By approving/validating the voucher you are:
Confirming that you are the HSD for the ASDB student
Are aware that the student is receiving services from ASDB through an institutional voucher

A State Placing Agency (SPA) places a student in a Residential Treatment Center (RTC) for care purposes.
The RTC sends an Initial application to the Home
School District (HSD)
within 5 days of entry.
The HSD approves the Initial application. The HSD has 60 days from the student’s date of entry to the RTC to complete a Home School District Education Application with the required statutory documents uploaded within the application.
ADE (ESS Vouchers Unit) has two reviewers evaluate the application/documents. If approved, the student’s funding is authorized for the remainder of the fiscal year (6/30/19).

"Home school district" means the
school district
in which the person resides who has legal custody of the child, as provided in section 15-824, subsection B. If the child is a ward of the state and a specific person does not have legal custody of the child or is a ward of this state and the child is enrolled in an accommodation school pursuant to section 15-913, the home school district is the
district
that the child last attended or, if the child has not previously attended a public school in this state, the
school district
within which the child currently resides.

Students currently attending any public education agency (PEA) in the state of Arizona. This also includes students who have
not been enrolled in a PEA for ten days or less
aligning with similar attendance guidelines from A.R.S. § 15-901.
Charters can only be an HSD if the student falls within the category of actively/recently enrolled.

The RTC representative is part of the team and needs to provide input into the evaluation as per A.R.S. §15-765 (see above).
Although a RTC representative must provide information for the MET report either in person or by phone,

ESS does not have any information pertaining to legal guardianship or any addresses for students in the educational voucher system. This information must be gathered through the appropriate state placing agency.

Yes
A GED is not considered a recognized high school diploma. If a student with a GED up to 21 years old wants to continue his/her education at the RTC, the student is eligible for funding.
