Appointment

1.First Appointment: My patient was an NSPT patient, so after completing the microscope exam, She was very motivated to improve her oral hygiene. The sight of multiple Amoebas was the motivating factor for her. She had taken microbiology in the past and was moderately knowledgeable about certain bacteria. Initial plaque index: 38% plaque free, 62% plaque found. Generalized plaque seen interproximally, around crown margins and in areas of malocclusion.OHI techniques demonstrated: Brushing using the circular and sulcular action, and flossing using the “C” to hug the tooth. We discussed the areas of remaining plaque and how to reach those areas.

She had previously used a linear motion to brush her teeth, and didn’t floss as regularly as she would like. She was interested in learning the new techniques and very willing to change her habits to improve her oral health.

2. Second Appointment

:

At the second appointment she explained that she had began a routine of flossing regularly and brushing more thoroughly using the techniques that were demonstrated. She explained that her gums felt more firm and she was experiencing less bleeding when brushing and flossing.Second plaque index: 81% plaque free. 19% plague remaining. Slight Plaque remaining, seen in localized interproximal areas of malocclusion. Revised OHI techniques demonstrated:

After reviewing the previously demonstrated brushing and flossing techniques. I noticed she needed a new technique to reach the areas of deeper subgingival pocketing. To help my patient achieve her goal of removing the amoebas and arresting the infection process, I decided to demonstrate the use of the Perio-aid around the margins of crowns and areas of deeper pockets.

We discussed the process of the NSPT, regular plaque removal and disrupting the bacterial colonies as a way to remove these bacteria.

Challenges of working with patients to change behavior to improve oral health:

I was really lucky to have a patient that was motivated and moderately knowledgeable about bacteria and the disease process. The microscope was the biggest motivator for my patient. When she saw the amoeba, she was very stunned. I didn’t have to do much more than show her the findings on the microscope, for her to realize the importance of behavior changes to improve her oral hygiene. Discussion:

After reviewing the plaque index score and location of plaque remaining, I was able to individualize my patients oral hygiene instructions. Most of her plaque was seen interproximally and in areas of

malocclusion. At the initial appointment we began with Brushing and flossing techniques. We discussed the disease progress when amoebas are present and the destructive nature of certain bacteria. At the second appointment, seeing improvement in her oral hygiene,

I demonstrated the use of the perio-aid for around the crown margins and to disrupt the bacteria in areas of deeper pocketing.Slide8

Completion of OHI Case Study

At the final appointment my patient explained that she was very appreciative of everything I had done and taught her. She described to me how the information and the findings on the microscope had changed her oral hygiene habits and routine.

I added this picture, with permission from my patient, because it is easy to forget that there are people connected to each mouth we work on. As my patient reminded me, ”Its

not just a set of chompers.” Sometimes its easy to forget the empathetic part of this career, and how much it can really change a persons life.
