17. Qualified Health PlanA qualified health plan is one that satisfies certain requirements with respect to minimum deductibles and maximum out-of-pocket expenses. Employees who enroll one or more dependents will be considered family and are subject to family deductibles and family out-of pocket-maximums.You are responsible for paying your covered medical expenses up to the deductible(s) stated in your health plan.All expenses, both medical and pharmacy apply to the plan deductible and out-of-pocket maximum. Provides comprehensive coverage for medical and pharmacy benefits (it is not a limited benefit plan.)Operates with a large network of doctors (the same as the Traditional Plan.) Can be coupled with a Health Savings Account.17

18. Health Savings Account (HSA)18A Health Savings Account (HSA) is a bank account that lets you put money aside, tax-free, to save and pay for qualified health care expenses. This money can be deducted from your paycheck, pre-tax.When you deposit money into an HSA, you won’t haveto pay federal income tax on:Deposits you into your HSA via payroll deductions.Money you spend from your HSA on qualified expenses.Interest earned from the HSA.Additional Benefits of HSAs:There is no “use it or lose it” rule.Unspent balances remain there until spent. Your account is portable. Even if you change jobs!You decide how much you will contribute to your account, up to the IRS limit (minus your Employer’s contribution, if they make one)You decide when you want to use your savings to pay for, or be reimbursed for, qualified medical expenses.

19. Qualified Medical Expenses19Expenses that qualify for payment or reimbursement from your HSA tax-free are defined by the IRS. *Dental and Vision will not count towards your medical deductible. For more information about qualified medical expenses, visit the IRS website at www.IRS.gov.Doctor’s office visits, hospital services, urgent care, virtual visits. The following is a short list of some products and services in this category:Prescription medicationsDiabetic supplies including insulin, meters and test strips. HSA EligibilityIf you have a qualifying health plan on the first day of any month, you may be eligible to contribute to an HSA if:You are not covered by any other non-HDHP health plan, such as a spouse’s plan, that provides any benefits covered by your qualified plan. You are not enrolled in Medicare.You do not receive health benefits under TRICARE.You have not received Veterans Administration (VA) benefits within the past three months, except for preventive care. (If you are a veteran with a disability rating from the VA, this exclusion does not apply.)You cannot be claimed as a dependent on another person’s tax return.You are not covered by a general purpose health care flexible spending arrangement (FSA) or health reimbursement arrangement (HRA). Alternative plan designs, such as a limited-purpose FSA or HRA, might be permitted.If your circumstances change and you are no longer eligible to contribute to an HSA, you can keep the account as long as you like and use it to pay for qualified medical expenses income-tax-free.20

Contributions Limits21Annual contribution limits are set by the IRS.Individual Limit - $3,550Family Limit - $7,100Note: the tax-free contribution limits include any employer contributions to your HSA.2020 Annual IRS Contribution Limits:Additional Funding:If you are 55 or older, you can make “catch-up” contributions, meaning you can deposit an additional $1,000. If your spouse is also 55 or older, he or she may establish a separate HSA and make a “catch-up” contribution to that account.Excess Contribution Rule:Your excess contributions are subject to standard income tax rates plus a 6% penalty.

23. Claim and billing process23Paying for your doctor visit with a health savings account (HSA).Doctor visit.Some network doctors can instantly submit your claim online. The doctor’s office may ask you to pay some or all of the bill before you leave. Most of the time, claims will be submitted to UnitedHealthcare before the doctor will bill you.Claim submission.UnitedHealthcare reviews the claim to see if the service is covered by your plan and if your care was from a network doctor or facility. If the service is covered, and you used a network doctor or facility, UnitedHealthcare applies the network discount.Preventive Care - The claim will be paid 100 percent. You will not have out-of-pocket costs.Non-preventive Care -Your doctor will bill you directly for payment.Payment.You can choose to pay the bill using the funds in your HSA or pay another way (cash, credit card, check). Your claim detail will appear on myuhc.com under Claims & Accounts. You’ll receive a health statement by mail or email.

Investing with your HSA24Once your HSA reaches the investment threshold (typically $2,000), you may choose to invest a portion of your HSA dollars in mutual funds — just like you would with a 401(k). You can choose from a wide variety of mutual funds at optumbank.com. Any investment earnings such as interest or dividends are income tax-free.Have an unexpected medical expense?No problem. Whenever you need additional money to pay for qualified medical expenses, you can easily transfer your investment funds back into your HSA.

25. Don’t forget:25Keep all medical receipts. You can save paper copies or store them online. The convenient online receipt vault allows you to upload images of receipts for qualified medical expenses and then store them within your HSA.Track your health care expenses with the expense tracker worksheet on optumbank.comRetain all tax documents you receive from Optum Bank for filing your tax returns and maintaining your records.When you set up an HSA, it is important that you also select a beneficiary.This will ensure that your HSA money is immediately available to yourbeneficiary upon your death.
