Primary care networks: development and support

Provides support for local communities, building on learning from the existing models;Provides advice on the key areas commissioners and practices might consider in establishing primary care networks locally;Sets out the vision for networks, core characteristics, care models at the heart of primary care at scale;Identifies key enablers that underpin effective development of networks;Shared widely for comments and we’d like your views – released in autumn. Primary care networks: a reminder 2 A WORKING DEFINITIONPrimary care networks enable the provision of proactive, accessible, coordinated and more integrated primary and community care improving outcomes for patients. They are likely to be formed around natural communities based on GP registered lists, often serving populations of around 30,000 to 50,000. Networks will be small enough to still provide the personal care valued by both patients and GPs, but large enough to have impact through deeper collaboration between practices and others in the local health (community and primary care) and social care system. They will provide a platform for providers of care being sustainable into the longer term.

We know that :We are starting from a great position, with many already part of a network… but we have the opportunity to really support the journey to PCNs.There will be multiple ways to facilitate development and we will have some choices to make.In order for development of PCNs to be successful we will need to build momentum and ownership.Co-production will be important if we want development to be successful and sustainable. We have good experience to build from.PCNs need to be locally driven.Supporting the development of PCNs 3

4 We have some great insight into what great PCNs look like and how they will develop

Primary Care Network Development : what could support look like? An OD programme aimed at supporting PCNs move across the PCN maturity matrixBuilt from learning and experience about what works well in developing PCNsFocussed on development in place and tailored to respond to local needPlace based Enabling mature PCNs Specific inputs and products on areas that need a single more technical response or enablement to enable PCNs to develop. Products could include - ‘how to guides’ for example in approaching estate or – policy intervention to unblock barriers Technical support National leadership development programme for primary care leaders Focussed on primary care leaders for today and tomorrow Network of networks? Primary care leadership programme 5

This could include: Support for development of network leaders which specifically seeks to build leadership from a broad constituency including GPs (partnered and salaried/sessional), senior nurses and AHPs, pharmacists, and managers (practice, CCG and community services). External coaching, facilitation, change management, contracting and funding models, service redesign and improvement support delivered direct to networks and through peer network activities across an STP/ICS and/or region. Local network development e.g. diagnostic and transformation plan development sessions, rapid improvement cycle implementation.Engagement and peer learning events for networks across STPs/ICSs and regions. Subject matter expertise e.g. for network-level workforce analysis and redesign, population health, personalised care and patient engagement.Technical support e.g. on estates, digital, workforce development etc.6

Feedback from previous events… Flexible and not prescriptive – enable grass roots upProtected time and the headspace to think and plan/developInvest in longer term transformation – what happens after transformation funds? Clarify funding for PCNs including enabling development and freeing up time and capacityShare learning for free – more case studies and real examplesPeer support and sharingEmpower PCNsSet clear expectations – sensible approach to measuring PCN successAsk staff what training is neededPositive communications campaign – communication aimed at patients and the publicThis type of OD will take time - don’t rush and release time for practices Need help to outline benefits from different perspectives - practice/ system/patients Data – share short cuts – data sharing agreements Care home schemes Working on engagement to generate ideasRemoving fear of consequencesIM&T and online triageSupport for less mature groups of practices Next steps , vision and outcomes - don’t shy away Use PCH vanguard experience and see if we can set our own up Share step by step experience and case studies Resolution of technical barriers Service directories with clarity on offer live and working STP estates strategies - maximise for PCNs Benchmarking and examples - big and smallCreate head space 7What could support look like for you? What next for you?

Webinars Webinars to provide further information on the emerging PCN development offer, based on the feedback that has been received through the events, will take place on the following dates:Thursday 8 November 2018, 12.00pm – 1.00pmMonday 26 November 2018, 1.00pm – 2.00pmDetails available at www.england.nhs.uk/pcn 8

What do you think about the reference guide? Good / bad / indifferent. What would make it more useful for you? Over to you… Primary Care Network Development - What could support look like? We’ve had some ideas on development for PCNs but what support do you think needs to be in place for PCNs as they set up and mature? How could that support be offered to be most useful for PCNs? Thoughts on a specific leadership development programme for PCN leaders? Primary Care Networks - What next for you?Reflecting on today what are your next steps? What do you need to help you do this? – at national, regional and local level. 9
