Referral Process
Upon selecting the appropriate Program and Provider for the client, the officer can simply create the referral to that Provider, upload documentation (Assessments, Case Plan, Authorization/Consent Forms, etc.) and include comments. Completed Referrals are then sent to the selected Program Provider where a

will appear in that Provider’s version of the

Program Providers are granted access to the
Provider Exchange
to simplify the Referral, Enrollment and Attendance Tracking process. The Provider Exchange is provided to Program Providers at no cost.Slide16

Enrollments
Program Providers can view a summary of all Enrolled clients in all programs that Provider offers to the Probation agency. The list of Referral will include Client Name, Enrolled Program, Referring Agency and Attendance overview. Slide17

Program Providers can view a summary of all Terminated clients from all programs that Provider offers to the Probation agency. The list of Terminations will include Client Name, Enrolled Program, Referring Agency, Enrolled Dates and Attendance overview. Slide18

Program Providers can easily record attendance by individual or entire session/date for all enrolled clients. Defaults can be set for Status (Attended or Absent), Participation Level (Low Moderate, High) and Hours/Dosage to further simplify recording attendance. Comments can also be recorded for each customer .Slide19
