��1 &#x/MCI; 0 ;&#x/MCI; 0 ;STATEMENT OF WORKBOMO Annex Lawn CareSCOPEPennsylvania Department of Transportation (PennDOT), Bureau of Management Operations (BOMO) Annex Facility,requires a Contractor to provide lawn care servicesto include approximately 10,square feet of mowing and weeding Mandatory PreScopeSite Meeting:The contractor will participate in a scopmeetingwith Site Meeting is mandatoryThis meeting will take place at the BOMOAnnex, 907 Elmerton Questions regarding the technical aspects of this bid should be directed to Colin McClenahen, Roadway Programs Manager,at 717.783 ��2 &#x/MCI; 3 ;&#x/MCI; 3 ;1. The grass bank alongthesouth and west side of the building and along the parking lot to the tree line shallbe cut to a 3inch heighttwice a year, May and again in Septemberof the subsequent yearThe brush and small trees shall be cut along the grass bank. The areaalongthe rearof the building will have all grass, brush and small trees cut from building edge to tree line. Shrubbery Maintenance Shrubbery maintenance shall be performed two times per year (April and early October) of the subsequent year.Shrubbery Maintenance shall include the inspection of each shrub, pruning and removing broken limbs as necessary and removal of dead or diseased shrubs.All shrubs shall be maintained in accordance with the requirements of the species.Shrubs shall be pruned behind sidewalks and to window height of the building.Shrubs shall be pruned, in accordance with the following guidelines, to selectively remove unwanted growth and encourage shrubs to grow or respond in a desired manner:Prior to starting work, the Contractor must propose a pruning plan for each cycle for the

approval of PENNDOT. b.All pruning must be accomplished as follows:(1)Hedges must be sheared or pruned, as appropriate, to maintain their established shape and appearance.(2)Remove dead, damaged or diseased wood; or structurally weak limbs that may cause a safety hazard or unsightly appearance.In the event the entire shrub/plant should die, Contractor shall remove to a minimum of 6 inches below ground level.In the event lawn area around the shrub/plant removed, the hole and/or any other surface damage caused by removal shall be filled with topsoil, fertilized, raked and seeded.In the event the hole is notin a lawn area, the hole shall be filled with topsoil and the surface prepared to match the surrounding surfaceiv.In the event new shrub is require, the Contractor shall provide a delivery order for approval. Once approvedby PennDot, the Contractor shall be responsible forthe order, delivery and planting of the shrub. (3)Remove ground cover, or any other extraneous vegetative growth from under or on the shrub/plant.(4)Prune and/or remove growth in front of windows, over entrance ways or walks, or where vision is obstructed.All pruning must be accomplished by personnel who are trained, experienced and otherwise qualified in proper pruning techniques. Verification of such training must be acceptable to PENNDOT prior to initiation of any pruning.All clippings and debris shall be removed from the site the same day and disposed of properlyAnnual Mulch Annual mulched for three flower beds is approximately 250 square feet. Mulch will beadded to the three flowerbeds in the front of the buildingin April/May(approximately 1 extra yard to cover mulching through the remainder of the season).Mulch will be applied at

Remove all leaves, branches and other debris that have accumulated through the course of the previous season.Contractor shall properly dispose of all leaves, branches and other debris.G.Fall/Winter Cleanup Fall cleanup shallbe completed in midNovember. The Department reserves the right to supply materials to be installed by the Contractor when it is deemed to be in the best interest of the Commonwealth. Contractor shall invoice PennDOT with supporting documentation for actual services performed. Contractor shall be paid at the unit price bidfor actual services performed and materials used during Lawn CareService.No invoices will be paid without an executed Attachment D, OS501, Confirmation of Servicesform. PennDOT is tax exempt and invoices shall not include Pennsylvania StateSales Tax.BID AWARD: ��4 &#x/MCI; 0 ;&#x/MCI; 0 ;Bid Award shall be to the lowest responsive and responsible bidder. Bidder must bid on all line items in the electronic bid submission. VI.CONTRACT QUANTITIES:The quantities are estimated only and may increase or decrease depending upon the needs of the PennD. VII.CONTRACT TERM:The contract shall commence Julyand terminate June 30, 202. Further, the parties hereto may agree to renew this agreement for up to four (4) additional consecutive annual terms, with a final termination date of June 30, 202, upon the same terms and conditions as set forth in the original contract.At the time of each renewal the Department and Vendor shall negotiate the contract unit price not to exceed a maximum of three percent (3%) over the unit price in effect of each renewal term.PennDOT shall submit a renewal letter at least 90 days prior to the termination date of the Contract and request the renewal
