Serv ice s Page 3 of 11 G RANT INITI AT I V E GU I DELI NES 1. O VERVIEW Pennsylvania has been experiencing the worst overdose crisis in recent h istory. As a result, on Janua r y 10, 2 0 18 the G ov ernor issued a st a te wide dis a st e r decl aration to enhance state response, increas e access to treatment and save lives. Th e Governor then extended the disaster declaration to continue working to improve respo nse coordination for those suf f er ing f r om subst an ce use disorders. DDA P has i dentified a continued need for pregnancy support services to include care for pregnant w om en and postpartum women with stimulant or opioid misuse issu es . 2. FU NDING AVAILABILITY Applicants can submit a g r ant appl ication for an amount up to $ 4 00 ,000 . Aw ard amounts an d number of awards shall be dependent upon number of applications received , meeting the defined criteria of this solicitation, amount of funds being requested, and the amount of availa b le fu nds to be aw arded. 3. P RO JECT D ATES Continge nt upon th e a vailability of state and federa l funds and the receipt of q uali fi ed a p plications, the term of the project will comme nc e on December 1 , 2 0 20 and end o n September 29 , 20 2 1 wit h the possibility of one ( 1) year renewa l option, if f und i n g is available . 4. ELIG IBLE A PPLICANT S All eligible Applicants shall be able to serve pregnant or postpartum women which have stimu lant or opioid misuse is s ues w ho present for support service s . If either a stimulant or opioid m isuse pro bl em (history) e xists concurrently with ot her substan ce use , all substance use issues may be addressed . Applicants that cannot meet t hi s requirement will not be co nsidered. 5. APPLICATION REQUIREMENTS Appl icant shall provide a narrative proposal to include t he project des cription that includes th e information d e linea ted below and budge t narrativ e which is a min im um o f 5 pages an d a m axi mu m of 7 pages (single sided , Times New Roman, font size 12 , with narrati ve pages numbered ). Project Description: All grant a pp lications sh all include , at a minim u m, a detailed p l an desc ribing the delivery o f pregnancy support s e rv ice to ind iv iduals with stimulant or opioid misu se i ssues . a. T h e A p p lic ant shall provide a S ta tement of Ne e d for any pregnant or postpartum women with stimulant or o pioid misuse issues . The S tatement of Ne ed s hall , at a minimum: GI FA 2 0 - 0 5 – Pregnan cy Supp o r t Serv ice s Page 4 of 11 • I nclude prevalenc e rates or incident dat a of OUD and stimulant use among pregnant and postpartum women and indicate the lack of current reso u rc e s to this population ; • I dentif y t he source of the data , and ; • D escribe the existing capac ity for MA T as it relates to the continuum of care . Appli cants ma y submit other valid data, as appropriate to support t his applica tion. b. T he applic a ti on shall incl ude the total number of pregnant and postpartum wom en to b e served for the du r ation of the project wit h an anticipated n umber to be served monthly . Applic an t shal l include the type s and amount s o f support servi ces the A pplic a nt wi l l prov id e on a mont h ly b asis. c. S ervices shall be provided ei ther by the A p p l icant or through M emoran da of Understa ndin g or Agreements (MOUs/MOAs) with partners . The services f or pr egnant and postpartum w om en with O U D s h all includ e but not limited to the following : • Expa nsion of cu rrent GIFA 19.01 projects to includ e ad ditional c ounties ; • Outreach and e nga gement in serv ices provided by the Applicant ; • Coordinate with the local Singl e County Authority (SCA) for assessment and provision of MAT/OUD / s timulant misuse a nd use treat me nt services; • Substance abus e e ducation an d relapse educ a t ion ; • R ef err al for necess ary m e di cal, d ental, a nd other health care servic es , including obstetrics , gyn ecology , diabetes, hyp ertension and prenatal ca re ; • Postpartum health care i ncluding at te ntion to depression

and anx i et y disorders, and med ication nee ds; • S pecialized assessment, m on ito r in g, and referra ls for education, peer suppo rt , therapeu tic intervention and ph ysical safety; • Mental health care that i nc ludes a trauma - informed sy stem of ass es sments and interventions; • Pr ovision of evidence - ba sed interventi ons, or referral to evidence - based inter venti ons; • P a re nting education a nd interventio ns to include child develop m ent ; • Contingency Management ; • N u tr itional educat ion; • Home management an d life s k ills tra in ing; • Education, testin g, counseling, an d treatment of hepatiti s, HIV/AIDS, othe r STDs and rel ated is su e s; • Employment readiness, a n d job training and place ment; • Educ atio n and tut oring assistan ce for ob taining a GED, high school diploma and bey ond ; • C hi l dca re during periods in which th e women are enga ged in ther apy or in othe r necessary health or reha bilitative a ct i vities ; GI FA 2 0 - 0 5 – Pregnan cy Supp o r t Serv ice s Page 5 of 11 • Re covery suppo rt a cti vities such as gr ou ps, mentorin g and co a ching; and • Transpor t ati on and other n ecessary wrapar ound s er v ices. d. The A pplica nt shall d emonstrate the capacity beg i nn ing on Decemb er 1, 20 20 to provide supp ort ser vice s t o pregnant and postpartum women with stimulant and opioid misuse issues. A ll personnel, systems, procedures and resource s shall be in place to perform or referra l for all of the serv i ce s under this gra nt . e. The Appl ic ant shall e x plain its process regarding points of entry for individuals seek in g preg na n cy support s erv ices to include s creeni n g a nd ho w eligibility to the pr og ram is d e term ined . I n addition, p rovi de a ny eligibility restrictions whi ch m ay ex i st . f. The Appl ic ant shall e x plain the care coord ination process in the cat chment area or s ur rounding area in which pren ata l care t o pre gn ant a n d p ostpartum care women with stimulant or o pioid misuse issues can rece ive s upport service s . g. T he A pplicant shall explain coord in at ion of c ar e and assure a referral for 100 percent of the service s to i nclude time fr ames for both . h. If the A pplicant shall pro vide any serv ices through a pa r tne r under MOUs/MO As , the A pplicant shall submit the MOUs/ MOAs wi th the appl i catio n. MO Us /MOAs w ill not count toward the applicati on pro jec t description and budge t n arrat ive page limit . i. T he Applicant shall explain a communi cation str ategy t o facilitate coordination among th e obstetric p rovider, stimulant or opioid misuse t reatme nt pro vide r, h ealth s ystem clinical staff an d child welf are s e r vices . j. The Appl icant shall contain an e xplanat io n how it will gather data and submit reports r equired by se ction 9 . If applicable , spe cify an d ju stify any ad ditional measures or i nst rument s th e A p p li c ant plan s to uti lize . Budget Nar r ative : Th e b udget narrative sh a ll b e inc lude d in the 7 - page ma ximum . Th is budget narrative shall provide ac curate bu dget d etail s and clearly explai n the relationshi p betw een the budgeted costs and the p ropose d projec t . T he bud ge t narrative shall p rovide a ju stifi cati o n for all t he costs a nd figures submitted i n th e Bud g et Tem plate , Appe nd ix C . Budge t Template: Completion of the Budg et Template, Appendix C is required in addition to the 7 - page maximum and sh all alig n w ith t he Project Description , and Budget Narr ative . 6. E LIGIBLE PROGRA M ACTIVI T IES AND EXPENSES Al l expenses shall be described and justified in the B udget Narrative and cor respond to the Budget Templa te, Appendix C provided. DDAP has app roval of all f inal bu dgets . Eac h cos t shall be a separa te li ne item and include the itemization/ calculation. The following are examples of expenses that directly sup port this grant initiat ive funding application: a. Personnel Services: This budget category shall identi f y each posit ion by j ob t itle, hourly rate, and number of hours allocated to the pro ject. Fringe benef its are to be shown as a separate line item by perce ntage and shall include a detailed listing of t he benefits being covered. b. Consultant Ser vices: This b u dget ca tegor y shal l iden tify the services to be provided by each consultant inclu ding hourly rate and number of hours to be utilized under this grant. Id entify services to be p rovided. Do not name the consultant(s) that will be retained. c. Sub contra ct Serv i ces: Th is bu dget c at egor y shall identify t he se rvices to be provided by each subcontractor under this grant. Identify services to be provided. Do not nam e subcontractor to be r etained. d. Patient Servic es: This budget category shall reflect fun ding d edicate d for pa tient servi ce s. Transportation costs to connect an individual to treatment within the Commonwe alth of Pennsylvania is an allowable expense. Contin gencies may be used to reward and incentivize stimulant treatment compliance with a maxim um con tingency value being $15 per contingency. Each patient may not receive contingencies totaling more than $75 per year of his/her treatment. GI FA 2 0 - 0 5 – Pregnan cy Supp o r t Serv ice s Page 7 of 11 e. Equipment: This b udget categ or y shall reflect the actual or projected cost of any equipment which is $5000/ unit or gr eat er ne cessary for the direct execution of the project . I dentify equipment, the quantity an d uni t cost. f. Supplies: This budget category shall reflect expected costs fo r general office supplies. Identify su pplies in general terms. Equipment item s un der $5 000 - unit cost s hall be li st ed h ere. All personal comput ers under $5000 - unit shall be conside red as office suppl ies . g. Travel: Thi s budget category shall include anticip at ed expenditures for travel including mileage, hotels and subsistence for staf f pe rson ne l. Milea ge betw een sites for s taff personnel is an allo wable expense. h. Other Costs: This bu dget category shall be used for a nticipated expenditures that do not fit in to any of the other budget categ or ies such as postage, t elephone, printing, out reac h, a nd in direc t costs (overhead, general and administrative). Indirect rates cannot exceed the pro viders Federally approved indirec t cost rate schedule. There are no caps to t he Federal rate. If the provid er doe s not have a Feder ally approved indirect rate , th en th ey ma y use t he de minimis rate which is 10% of modif ied total direct costs . In the descri ption area under “Other Cost” include the % that the rate reflec ts, identif y the budget categories to w hich t he rate was applied, and l

Fundin g shall not be used to supplant/repla ce federal, state or local funds that would otherwise be availab le to provi de for program - related servi ces. DDAP funding is to be used in addit ion to oth er f unds t hat are made av ailable for services. Awards will be th rough a grant agreement between the A pplicant and DDAP. The a n ticipated Gra nt Agreemen t te rm is December 1 , 2020 to September 29 , 2 0 2 1 . The ov era ll 10 - month bu dge t for the application shall not excee d $ 4 00, 0 00 and shal l b e paid by monthl y invoices on a c ost reimbursement basis. C onstructi on up grades and re modeling ; GI FA 2 0 - 0 5 – Pregnan cy Supp o r t Serv ice s Page 8 of 11 d. Fixe d As sets of $5000 or above (f ixe d assets inclu de furniture, equipment, computers, et c. which have a life o f more tha n one year); e. Cas h pay me nts d ire ctly to th e int en de d re c i pients of the se rvices ; f. E xp en s es related to any star t - up costs ; and g. DD AP is no t li ab le f or co sts incu rred prior t o t he official st ar t date of the award. 8. STANDARDS F OR FINANCIAL MANAGEMENT All grant a ward recipients are requi red to meet the standards an d requireme n ts for financi a l management sys t e ms set fo rt h i n 45 CFR P art 75. The finan cial syst em s shal l enable the rec ipient to mai nta in re cor ds that adequately identif y the sources of funds for federal ly assisted acti v ities and the purposes f or which th e aw ard was u sed, including a ut horizations, ob ligations, u nob ligations ba lances, a s sets , li abili tie s , ou tl ays or expe ndi tures, and an y p rogra m i ncome. The system shall als o enable the recipient to compare actual expenditu r es or out lays with the a ppr oved bud get for the award. These fu n ds shall retai n th eir award - sp eci fic identi ty. The fun d s may not b e c om m i n gled with state fu nds or othe r f ed eral fun ds. [“Commingling funds” t ypically means deposi ting or recor ding funds in a g eneral ac count without t he ability to i dentif y each specific so u rc e of funds fo r any expend it ure .] Nor sh all thes e f u nd s su pplan t, in a n y wa y, current s ta te or feder al fu nds f or already existing services . 9. R EPORTING REQUIREMEN TS All grant applications shall include a detai l ed out line of the Applicant ’ s ability to collec t r equired in fo rm ation and s ub mit report s in a t imely m anner to the De pa r tmen t . Sample r epo rts (whi ch are n ot i ncl uded in the 7 - page narrativ e maximum) shall be included with y our grant applic a tion to i ncl ude the foll owing infor mati on : a. D ata Reports. The A pplicant s hal l submit mo n thly reports beginning Jan uar y 1, 20 21 to DDA P to inc lude, at a minim um, th e f ollowing in formatio n : • Number of r e ferrals r eceived; • Numbe r of w o me n served by age, race, and ethni city; • Types an d n umber of services rece ived per individual ; • Outc o me and process m easur e s as determi n ed by th e prog ram but to includ e at le ast: o Percenta ge of participants active in program at 3 , 6, and 9 months post initi ation of servic es; o Perce nt a ge of partic ipants reporting a r e d uction in su bstan ce use at 3, 6 , and

Applicant shall maintain a n ong oing recor d of n umber of GPRA intake s , follow - up, a n d disch arg e s complet ed for their pr o je ct. • I n addition, the gra nt A p plicant sh a ll submi t a ny addi t i onal fin ancial repo rts which are deemed nece ssary by DDAP or required fo r f ederal repor ti ng requirement s , and a ny o ther s uch repor t s as DDA P may requir e. b. A nnual R epo r t. T he Ap pl i cant shall s ubm it a n an nual report by O c tober 15 of ea c h calend a r yea r during the gr ant agreement . T he re p ort sh all include the following information : • Overall S ummary : This section shall be a b rie f descrip tio n of the project, counti e s served, i n di vidual s s er ved, key c han ges in p rogr amming and /or staf fing, and other in f o rmat ion the Appl ica nt believe s is import a nt for DDAP to un derstand regarding the project. • Outc omes: This section sh all list w hat me asurable out comes did the Applicant e s t ablish f o r this p r oje ct, what ind ica tors did the Applicant use to mea sur e p e rformanc e , wha t extent did th e project achie ve the projected outcome s , if multiple projects exist, repor t outcom es for each i ndividual pro jec t. • Barrie rs: This section shall disc u ss the ch a l l enges th e A pplicant fac ed d uring t he p roje ct and what ac tions did the Appl i c ant take to addr ess these challenges. • To t al Expenditures : This section shall provide a final e xpend iture report for the proje ct dur ing the p roc eeding federal fiscal ye a r . • Future Im p licatio ns : This section s hal l indicate what i mpact does the Appli ca nt thin k thi s project has had to date, what are the le s sons the Applican t learned from und ertaking this proj ect, what are s ome succes s stor ies and w hat can the Applicant do to imp rove t he proj e ct for futur e years. c. P roblem Iden ti fi catio n Repor t. When t he ne ed a rises o r as required b y th e D epartment, complet e a Probl em Identi fication Report. This report shall describe pro ble m areas an d its imp act. T he report shall list differ e nt choice s w ith a dva ntages a n d disadva nta ges of each a nd incl ude App licant rec om men dati ons w it h supporting ra t ion a le . O VERVIEW Pennsylvania has been experiencing the worst overdose crisis in recent h istory. As a result, on Janua r y 10, 2 0 18 the G ov ernor issued a st a te wide dis a st e r decl aration to enhance state response, increas e access to treatment and save lives. Th e Governor then extended the disaster declaration to continue working to improve respo nse coordination for those suf f er ing f r om subst an ce use disorders. DDA P has i dentified a continued need for pregnancy support services to include care for pregnant w om en and postpartum women with stimulant or opioid misuse issu es . 2. FU NDING AVAILABILITY Applicants can submit a g r ant appl ication for an amount up to $ 4 00 ,000 . Aw ard amounts an d number of awards shall be dependent upon number of applications received , meeting the defined criteria of this solicitation, amount of funds being requested, and the amount of availa b le fu nds to be aw arded. 3.
