Benefit Statement (in case of Luxmed, Mybenefit)– mandatory document (without it benefits will not be ordered) Luxmed: In case of Luxmed with benefit statement you need to deliver additional document: Declaration of joining If you want to have Luxmed medical package active from the 1st day of employment - you must send a scan of the Luxmed declaration of joining with your hand signature till 20th day before the planned start date. Activation of Best help for NJs is available from the 1st day of the following month (after employment), but you must deliver scan of the Declaration of Joining – Best Help with your hand signature till 20th day after employment. Documents – Activation Private Hospital Insurance from Lux Med for NJs is available from the 1st day of the following month (after employment), but you must deliver hard copy of Benefit Statement with your hand signature till 20th day after employment. In addition, around the 20th of the month, Lux Med will contact you directly through your business e-mail address with a request to complete the e-declaration of joining. Failure to complete the e-declaration of joining by the deadline will result in not joining the Luxmed Hospital Insurance. Mybenefit: Activation of Mybenefit (multisport card/multicafeteria/multilife) for NJs is available from the 1st day of the following month (after employment), but you must send a scan of the benefit statement with your hand signature till 15th day after employment. Documents April 14, 2023 E-declaration of joining and consent for deduction* (in case of UNUM life insurance) If you want to have UNUM life insurance active from the 1st day of employment, you must fill e-declaration of joining to insurance available at the link: idz.do/Luxoft send
