What are the individuals preferences?Consider the need for an Indwelling Urinary Catheter. If there is no indication make that Catheter disappear! If no longer a rationale for an indwelling urinary catheter consider a Trial Without Catheter (TWOC) Remove the catheter, follow TWOC as per your local guidelines. If your patient fails a TWOC, consider teaching them or a carer Intermittent Self –Catheterisation (ISC). An example Urinary Catheter Care Plan, includes HOUDINI. Hospitals use pages 1-3, Community uses page’s 1 & 3 only Care_plan.pdf (england.nhs.uk)EscalationDoes the Urinary Catheter needs to remain? If Yes:Does your patient have a personalised Urinary Catheter Care Plan and Escalation plan? Consider using the Catheter Passport Does the Urinary Catheter Care Plan need reviewing? Is the patient and/or their carer aware where this is located?Use your local catheter passport or local organisation’s tools. What other specialist services can you liaise with? For example: Specialist Urology Nurses or Specialist Continence team where you can discuss any concerns or complications to enable the patient to stay at home and avoid hospital admission? Keep ReviewingHave you scheduled a date to continually review your patient and their Urinary Catheter? Think Catheter, CHECK!
