Presentation Transcript

1. Optimisation of the Pre-term InfantRecording the measures on Badgernet| National Patient Safety Improvement Programmes1

2. Areas of Optimisation 1-3Place of birth: All babies delivered should be delivered in appropriate care setting for gestation (in a care setting with an NICU for singletons <27+0 weeks or <800gms, or all multiples <28+0 weeksMagnesium Sulphate: All women giving birth <30 weeks of gestation, should receive antenatal magnesium sulphate within the 24 hours prior to birthCorticosteroids: All women giving birth <34 weeks of gestation, should receive a full course of antenatal steroids no longer than 7 days prior to birth, and ideally within 24-48 hours The first 3 measures are all completed on the admission tab, maternal medical /antenatal history as for NNAP data| National Patient Safety Improvement Programmes2

3. Area of Optimisation 4Antibiotics: All women in established preterm labour <34 weeks of gestation should receive intrapartum antibiotics prophylaxis at least 4 hours before birthThe recording of maternal antibiotics is a new measure. Please tick if given during labour and the time given. Select Labour & birth on the admission tab and simply tick yes, no or unknown & add the time. Unknown will be counted as not being given. It is also important to ensure the type of delivery has been included as if ‘not in labour’ the measure is N/ANote – This field previously included oral as well. Currently checking with the national team if IV only is incorrect| National Patient Safety Improvement Programmes3

4. Area of Optimisation 5Optimal Cord Management: All eligible babies <34 weeks gestational age should not have their umbilical cord clamped until at least 60 seconds after birthWhen completing the delayed time it must be 1 minute / 60 seconds or more. Again on the admissions, labour & birth tab and unknown but be counted as not met.| National Patient Safety Improvement Programmes4

5. Area of Optimisation 5Normothermia: Babies <32 weeks gestational age should have a first temperature on admission between 36.5 - 37.5°C and measured within an hour of birthTemperature on admission should be completed on Badgernet as per NNAP. Please ensure the correct date is entered (a common error especially if birth is around midnight) and that it is within an hour of birth| National Patient Safety Improvement Programmes5

6. Area of Optimisation 7Maternal Breast Milk: All babies less than 34 weeks gestational age should receive maternal breast milk with 24 hours of birthRecording of first BM is a recently added field on Badgernet. On the main discharge page there is an option for UNICEF. Click on the arrow and it will take you to the screen below. Date & time of the first breast milk given must be recorded here for the optimisation data & BFI evidence. This should include first colostrum for mouth care, buccal, suckle at breast or EBM feed. Whilst the measure states <34 weeks it is good practice to enter information for all babies. It is suggested the field is completed and saved when given, not when completing the discharge. | National Patient Safety Improvement Programmes6
