Interventions:

1a. Provide sleep hygiene information.

1b. Encourage the patient to participate in the sleep enhancement psycho-educational group.

2b. Encourage patient to raise questions/concerns related to medication

2c. Assist patient with the identification of possible barriers to taking medication as prescribed.

As evidenced by

patient’s self-report of feeling well-rested in the morning and congruent with the report of activity from night rounds.

As evidenced by

the patient verbally describing his/her self-administration of medications as prescribed or in the event there are variations, he/she will initiate notification of barriers, challenges or questions related to possible side effects.Slide7
