Entry Exit – Adding Household Members Adding a new member to the family – if a child is born You first add them on the household tab Then add them on the Entry/Exit tab

Entry Exit – Adding Household Members Click Entry/Exit tab Click the pencil next to the entry date When pop-up comes up click “Include Additional Household Members”

Entry Exit – Adding Household Members Click additional household member that you want added. Click Continue Then Save & Continue

Entry Exit – Adding Household Members When adding a baby to a household their entry date can not be the same as their date of birth, use a couple days after. When adding a new household member at a later date you need to change the date on the entry/exit assessment. Click the pencil next to the date you want to change – use the date the client entered the household. Uncheck the household members that entry date does not need to be updated.

Entry Exit – Adding Household Members Click on the new household member and update their Entry Assessment

Removing a Client from Household Search the client’s name or ID # that you are wanting to removing Click on the pencil next to the client’s name

Removing a Client from Household Select the Entry/Exit tab, click on the pencil next to the “Exit Date” for your program

Removing a Client from Household Make sure that only the client(s) that is leaving the program is checked. Enter Exit Date Answer the “Reason for Leaving” and “Destination”. Then click “Save & Continue”.

Removing a Client from Household Click on the red minus sign to the left of the client that is leaving the household. A pop-up window will open. Enter the date that the client left the household. You do not have to be in backdate mode to enter a date. Once the date has been entered click “save”.

Removing a Client from Household Clicking on the right arrow next to Previous Household Members, you can see the former members of the household. To add the client back in the Household you can click the curved arrow

Interim Updates Used to update data for a client Used for Annual Assessments – required with ESG and CoC projects Any project that uses Entry/Exits can use Interim Updates To record Interim Update for a particular period in time use Backdate Mode Used to show increase in income for clients which is reported on System Performance Measure reports to HUD

Interim Reviews Interim Review in ServicePoint It may be required that you update information throughout a client’s participation in a program (HUD Programs, VA, Etc.) Required Annual Assessments for clients participating in a program for more than 365 days (HUD Programs) Annual Assessments should be completed during a 60 day window from the “Project Start Date” (30 days before and 30 days after). Annual Assessments includes updating for both the Head of Household and other family members at the same time. To create Interim reviews: Go to Client’s record Click the Entry/Exit tab Click the “File” icon with Interims for that Entry

Interim Reviews Interim Reviews pop-up will display Click Add Interim Review Select all household members that should be included in the Interim Review Select the Interim Review Type, then click Save & Continue. A new pop-up will display.

Interim Reviews Complete Assessment as necessary If no changes need to be made, scroll down, then click Save. Click on each household member, then click save. Each will display a green check mark once saved. Click Save & Exit at this time! An Interim will be automatically recorded as soon as you Save! For changes see next slides.

Interim Reviews Special Note on Interim Reviews: Recommended Workflow for Updating Income, Non-Cash Benefits, Health Insurance, and Disabilities with HUD Verification If Sub-Assessment answers already exists, end the date for the ones that are no longer true by clicking on the pencil in the sub-assessment (remembering to end date using the prior day). If there are no answers or everything is true, go to the next step. As end dates are entered, the HUD verification icon could change from green to the red triangle. Click on the HUD Verification link which brings up a pop-up window showing the active answers for that date for each type. If an answer in incomplete, click on the yes or no as appropriate. Clicking No automatically fills in the response and a start date. Clicking Yes brings up a pop-up with the Yes response and Start Date but also any other HUD questions that need to be filled out.

Interim Reviews To make a change on income, change the overall question to “No” You will then need to end the previous income for the day prior the new income starting

Exit Client Before creating an Exit for your client, ensure that all Service Transactions, Measurements, and Case Plans are completed and Closed. End Case Manger relationship by adding an End Date. Go to the Entry/Exit tab within the Client Record, then click the Pencil to the left of the Exit Date. Select all household members to be Exited (in most case all!). Complete all fields as necessary Click Save & Continue

Exit Client Complete Exit Assessment information for all clients in the household. Green check marks will appear when each client is saved. Click Save & Exit when finished.
