invoices must
have:

One invoice per client
Dates
of service
• DVR authorization number
• Invoice total
• Itemized list of fees according to authorization
• Name of the provider
• Provider’s invoice number
• Provider mailing addressSlide8
Best Practices: Invoices
Invoices are due to the
originating DVR office by mail, fax, or encrypted email as follows:
• Within 15 days of completion of a service or receipt of a good/service, or,
• The 15th day of the following month for ongoing services (e.g. job preparation and development services
)
Invoices must be received within
45 days
of the last date of service. Check with your local office to identify bill payers and/or the point person for billing

Make sure to have the actual placement provider and/or job coach come to the meet and
greet (job placement agreement).
No service
can be
provided with out an
authorization—DVR cannot pay for unauthorized services.
Keep
track of your authorizations and make sure they are up to
date:
Remind
counselor if you need a new
authorization.
Don’t go
over the hours
authorized.
Always put an authorization number on
invoices—the only
way VR can
