Care NavigationPathfields is a Medical Group covering multiple sites across Plymouth. This gives our patients choice: choice over where they visit and who they see.As a Practice we have come to understand that an appointment is not always necessary and actually may lead to delays in treatment. Often patients may be able to self-care or direct themselves to the most appropriate healthcare professional. We hope that this leaflet will help to empower our patients to be able to do this effectively.
