Function of a health system concerned with the mobilization, accumulation and distribution of money (national to local) to cover the health needs of the people. One of the six building blocks of a health system. It has three inter-related functions; revenue collection, pooling and purchasing /provision of services. Source: (Barroy, Sparkes, Dale, & Mathonnat, 2018) Health System Building Blocks Health Financing Health Workforce Health Information Systems Health Service Delivery Medical products, vaccines and technologies Leadership and Governance Objectives of Health Financing Significance Objectives cannot be achieved without some form of pre-payment and subsequent pooling of collected revenues. To raise sufficient funds To provide financial risk protection to the population Efficiency in resource utilization i.e. reduce wastage Health Financing Understanding health financing can help answer questions: Are the resources available for the health sector adequate to meet its key functions? Are resource mobilization mechanisms progressive? (Rich paying more than the poor) Is the distribution of resources equitable and progressive? (Poorer populations benefiting more from public financing than wealthier populations) Are resources distributed according to need i.e. equity? Do provider payments reward efficiency? Quality? Health Financing - Revenue Raising Involves raising money – domestic and external sources Sources: Households Government Donors (loans and grants) Significance Available resources for health often insufficient to achieve UHC in most LMICs User fees are regressive, limit access, do not provide financial risk protection – catastrophic expenditures and impoverishment External funding can be unreliable; more can be done to raise funds or raise them more efficiently domestically Health financing policy must address the question of how to raise funds equitably/progressively, ensure access and provide financial risk protection Health Financing – Revenue Raising Revenue Raising Health systems in low and middle-income countries that have a large and strong public sectors, substantially funded through tax revenue, are most equitable Demonstrated in Hong-Kong; Malaysia, Sri Lanka and Thailand-all have 50% or more of total health care expenditure funded from the government Health Financing - Revenue Raising Advocacy Opportunity for Increased Government Funding for KP and AGYW Programs The significant reliance on donors for financing and implementation of HIV, KP and AGYW programs compromises the sustainability of the interventions (McIntyre, 2018).
