The rubric details what goes into each of the numeric scores for every portion of the inspection. The rubric is an excellent teaching / training tool when discussed with the entire team. CUSTODIAL INSPECTION RUBRIC CUSTODIAL INSPECTION RUBRIC (Revised 10/14/13)     4-SUPERIOR: Highest score possible four out of a possible four points.   Floors: All floors are neat and clean with no loose debris present. Carpet is spot free and tile floors have an overall consistent appearance and luster. This covers all floors in all areas. Bathrooms: All fixtures are clean with no buildup present anywhere, no odor is present, all supplies are filled properly, vents and stall walls are clean/dust free and trash is emptied with proper liner in place. This covers all bathrooms in all areas. Classrooms: All trash cans are emptied with liner in place, ledges/window sills/file cabinets/vents and air returns are all dust free. Light Bulbs and Ceiling Tiles: Scant few - light bulbs burned out, lenses broken and ceiling tiles broken, missing, stained. Windows and Door Glass: All windows and door glass are clean and streak free. Locker Room and Gym: Gym floor is clean and free of loose debris, PE office and bathroom are completely clean, any weight room equipment is clean/dust free and lockers are dust free. Auditorium: Free of loose debris on floor and stage/dressing rooms are swept/mopped and the restrooms are cleaned. Cafeteria: Floors are swept/free of all loose debris/free of spots and spills, trash cans are emptied and all tables are wiped clean and ready for use. Kitchen: Bathroom cleaned, hand sinks are completely clean and free of buildup, all ceiling vents are dust/dirt free. Building Exterior: Free of trash and debris on grounds, trash cans are emptied, grass consistently mowed/edges string trimmed, weeds removed from sidewalks/parking lots, mulch is present and properly spread in beds/playground, shrubs are trimmed, and the loading dock is neat/clean/organized and no evidence of wastewater dumping present. All grounds equipment is in good repair/ready for use and the tractor shed is neat/orderly with all gas cans in their proper place. Custodial Closets: Floors are neat and clean, sinks have drain in place and are clean with no odor, equipment is in good repair and ready for use, 100% of all cleaning products are properly diluted and labeled. No evidence of toilet bowl brushes being used to maintain sinks or drinking fountains. No non-FCPS chemicals present on any custodial carts or in any custodial closets. Lead Custodian Administration: Crisis manual and the MSDS book are updated and readily accessible, 100% of the IPM and utility reports have been completed and turned in on time to correct department, all closets have schedules/ assignment maps and emergency contact phone numbers posted.

3-GOOD: Second highest score with three out of a possible four points.   Floors: Majority of floors are neat and clean with no loose debris present. Carpet has few spots present and tile floors are in need of burnishing to improve luster. This covers all floors in all areas. Bathrooms: Majority of fixtures are clean with no buildup, presence of odor possible in some areas, some supplies might not be filled, some dust present in few areas on vents/stall walls and some trash might be in cans. This covers all bathrooms in all areas. Classrooms: Majority of the rooms have had trash emptied/liner in place, ledges/window sills/file cabinets/vents & air returns are dust free but some issues in a few rooms. Light Bulbs and Ceiling Tiles: Minimal number of light bulbs or ceiling tiles in need of replacement. Some stains present. Locker Room and Gym: Some dust/debris present on floors, minimal dust present in PE office, some weight room equipment in need of wipe down/dust present, minimal dust present on top of lockers. Auditorium: Some dust/debris present on floors and stage/dressing rooms could be in need of dusting. Cafeteria: Some loose debris could be present on floors, presence of minimal amount of spots/spills, trash emptied, tables are wiped and ready for use. Kitchen: Bathroom cleaned, majority of hand sinks are clean and free of buildup, most ceiling vents are dust and dirt free. Building Exterior: Very minimal amount of debris/trash on grounds, trash cans are emptied, grass consistently mowed but potential for string trimming and/or weed removal issues, mulch is present but might need spreading, shrubs are trimmed, some debris/skids/wastewater dumping could be present. All grounds equipment is in good repair/ready for use but shed might be in need of organizing or gas cans not properly stored. Custodial Closets: Majority have neat and clean floors, sinks have drains in place and are odor free, some equipment might be in need of organizing. Anything less than 100% compliance with the following will result in a “1” for that specific line item: proper labeling and dilution of all chemicals, none of the custodial carts show evidence of toilet bowl brushes being used to maintain sinks / drinking fountains, and no non-FCPS supplied chemicals are present in closets or on custodial carts. Bathrooms: Some of the fixtures are clean with no buildup, presence of odor possible in some areas, some supplies not filled, dust present on air vents/top of stall walls and some trash present in cans. This covers all bathrooms in all areas. Classrooms: Some of the rooms have had trash emptied/liner in place, ledges/window sills/file cabinets/vents and air returns are dust free but issues exist in quite a few rooms. Light Bulbs and Ceiling Tiles: A number of light bulbs or ceiling tiles are in need of replacement. Stains present on a number of ceiling tiles. Locker Room and Gym: Loose debris present on floor, some dust present in PE office and/or on top of lockers and some weight room equipment in need of wipe down/dust present. Auditorium: Dust/debris present on the floors in some areas and stage/dressing rooms are in need of dusting. Cafeteria: Some loose debris present on floors, spots/spills present, trash could be present in cans, some tables could be in need of wiping down to be ready to be used. Kitchen: Bathroom could have cleaning issues, hand sinks could have build up but are wiped clean, dust present on ceiling vents and floor could be in need of machine scrubbing. Building Exterior: Some trash/debris present on grounds, trash present in few cans, grass being consistently mowed but string trimming and/or weed removal issues, mulch is thin and needs to be ordered, shrubs trimmed, some debris/skids/wastewater dumping present. 1-POOR: One out of a possible four points. Lowest possible score.   Floors: Majority of the floors have loose debris present/some appear to be undone completely. Spots/gum/ traffic lane soiling present on carpeting and tile floors have issues with ongoing maintenance. This covers all floors in all areas. Bathrooms: Majority of bathrooms have significant issues with fixtures that have not been cleaned and build up is present, odor could be present, trash present in cans, extreme dust present on top of stalls/vents and supplies in numerous areas are not filled. Classrooms: Majority of the rooms have issues with trash in cans, significant issues with dusting of flat surfaces/vents/ corners. Some areas appear to be undone with minimal service provided. Light Bulbs and Ceiling Tiles: Large number of light bulbs burned out and large number of ceiling tiles are stained or broken and in need of replacement. Locker Room and Gym: Loose debris on all floors, trash not emptied, extreme dusting issues in PE office and tops of lockers, weight room equipment has extreme dust present and some appear undone. Auditorium: Loose debris on floors throughout, dust present on stage/dressing rooms in heavy amounts and appears as though some areas are undone. Cafeteria: Large amount of loose debris/food present on floor, spots/spills present, trash could be present in cans, some tables could be in need of wiping down to be ready for use. Kitchen: Bathroom/hand sinks could be completely undone, heavy dust buildup on air vents, floors in need of machine scrubbing to remove buildup. Building Exterior: Trash/debris present on ground, trash present in few cans, grass not being consistently mowed/string trimming and or weed removal issues, mulch is thin with weeds present, shrubs could need trimmed and loose debris/skids/wastewater present on loading dock. Ground equipment not being properly maintained on an ongoing basis and is not ready to be used. Shed is in need of organizing and or gas cans not stored properly. Custodial Closets: Most are in need of cleaning/organizing, drain covers missing/odor present and carts are not ready for use. Anything less than 100% compliance with the following will result in a “1” for that specific line item: proper labeling and dilution of all chemicals, none of the custodial carts show evidence of toilet bowl brushes being used to maintain sinks / drinking fountains, and no non-FCPS supplied chemicals are present in closets or on custodial carts. Lead Custodian Administration: Crisis manual or MSDS book are not readily accessible, IPM/utility reports are not being completed consistently and are not being turned in on time to correct department. Most of the closets do not have schedules/assignment maps and emergency contact phone numbers posted.

The current 2013-2014 inspection form was handed out today. Share copies with the entire team Inspection form can be found on Form Finder under Operations NO changes on this years inspection form! Inspection Form 2013-2014

Inspection FAQ’s 1) Q: We were short staffed last night? A: Please use the “sub request form” each and every time someone is absent. If we know you were short handed we won’t inspect your school. 2) Q: Why was I marked down for stained ceiling tiles? A: If you have stained ceiling tiles due to a roof / pipe leak, and you have a work order in to Maintenance, tell us when we get there. 3) Q: Why did I get a 1 for “Chemicals properly stored, labeled and mixed”? A: Having improperly diluted or un-marked chemicals / spray bottles is an OSHA violation and adversely impacts their effectiveness. This is graded as a 4 or 1.

10. Complete properly and turn in your IPM reports on a monthly basis. (Checking “no traps necessary” or turning in several months worth at a time is not what the Maryland Department of Agriculture is looking for) 9. Make sure all of your custodial closets have copies of the custodial assignment maps and emergency contact lists. 8. Clean the salt out of the salt spreader after each and every use. (This not only prepares it for the next use but protects the equipment over the long term) 7. Make sure your gym floor has been completely dry mopped. (including the corners) and no spills or gum are present. 6. Clean the glass doors in the hallways, classrooms, and cafeteria. 5. Make certain that 100% of your products are properly diluted and are in containers that are marked correctly. (Remember that this is scored as either a 1 or a 4 item and if either isn’t done this is an OSHA violation) 4. Dust or use the back pack to vacuum all ceiling vents, returns, and diffusers throughout your facility. 3. Wipe the base of every toilet and the underside of every urinal. (Not only impacts the cleanliness but helps combat odor as well) 2. Clean the restroom floors completely including behind the toilets. 1. Make sure the floors (carpet and tile) are being addressed on a nightly basis and are free of loose debris. (Please don’t “trash and dash” ignoring your floors) Top 10 Things You Can Do To Improve Your Inspection Score

10. Properly complete and turn in your IPM reports monthly . (Checking “no traps necessary” or turning in several months worth of reports at one time is not acceptable)

9. Make sure all of your custodial closets have a copy of the section assignment map and an emergency contact list.

8. Clean out the salt spreader after each use . (This not only prepares it for the next use but eliminates/reduces corrosion and breakage)

7. Make sure your gym floor has been completely dry mopped (including the corners) and no spills or gum are present.

6. Clean all glass on the doors throughout your facility. (hallway, classroom, cafeteria, etc.)
