Yellow Food Fortification Leaflet Please use this in conjunction with Blackpool South PCN ONS for Adults Prescribing Guidelines Agree Goals of Nutritional Intervention
Improve weight to appropriate BMI (Against GLIM criterion if aged 70 or over)
Prevent Further weight Loss (e.g., where BMI already high, but weight loss has triggered a MUST Score)
Improve Functional Status e.g., strength, gait or balance improvement.
Optimise nutrient intake, especially during acute or acute on chronic illness.
Support healing of wounds / pressure ulcers / deep tissue injuries Is Sarcopenia present? Provide High Protein Food Fortification Advice
High Protein Foods and Meals Yes MUST 2+ or GLIM Severe No Yes Yes No MONITOR Deprescribe (including Food Fortification and High Protein Advice) Address potential causes of malnutrition to prevent deterioration.
