slide1. Incident Title: Beckton STW Upgrade – EIW Undercroft (Pigeon alley) (SpheraCloud Reference: 22433997) Learning Bulletin Why did it really happen? What Happened? Electricians were engaged in 440V power cable replacement (total 200m), The objective was to replace a cable that had now been deemed too short.
The electricians traced the cable from the field end down the riser CMS and onto the ladder rack, where they encountered a section that required them to climb over. After climbing over the ladder rack, they mistakenly identified another cable as the target cable and began pulling it, with one operative at each end.
Believing it to be the correct cable, they decided to cut it in place for easier removal. However, upon cutting the cable with an insulated ratchet cutter, a small spark occurred.
A full isolation procedure was then implemented to ensure safety during further investigations and repairs. The team were working to a cable installation method statement and risk assessment. Did not include removal or cutting of cabling.
Operative incorrectly identified and cut the incorrect cable (live).
The task safe system of work was not clear and had no mention of cable removal and therefore no detail on how to trace, track or remove a cable.
Supervisor did not recognize the change in work dynamics What can I do differently? Detailed DAB's briefing with clearly identified hazards must be provided to operatives.
