CALL T O ORDER John Byrd, Chairperson , called the meeting to order at 10:0 1 a.m. ROLL CALL Members Present: John Byrd, Public Member, Chairperson Veena Erinjeri, L.R.T. Jonathan Vono, R.R.T. Members Absent: Elizabeth Glasser, M.S.H.S., R.R.T. Elizabeth Weir, Public Member Staff Present: Kimmy Catlin, Board Support, Boards and Committees Section Kiran Parag, Analyst, Compliance Section APPROVAL OF AGENDA MOTION by Erinjeri , seconded by Vono , to approve the agenda as presented.

A voice vote was taken. MOTION PREVAILED Discussion was held. MOTION by Erinjeri, seconded by Vono, to suspend the Respondent ’s license for a minimum of one day. The Respondent's license will be automatically reinstated if within six months the Respondent has undergone a substance abuse/chemical dependency evaluation as directed by the HPRP, has entered into a disciplinary, non - confidential monitoring agreement with HPRP , and the HPRP has endorsed the Respondent as safe to practice or the HPRP has determined that the Respondent does not r

4 equire treatment monitoring. Upon a
equire treatment monitoring. Upon automatic reinstatement , the Respondent is placed on probation for a minimum of one day not to exceed six months. The Respondent shall comply fully with terms of HPRP monitoring agreement, if applicable. In addition, the Respondent is fined $250.00 to be paid prior to reinstatement. If the license remains suspended for more than six months the Respondent shall petition for reinsta tement.
