47 y/o athletic male referred for persistent groin pain
Pain began 3 years ago while training for a marathon
Eventually diagnosed with FAI and adductor tendinopathy
Had arthroscopic
femoroplasty
, labral repair, and adductor lengthening 11 months ago
Intra-articular pain resolved post-op, but adductor pain continued
Aching, tight quality
0-7/10
Increased with walking, running, golfing
Better with rest
Tx
: activity modification, OTC meds, modalities, PT, US-guided corticosteroid injection, and adductor lengthening with limited improvement

Normal gait
TTP over left adductor origin
Full, symmetric, pain-free hip range of motion
Normal neurologic examination
Negative hip provocative maneuvers
Pain with resisted hip adduction with hips in neutral and 45 degree flexed positions
No pain with resisted hip flexion or abdominal muscle activation
Negative hernia examination
