Slide2Leadership is helping ordinary people do exceptional work
Slide3Case Study 1
There is a new Family
Medicine doctor
coming to work at a District Hospital. The hospital has only one doctor. After 6
months the other doctor moved
away so the Family Medicine doctor is alone. He
was asked by MOH to become the Acting Director.
Many
staff members came to
him
to complain about problems.
Slide4The staff made complaints about many different things:
They
were not paid regularly
The hospital
did not have basic supplies and
medicine to care for patients
Some of the other staff were often late or absent
from work
There
was enough money for the Director to take his car to the city, but not enough money to buy hospital supplies
Slide5How should the new Acting Director respond to these complaints?
He
should tell them to stop complaining
He
should let the Director of Public Health do something because the problems are too
big
He
should to listen to everyone and ask for suggestions about how things can be
improved
He
should make a list of the most important problems, then meet with the staff and get their comments and ideas about how to improve things
Slide6Will the staff expect
the
Acting Director
to solve most of the problems?
How
should
the Acting
Director
prioritize
the problems to be worked on?
Slide7From the list of important problems, should th
e Acting Director
Try to solve most of them?
Choose a few to improve first, then try to solve more problems one at a time?
Slide8If the Acting Director was able to make a list of the most important problems with the staff, whom should he go to for help?
Hospital
Director in the next District
Director of the Provincial Hospital
Director of Public Health
in the province
Minister of Health for Lao PDR
Slide9Case Study 2
A
Family Medicine doctor
works at a District
Hospital.
She graduated from the UHS FM training
3
years
ago. MOH
appointed her to a quota job 2 years ago
.
Her
home was in
Champasack
Province and she did not know the staff well at the District Hospital before she arrived.
Slide10When she arrived she learned that…
There
were not many
patients coming to the hospital.
The
patients who came
were
usually very sick
.
Some
of the doctors were
often
absent.
Patients
were never sure whether they would get complete and careful assessment
.
Even
the hospital staff did not
bring
their families to the hospital for care.
Slide11The doctor is a little discouraged
She
mentioned this to the Hospital Director, but she was very careful because he is an older man and did not seem interested.
Slide12Make a list of the types of staff members at a District Hospital.
Which
staff will have the biggest impact on patient care?
Which
staff members have no impact on patient care?
Slide13Should
the doctor
begin working to improve the effectiveness of the hospital
?
What
methods could
she
use to do this?
Slide14If possible, the doctor
would like to
look over
the Hospital’s patient
records for the
last 6
months. What
information can she get from this?
The problems
the patients
came to hospital for?
The final diagnosis?
The treatment that the patient received?
The follow up that the patient received?
Slide15The doctor thinks of making a survey
for
patients and families to get information about their concerns
Who
should
approve making the survey?
What questions should the survey ask?
How will
the doctor
and
the staff know if problems are being improved?
Can
they demonstrate improvement with
this information?
Slide16The doctor was able to summarize the hospital data and found that many things could be improved. To whom should she present the information first?
To
other staff who agree with her that there are many problems with patient care
To
the Provincial Health Officer
To
the Hospital Director
Not show it to anyone and first try to improve the care for patients whom she sees and keep good records to show if the care can be improved?
