Eligible Applicants: Entity must be able to s erve pregnant women and postpartum w omen with Opioid Us e Disorder (OUD ) . Single C ounty Authorities (SCAs) are not eligible to apply under this funding anno uncement. PROJECT SUMMARY : DDAP i s a ccepting applications for provi d e rs who will provide pregnancy sup p ort services or make referrals to address the needs of pregnant and postpartum women with OUD . In addition, these community pro v iders wi ll facilitate or m ake referr a ls f o r outpatient Medicat ion - Assisted Treatment (MA T ). GIFA No. 1 9 - 01 – Pregnan cy Supp o rt Services 2 Part 1: GRANT INITI AT I V E GUID EL IN ES 1. OVERVIEW Pennsylvania has been experiencing the worst overdose crisis in recent h istory. As a result, on January 10, 2 0 18 the Gov ernor issued a st a te wide dis a st e r declaration to enhance state response, increas e access to treatment and save lives. Th e Governor then extended the disaster declaration to continue working to improve respo nse coordination f

2 or those suffering f r om substan ce us
or those suffering f r om substan ce use disorders. DDA P has i dentified a continued need for pregnancy support services to include care for pregnant w om en and postpartum women with OUD. 2. FUNDING AVAILABILITY Applicant s can submi t a grant applicat i on for an amo u n t up to $ 4 00, 000 annually . 3. P ROJECT DATES Con tingent upon the availability of state and federal funds and the receipt of qualified applicatio ns , the term of the project wil l commence on October 1, 2019 and end o n Septemb er 29 , 20 20 with t h e possibility o f one (1) year renewal option, if funding is av ailable . 4. ELIGIBLE APPLICANTS All eligi ble A pplicants must be able to serv e pregnant or postpar tu m women with OUD who present for support service . Applicants that cannot meet this req uireme n t will not be considered. 5. APPLICATION REQUIREMENTS Applicant must provide a narrative proposal to include the pr oject description and budget narrative whic h is a min imum of 7 pages and a maximum of 1 0 pages (single sided , Time s New R oman , fo nt siz e 12 ). Project Descri ption: All g rant applications must in clude , at a minimum, a detailed plan describing the management of the day - to - day operations as follows : a. T h e A p p lic ant shall provide a S ta tement of Ne e d for any pregnant or postpartum women with OUD . The Statement of Need s hall , at a minimum: • I nclude prevalenc e rates or incident data of OUD among pregnant and postpartum women and indicate the lack of current reso u rc e s to this population ; • I dentif y t he source of the data , and ; • D escribe the existing capac ity for MA T as it relates to the continuum of care . Appli cants may submit other valid data, as appropriate to support this applica tion. GIFA No. 1 9 - 01 – Pregnan cy Supp o rt Services 3 b. T he applic a ti on shall incl ude the total number of pregnant and postpartum women to b e served for the dur ation of the project wit h an anticipated n umber to be served monthly . Applicant shal l include the type s and amount s o f support services the A pplic a nt wi l l prov id e on a mont h ly b asis. c. S ervices must be provided either by the A

3 pp l icant or through M emoranda of
pp l icant or through M emoranda of Understandin g or Agreements (MOUs/MOAs) with partners . The services f or pregnant and postpartum w om en with O U D s h all includ e but not limited to the following : • Outreach and e ngagement in services provided by the Applicant ; • Coordinate with the local Single County Authority (SCA) for assessment and provision of MAT/OUD treat me nt services; • Substance abus e e ducation and relapse educa tion ; • R ef err al for necessary m e di cal, dental, a nd other health care servic es , including obstetrics , gynecology , diabetes, hyp ertension and prenatal ca re ; • Postpartum health care including at te ntion to depression and anx i et y disorders, and medication nee ds; • Specialized assessment, mon ito r in g, and referra ls for education, peer suppo rt , therapeu tic intervention and ph ysical safety; • Mental health care that i nc ludes a trauma - informed system of ass es sments and interventions; • P a re nting education a nd interventio ns to include child develop m ent ; • N u tr itional educat ion; • Home management an d life s k ills tra ining; • Education, testin g, counseling, and treatment of hepatiti s, HIV/AIDS, other STDs and related is su e s; • Employment readiness, a n d job training and placement; • Educ ation and tutoring assistan ce for ob taining a GED, high school diploma and bey ond ; • C hi l dca re during periods in which th e women is enga ged in therapy or in othe r necessary health or rehabilitative a ct i vities ; • Re covery suppo rt a cti vities such as grou ps, mentoring and co aching; and • Transpor t ati on and other n ecessary wrapar ound s er v ices. d. The A pplicant shall d emonstrate the capacity beg i nn ing on October 1, 201 9 to provide support ser vices to pregnant and postpartum women with OUD. A ll personnel, systems, procedures and resource s mu st be in place to perform o r assure an in - per so n, direct referra l for all of the serv i ce s under this grant . e. The Applicant mus t e xplain its process regarding points of entry for individuals seekin g preg na n cy support serv ices to include s creeni n g a nd ho w eligibility to the pr og ram is d e term ined . I n addition, p rovi de a ny eligibility restrictions whi ch m ay

The Applicant must explain the care coord ination process in the cat chment area or s urrounding area in which pren ata l care t o pregn ant a n d p ostpartum care women with OUD can rece ive s upport service s . g. T he A pplicant must explain coord inat ation of care and assure a n in - pe rson, d irect referral for 100 percent of the services to i nclude time fr ames for both . h. If the A pplicant w ill provide any serv ices through a par tne r under MOUs/MO As , the A pplicant must submit the MOUs/ MOAs wi th the appli cation. MOUs/MOAs will not count toward the applicati on project description and budget n arrat ive page limit . i. T he Applicant must explain a communication str ategy to facilitate coordination among th e obstetric p rovider, OUD t reatme nt provider, health system clinical staff an d child welf are ser vices . j. T he application shall include a listing of additiona l r esources and adjunct services cu rrently available in the com munity which will s tre ng then p r opos ed i nitiative and assist the individuals served . k. The Applicant must p rov id e an ex p lanation regarding k nowledge and adherence of t he Federal (C hild Abus e Prevention Treatme nt Act – CAP TA), state and county r eporting guide lines for sub stance - exposed infants an d how the p rog ra m will support th ese guidelines . B ud get Narrative /Detail : Th e budget narrat ive sh all be included in th e 1 0 - pag e maximum .. It must provide a justification for the cost s and f igures submi tted in the Budget T emplate , Appendix C - 1 . C omple tion of t he Budget Template , Append ix C - 1 is required i n ad di ti on to the 10 - pa g e maxi mum and m ust align with the Project Descri ption and Budget Narrati v e. EL IGIBLE PROGRAM ACTIVI T IES AND EXPENSES All expense s mus t be described and justifie d in the Bu d get Narrati ve/ De tail and correspond to the B udget T em plat e , Appendix C - 1 pr ov id ed . DDAP has a p proval of all f inal bu d gets . Each cost must be a separat e line item and inclu de the itemizatio n/ calculation. The following are example s of expenses which directl y suppor t th i s grant ini tiative funding application : a. Personnel Servic es ; b. C onsultant/Subc on t r acted Servic es , if applicable; c. Patient Services - includi ng su pplementa l or adj unct services to program participants such as transport ation and child care cost s pr ov ided in di re ct relation to project ser vices (i.e., tr avel to and fro m OUD treatment , relat e d medi cal appoi ntments or childcare during parti c ipants en gagement in th o se serv ices) ; d. Supplie s /Equipment – equipment under $5 ,000 nece ssa ry for the direc t executi on of the pr oj ect ; e. Travel ; f. Other Costs – as cont ained in the Budget Temp late , Appendix C - 1 ; g. Indirect (Facilit i es and Administ ration) Costs ; • Current a pproved federall y negoti ated ind irect cost rate or • Cost alloc ation plan or • I ndirect cost pr oposal wit h a defined in direct cost r ate . 7. I NELIGIBLE EXPENSES T he fo llowi n g are ineligib le e xpenses through this gra nt initiativ e funding application: a. Expenses related

Ca pital expend itures for purchases and/or im p rovements to land, building or equipme nt w hich material ly inc rea s e thei r value o r useful li fe , i.e., “ Brick s and Mor t ar ” ; c. Const ruction upgrad e s and remodeling ; d. Fixed Assets o f $ 5000 or above (fixed asse ts include furniture, equ ipme nt , computers , etc. whi ch have a life of m ore than o ne year ) ; e. Cas h paym ents dir ectly to t he int ended rec ipients of the services . 8. STANDARDS F O R FINANCIAL MANAGEMENT A l l grant a ward recipient s are r e q uire d to meet the standa rds and requirements for financia l management systems se t forth in 45 CFR part 75. Th e financ ia l systems must en able t he r ecipie nt to mai ntain recor ds that adequately identify t he sourc es of funds fo r federally assisted act i vities a nd the purposes f or whic h the award was used, including authorizations , obligati ons, unobligat ions balances, assets, li abilities , outlay s or e xpe n diture s, and an y program inc ome. The system must al s o e nable th e recipient to compare actual expendit u res or o utl ays with the a pproved budget for the award. These fu nds must retai n their aw ard - specific i dentity. The f unds may no t be comm ingled w ith st ate funds or other federal fu nds . R EPORTING REQUIREMENTS All grant applications m ust in cl ude a detail ed ou tline o f the grantee’s ability to collec t required in formation a nd submit repo rts in a timel y manner t o the Dep artment . Samp le r eports (which a re not included in the 10 - page narrat i ve maximum) must be incl u ded with your grant app l icatio n to i nclude the fo llowing information : a. Data Reports. The A pplicant shal l submit mo n thly reports beginning Octo ber 31, 2019 to DDA P to inc lude, at a minim um, the f ollowing in

formatio n : • Number of r e ferrals r eceived; • Number of w o me n admitted ; • Types an d n umber of services rece ived per individual ; • L eng th of s tay ; • Outc o me m easures as determi n ed by the prog ram but to includ e at le ast: o Aver age length of engag e ment ; o Perce nt a ge abstinent at 6 months post di scharge; o Post - d elivery engagement in current servic es or refer ral to other services ; o Reas on for di scharge ; o Other measur e s as determined/identi f ied in proposa l . • Comp letion of d ata el ements as required b y the Go v er nment Per fo r mance and Results Act (GPRA) . • I n addition, the gra nt A pplicant sha ll submit a ny addi t i onal fin ancial repo rts which are deemed nece ssary by DDAP or required fo r federal repor ti ng requirement s , and a ny other such report s as DDA P may requir e. b. Problem Identification Repor t. When the need arises or as requi red by the Depar tment, comp lete a P r oblem Id entificatio n R eport. This report sh all d e scribe problem areas a nd its impact . Th e rep or t shall list different choices wi th advantages a nd disadvantages of each and inclu de A pplican t recomm endations with suppor tin g rationale. 10. COMMONW EALTH USE Al l materia l submi tted shall be c onsidered the pro p erty o f the Com monwealth o f Pennsylvania and may be ret urned only at the Common we alth’s option. Notwithstanding any A pplicants’ c opyright de signations contained on p r o p o s a l s , the Commonwealth s hall h ave the right to make c opies and distr ibute p r o p o s a l s i n ter nal ly and to comply wit h public record or oth e r disc losure requirements unde r the provisions of any Commonwealth or United Stat es statute or regulation, or rule or order of any court of compe tent j urisdic t ion.
