If you do not have a MONTHLY record of meals served from the POS then you will need to enter each day’s meals served to get the monthly meal counts by school.Slide28

The bottom part of the Application is ONLY completed if there are changes in the schools listed above. For example, the district had three schools in 2015-16 and have added a new school for 2017-18, this information would be in this bottom section.Slide30

Child Nutrition Unit Process for Approval of Severe Need Application

After Child Nutrition Unit (CNU) receives application the total number of lunches will be compared to YTD for 2017-18If problems or questions the CND and/or Superintendent will be contacted by CNU

After approval the district

will receive letter from ADE CNU listing the schools that qualify for Severe Need breakfast

reimbursement for the 2019-2020 school yearSlide32
