test for $85.
